So he uses the open-top Sara Lee buns. And then he toasts the top of the bun is that correct? I know I always try to grill my buns but I have the regular open face buns I guess so. And I just put it face down on there Grill the inside. But I guess he just grills the top?

Yes a shelf or board would be nice. However, the way I see it is this man is doing with what he gots. He doesen't seem to make excuses for not utilizing his cart and making money for his family.
