I'm sure some one has pointed this out to you but if not the special ability of Vordt's Great Hammer that you are using is hyper poise and armor for a few seconds use it to face tank enemies when they attack and then counter as there attacks will not send you prone due to the poise buff very useful against most npc . Drop the shield while using great weapons so you can use beater armor.
