The biggest issue I see with Uvalde was a failure of the command structure. The blame lies solely with the person at the command position at that scene, since everyone else was relying on information/orders provided by the commander at that scene. When the Bor-Tac agents were alerted that it wasn't just a barricaded subject, they readied up and went in to eliminate the threat. You can guess that many other officers weren't told the same information. On top of this, the radios the Uvalde police use could not function inside the school buildings due to frequency issues. The only radios that worked, were Bor-Tac's. Uvalde was essentially a perfect storm of terrible things happening, so many things went wrong. School doors weren't functioning the way you'd think they should (classroom doors cannot be locked from inside the classroom, the exterior doors didn't actually lock the outside handles, whether it's because someone intentionally changed the setting or it was never set properly from the start is up for debate) I just hope people actually learn from incident and actually make meaningful changes to fix these issues that I assume are prevalent in many other schools across the state (if not the nation).
