BPD is different in males and females and is easily one of the most misunderstood disorders. There’s a host or reasons for this and in this video, I’ll confront some of the most confusing issues and then present three easily implemented strategies to help you work more efficiently and effectively with your clients with BPD.
Daniel J. Fox, Ph.D., is a licensed psychologist in Texas, international speaker, and a multi-award winning author. He has been specializing in the treatment and assessment of individuals with personality disorders for over 15 years in the state and federal prison system, universities, and in private practice. His specialty areas include personality disorders, ethics, burnout prevention, and emotional intelligence.
He has published several articles in these areas and is the author of:

Years ago I was inpatient at a well known and respected psych hospital. I was told that despite having every single marker for BPD I couldn’t have it because, and I quote, MEN DON’T GET BPD! It took years before I could get a proper diagnosis and treatment.

@Brandy Baker This is where the DSM is very bad and the ICD, the system used in most countries outside America, is much better. The ICD has scrapped all of the old personality disorders and replaced them with a dimensional, spectrum type model that records traits and their severity. This allows for a much more holistic approach that avoids the problems the DSM has: trying to shoehorn traits into arbitrary personality disorders, many of which have huge overlap anyway, and have massive stereotypes and stigma attached to them. Under the ICD system you'd be diagnosed as having 'Personality Disorder' with a list detailing all of the various dysfunctional traits you displayed, and their severity. It completely avoids things like 'You can't have BPD. You're a man and only women get it.'

New to this. 49 and just diagnosed with BPD. Feels like the last puzzle piece of my life has been found. I'm so grateful for your work, as so many people are stigmatised for having BDP. However I also feel that if I had been diagnosed earlier my life and the lives of others around me would have been totally different. I'm doing my MA in sociology and just want people to see that people with BDP are not dangerous but actually intelligent, kind (in an unkind world) and full of love. I cant help but to feel that the world could benefit from more love, kindness and empathy which people with BDP offer.

there's so many myths about us, and many of them hurtful. I think they get us mixed up with Narcissists and Socio / Psychopaths. I got my diagnosis at 46, though my first symptoms appeared before 10 years of age, so lots of time not understanding myself, and not being understood. Lots of wasted time, but also a wealth of experience. Best of luck to you in managing it! Get some DBT, it's life changing! (I'm male btw, so perhaps that factored into the late diagnosis)

I absolutely love the term "activation event". I've recently started realizing more and learning that once the activation event happens, it doesn't matter how far I shove it down or push it away or tell myself to not bring it up. No matter what, it's coming out one way or another. But the longer I sit on it and try to resist making a confrontation, the bigger the eventual confrontation is.

I would be interested in hearing more about nonbinary and transgender identification and BPD. Anecdotally I've heard that often clinicians may link those genders with BPD's symptom of unstable sense of self.
@le_th_
@le_th_ 2 жыл бұрын
I've wondered that myself many times, and especially since there was a time in the US (back in the 80s-90s) where if someone wanted to undergo surgery, they had to live, work, and financially support themselves as their desired gender for 2 years before they could be approved to have the surgery. I thought that may have been what was needed to make certain that someone with a highly unstable sense of self didn't slip through the cracks and be given a surgery they might later wish they'd never undergone. Now, when I look back at people like Caitlyn Jenner, back when she was an Olympic decathlon champion (which is admittedly impressive because it's mastery of 10 events at the Olympic level), and then tried to become a Hollywood actor, I honestly couldn't stand Jenner back then because she came across as hollow, fake, needing attention and narcissistic or psychopathic, to me. I find that I dislike Caitlyn just as much today as I disliked Bruce, because the "person" and "moral character" didn't change, just some physical, surgical changes. Jenner was an ass back then, and is just as much of an empathy-deficient asshole now. That has never changed, and she's still just as greed-driven, attention-seeking, and unapologetic as she ever was, and those aren't traits I associate with the people I've known through the decades as being transgender. Caitlyn has remained unapologetic for how her she completely deceived her wife, children, and step children, one of whom was very traumatized catching her cross-dressing in the family home when she was a teenager. Her wife took her from bankruptcy from serious overspending to wealthy, and she's never acknowledged her for that. I don't even like her now exwife, but you don't do that to your wife and step-children. You don't deceive and traumatize vulnerable kids like that. It makes them question everything they know, and it messes up their trust in parental figures, and it's f's up their love map so their relationships will never be quite right for most of their adult lives. That's NOT okay. Actually, I think Caitlyn Jenner has done more harm to the Transgender community than she has shone a positive light on it, sadly, and that's unfortunate. Do I think she gives a damn? Nope, as long as she gets paid, and gets to remain in the public eye enough that she can continue to get paid. Jenner has really made me question my assumption that some (not all) transgender individuals might be high in BPD traits, because if she's not a narcissist, she's gotta be close to psychopath. People with BPD have kindness within them, and I've just never seen that trait in Jenner, not 40 years ago and not today. So, it will be interesting to see some research on this.

