Thats all good and fun. But on the Blender 2.9 that I am running, shift click doesnt do a damn thing and I followed step by step. I had to take my cube and delete all the vertices but one because your method does not seem to exist on my computer. When I mirror the vertex, and check clipping and add the cage thing, when I extrude my vertex, its locked along the Z Axis. No matter what I do it wont move on freely. I didnt push E X Or E Y Or E Z. So that rules that out.

If you want to get rid of the triangle at kzbin.info/www/bejne/nIK9aId7mauXg7c all you need to do is add a corner to the mouth, this doesn't seem to do any damage to the edge flow just redirects it a bit, but not in a really bad way, if you like it as is, there's no problems with tris, I'm just sort of used to having a corner in the mouth, helps a bit with rigging and animation at later stages.