As a guy with BPD, I know that I'm a guy. Unstable sense of self is more like being confident in yourself to accomplish a goal that you're deadset on one moment, but then all of sudden start doubting yourself completely and losing all interest in that goal. You should look up histrionic personality disorder. You'll see which one lines up more with Trans/non binary.

If you haven't already, I would love to hear you talk about gender differences in the diagnosis of BPD/ADHD. Many of my female friends and I have been misdiagnosed with BPD which turned out to be ADHD (of course both can be the case sometimes). So many women I know are diagnosed with BPD that it almost feels like a modern version of the old diagnosis of hysteria. I know you focus on personality disorders but if it's up your alley, I'd also love to hear your thoughts on rejection sensitive dysphoria (RDS) as it's not in the DSM but many ADHDers identify with it and I believe it's one of the causes of misdiagnosis of BPD/ADHD. Thanks for the work you do!

Also, I have to say, this video was unexpectedly validating as a trans dude, worried a bit that I'd match the other side and feel bad. All my BPD traits align to the male side of things and it was also great hearing that it's not actually 1:3 for men to women, but just that the ratio is accurate for those in treatment...which I have been completely avoiding. It's nice to know that it's actually closer to 50/50 overall. Thanks for the video!

Ive worked my way through a dozen or so videos so far... and i get, understand and know that i do all of this! Ive said for years that my problem is that "Im TOO self aware" can that be an issue with BPD? I know im doing and feeling and acting this way when im doing it... but i cant stop it, because im just too mentally drained from AWAYS being on guard, self analyzing, planning, always reminding myself that "im wrong" take a breath, your over reacting... NO this isnt the end of the world! Etc... and then theres the anxiety, panic attacks, depression on top aswell! It just seems like a never ending constant battle becuase im constantly aware of everything im thinking, doing and feeling......

Well I wish I had seen this earlier. We had an explosive episode with my daughter last night and she ended up getting arrested after calling the police herself. This disorder is just awful and I’m at the end of my rope. No one wants to hospitalize her ever but we keep ending up back here. Rage and hate directed at us, and then the remorse (she’s still in jail so I guess that comes later than usual this time.) she’s been diagnosed but now claims she does not have bpd. Did a 5 week outpatient program and the benefits lasted almost 6 weeks. Won’t take meds/can’t find a prescriber. I’m babbling sorry but I’m runt out after a decade plus of this behavior and not sure where to go from here anymore. Her 4 yo son sits here with me now and it breaks my heart I can’t fix this for them.

Subjectively, I've noticed....(your mileage may vary, and so may your character traits) Females w/BPD: overt eating disorders (coupled with excessive exercise), self-medicating with alcohol (more binge drinking than daily drinking), self-harming through risky sex (and, I would argue, the eating disorder(s), excessive exercising, risky sex, and binge-drinking are a form of self-harm, but that's an opinion), marijuana-dependency, and suicidal ideation/threats. Covert serial-cheater and/or overt-poly-amorous. Males w/BPD: covert eating disorders (because they've been told only females have eating disorders), extreme sexual promiscuity coupled with bisexuality and porn-dependency (because this is socially accepted as manly), both alcohol and drug-dependency, highly controlling in romantic relationships to the point of abusiveness. If he's even moderately attractive, he'll likely be a covert serial-cheater with 2nd and 3rd string team who occasionally substitute-in for the starting 5. I've noticed this tendency toward bisexuality in every male or female with BPD I've come across, including my own oldest sister (although she will not admit it). I've also noticed a high degree of sex-addict-like behavior (group sex/orgies, prostitutes, Backpage, Craigslist personals (before the FBI shut both down), Only Fans, BDSM, swingers, and hot-wife type hookups). I'm sure I've left some out, too. I can't keep up with all the outlets. Oh, let's not forget the random hookups at rest-stops, cruising areas, and truck stops. A whole rainbow of possibilities I never had any idea even existed. It has been absolutely *heartbreaking* to discover that people do this stuff mostly because they feel worthless (so they treat themselves like they are worthless). Unless mental health professionals have post-doc education in differential diagnosis and treatment of Cluster B disorders, most of them don't even know this. They're out there conducting marital and relationship therapy without having any idea what a personality disorder is (a character disorder) or what is driving the cheater's obsessive-compulsive-like behavior. The mental health profession needs an over-arching requirement for post-doc education (in those already practicing) on how personality disorders drive the VAST MAJORITY of what they call "addictions" or "dependencies". If you know someone in AA, NA, SAA, an eating-disorder program, substance-abuse treatment program, or anyone with multiple DUIs, DWIs, "process addictions"...it is VERY LIKELY you're dealing with someone with an underlying personality disorder, and especially BPD, NPD and/or AsPD. On occasion, you find someone with OCPD. I think our prisons must be full of people with (often) undiagnosed Cluster B disorders. We have GOT TO FIND A WAY to start helping little ones deal with the trauma and neglect they endure (because their brains are too underdeveloped to cope any other way). As a society, we should be ashamed of ourselves for not doing better by infants, toddlers, and young children. We *MUST* do better.

Not like a narcassist who calculating and manipulating and isn’t often suddenly triggered. But again this explanation doesn’t negate the truth that damaging behavior still hurt people even the borderline. I couldn’t agree more people need tools to overcome the overwhelming feelings that negate facts that shouldn’t be. Fact over feelings, feelings lie.

Dr. D. J. Fox, I’m just getting to watch the video, I also sent a copy to my psychologist so we can go over a few of the topics you brought up that I thought would be beneficial. I do appreciate your understanding of people with BPD. Sometimes, when things are tough it’s hard to find anyone who really understands you when you need it. That, would be why you need to have coping skills and to be able to help yourself, at least to some extent. I feel I am finally getting there and other people are noticing. Thank you for all the help along the way with your videos. Of course, I also will give my psychologist his credit also, because without his help, I wouldn’t be where I am today.

Watching the heard/depp thing, has opened my eyes a bit. I ended a relationship with my partner of 5 years looking back she had lots of BPD traits she had violent outbursts that came out of nowhere or didn't fit the situation. punching / hitting / swinging, she was jealous constantly accusing me of cheating or looking at other women. even causing arguments or scenes with friends / neighbors. she would lie to her parents about me in order to get me to fight with them i no longer talk to talk to them she would tell my parents i was abusive to her, then come home and tell me that they were aggressive to her, to get us to fight i haven't spoken to my parents in a couple of years. she would belittle or talk to my friends behind my back, i no longer talk to anyone i used to. she has called the police on me and make up all sorts. now moving on, she will message women i am talking to, to sabotage my attempts. i have a kid with her. how do i get her to leave me alone ?

Start gathering evidence as best as possible bro. Preferably video or audio evidence of her outbursts at you or the kids ( video record her or audio record here . Unfortunately the legal system always favours the woman because there considered to be to "feminine" or "innocent" to be a antagonist . Long story short you need legal advice from a professional, preferably not another woman and you will need hard evidence to present during caught hearings that prove she is unstable and the kids would be best with you until she can recover. Without real hard evidence All she has to do is go bang her head against the bedroom drawer, give herself a few bruises and then cry wolf that u abused her then that's it. Over. Get some good advice from a male legal professional and try to obtain video or audio of her bad behavior

I think your health authority is admitting that they do more harm than good when they try, due to the stigma of having this diagnosis on file. Also, clinical psychologists are more qualified with this type of disorder because it responds to psychotherapy, not medication; many health services don't include clinical psychology. There's a lot of reform to be done.

Identifying as nonbinary (or somewhere outside the binary) does not erase the significance of sex specific data. Men (males) and women (females) are bound by a number of biological, environmental, physiological and hormonal differences that are necessary to take into account for treatment and observing outcomes. This does not ,however, invalidate anyone's personal understanding of themselves or how they identify. The material reality of biological sex is important. Individually subjective and undefineable concepts of gender are impossible to record. The following statement is an example of how data is presented: "30% of highschool females who identify as nonbinary are diagnosed with BPD as opposed to 5% of highschool males who identify as nonbinary". This information can lead researchers into delving into *why* significantly more highschool girls who identify as nonbinary are diagnosed with BPD, that boys who identify as nonbinary.

@@tmystery9505 that's a lot of words for "im a transphobe" lmao.. I love how you pretend to not be invalidating anybody's gender and then go on to say stuff like "girls who identify as nonbinary" and "boys who identify as nonbinary".. you're not nonbinary and you don't get to theorise about nonbinary people and what "causes" them as if we're labrats and concepts rather than people who are capable of conceptualizing our own experiences in life without without needing researches to psycho analyze us .. yes those environmental and biological factors play a role, a perosn who has more estrogen than testosterone or more testosterone than estrogen (both prenatally and present day) might display different behaviours and a person socialised in a specific way will also behave differently but don't you dare say being nonbinary could be caused by bpd because that is incredibly ableist to reduce somebodies identity to their personality disorder .. im not nonbinary because of my bpd and you don't get to sit on your high horse and make judgemnts like that.. thank you

@@munii3216 could you help me understand you better because how Mystery T phrases themselves didn't sound transphobic to me but maybe I'm missing it. I understood them as saying the focus of the investigation would now be the bpd and non-binary individuals and their experiences and what goes into forming those two states. The two being the subject of investigation as oppose to cis-gendered people because enough research has been done so as to already have a baseline of understanding that we needn't reference what goes into making their experience because weve sort of come to understand them at this point (both wider society and scientifically) - and specifically for the particular researchers question being investigated. To say, i believe cis-gendered people are still investigated and we dont think of them as "lab rats", and so I dont think you should see non-binary people as lab rats because they are trying to be scientifically understood. Isn't that for the better? Also, I dont think they were saying being non-binary is caused by BPD but rather that research would help explore experiences that lead to non-binary experience experiencing a higher percentage of BPD and determine whether it's causal, resultant, or is simply confounding. Though I'm not sure. Maybe you or someone else can clarify it for me?

Been to therapy for 3-4 years up until October 20. Most male borderlines aren't pure and usually have another diagnosis on top of it (ASPD or NPD)....I have BPD but it's mixed with schizoid, narcissistic and avoidant traits and some mixed depressive/anxiety disorder. The group therapies I was in had only one other guy next to me, we were about 15 in the group and I can't say that I could relate to anybody there, so it's more of an umbrella term used for men who are emotionally unstable and don't fit any other diagnosis, the women could relate to each other though. BPD is an exaggeration of female stereotypes, NPD is a gender-netural diagnosis (i.e raise or lower the voice of a narc by an octave and they sound and act entirely alike). The male answer for BPD is ASPD, not NPD and is proportionally as often diagnosed in males as females with BPD with a 3:1 male-to-female ratio.

I just would like to know if there's any study of how many victims of domestic violence (usually women) by their partners or even parents end up in psych wards with a BPD diagnosis. I contacted an association for women victims of domestic violence (I've been a victim of domestic violence from my parents, mainly my mother, since i was born until my early 20s) until I was able to move abroad but my mental health at the light of many events deteriorated and i became suicidal, because of this i was kicked out back to my home country and to my parents home, because I'm jobless, lost majority of my savings and I feel like death would be a better option. Meanwhile, "therapists" have the audacity to ask "but your mother doesn't beat you anymore, right? so your relationship should be better now". I wonder if they ask the same question to victims of sexual abuse "I know u have been raped by this man for 20 years and now u have to live under the same roof but he doesn't rape you anymore right? só why don't you let go of the past and move on in order to have a better relationship?" Anyway, she told me that few times, doctors from psych wards contact her association (she's a forensic psychologist) because they believe they have a case of domestic violence in hands, and women are displaying all these erratic behaviors not only because of abuse but also looking for she.lter in the hospital and for the hospital to give them shelter they have to "diagnose" them with some mental illness, BPD will be.
