Braces Explained: Pain Management
In this video, I will be answer the question of "how to deal with orthodontic pain?" I start off by describing that orthodontic pain can be divided into two categories: 1] dental, which is the pain related to your teeth and 2] soft tissue, which is the pain related to your gums/cheeks
To manage dental pain, I recommend doing everything you can to decrease inflammation. This includes eating soft food and keeping your mouth as cold as possible by eating cold foods. Also, anti-inflammatory medication can be used but they do slow down tooth movement. Tylenol is a great alternative for decreasing the pain and not having any impact on tooth movement! I recommend taking Tylenol or some anti-inflammatory one hour before your adjustments that way by the time we perform the necessary adjustments, your body has already absorbed the medication!
To manage soft tissue pain, which is caused by the braces irritating your gums/cheeks, you can use a number of things. Wax is the most commonly provided item by orthodontist and can be used to make braces smoother and push your cheeks away from your teeth and allow for your cheeks to hurt. Also, you can use a topical anesthetic (link below) to numb up your cheek until the pain goes away. To do this, just dry the area of your mouth and put just a bit of anesthetic in the area until the pain goes away. Lastly, you have got to keep your teeth clean. When your gums get inflamed, they become puffy and more food gets stuck, the brackets become more irritated. This is a vicious cycle so please please please make sure to brush and floss to make sure your teeth stay healthy!

@Jaina Sosa I’m a year into mine and by the fifth day I could eat normal food. A week in you should be fine and after that most appointments like readjustment shouldn’t really bother you. But when you get your elastics it hurts just like the first couple hours after you get them but a bit more.

Evantz Bernadin from my experience braces hurt the first week or so and when you get them tightened, but the results are amazing. And also you have to wear retainers forever, well as long as you want your teeth to be straight. But another thing is, if your teeth aren’t that crooked or messed up, then I wouldn’t get braces. And if you’re comfortable with the way your teeth are and you don’t care what anyone says then don’t get them

I know & that's good that you're thinking that way! I always say "pain is progress" & I know it sounds mean but it means that your teeth are moving =] Just be sure to wear your retainer once you're done with treatment to make sure that it stays straight for the rest of your life!

Well, unfortunately because of corona virus my apponitments were canceled for almost 6 months and the progress gone low and I still have my braces on because there are some little things left to do on my teeth, but my doctor said that I can take them off-at the end of this year-and I really HOPE that covid let me survive this situation lol. The worst is when a wire starts poking you in the cheek and you cant get to the orthodontists to have it clipped. I cant see it to do it myself and it is literally piercing my cheek. I have a hard time getting the wax to stay put. Thankfully I only have another 21 to 33 months of this. Yay.

Stephanie H it is a bit difficult, but can you try putting the end of the wire back to the last bracket that is kinda like a tunnel? My orthodontist encourages me to do that. I believe treatment time will be longer with a misplaced wire.

I know that's super annoying! Usually if the wire is thin enough, you can put the wire back in the tunnel like Rashmee said but in thicker wires, this wouldnt be possible! Best bet is to try and check before leaving your appointment to make sure the wire is short enough =] I know it's super annoying =/

Rashmee and Dr. Greg, the wire hasn't slipped out of the band it's to long and has come out the other side and is stabbing me in the cheek. My ortho said I could clip it but I cant see it as it's in the back of the mouth and the wax will not stay put.

Day 5 with braces. I didn't feel any more severe dental pain after the 2nd day but the gum soreness is SO BAD. It's so hard to speak. I have to keep my lips away from my brackets from time to time to ease the pain. My orthodontist didn't give me any wax to apply for this situation so I just ordered online. Can't wait for it to arrive!

This helps a lot. I've gotten my braces tightened twice now. The first time was incredibly painful and unbearable. The second time I went in I took a Advil 30 minutes before the appointment and it didn't hurt half as much as last time. Thanks!

I use swimmers silicone ear plugs. Super cheap and works so much better! Definitely worth a try.

You know what works really well? Try to dry the area around your braces before putting the wax on and make sure you get it all around the braces & wire. That helps it really hold on =] Give it a shot & let me know how it goes! Ear plugs work really well too!

Dr. Greg Asatrian I wonder if you could talk about orthodontic buttons I recently got this and had never heard of it before, I have to wear elastics in a square shape to try widening my arch and side teeth, there must be similar people who also have this. I can definitely make a video on buttons but I think your question is more about elastic configuration! Usually what I recommend my patients do is try putting the elastics on IN OFFICE and I double check them before they leave - Maybe ask your ortho next time to check the way you put them on so you know you're doing it properly =] Also take a picture on your phone before ou leave so you continue to do it the right way!

Confusing, but I just moved and my new orthodontist doesn’t supply my old orthodontist’s brackets but were willing to work with my brackets, I have Damon Braces. I got some brackets put on and repositioned. Now I have two different types of brackets on my teeth. Since most of my brackets are Damon braces while 4 brackets are traditional braces, and on my brother I can see the difference since 2 of his front teeth are Damon and the other are traditional. Any who, since I never really got my wire changed since I had Damon braces and maybe different orthodontist replace wires at different times, and now I need to get the wire changed every time because you need to do that to change the ties or colors on the traditional braces, and the braces hurt so much, thanks for the tips! ❤️

That's so interesting! I'm curious, did you feel more pain with the damon braces or traditional braces or do they feel about the same? Damon claims to be less painful so I'd love to hear your opinion after having both!

Dr. Greg Asatrian As of now, I would say the traditional will probably give less discomfort because the traditional are more rounded. With the Damon Braces they are basically a cube/square. They are rough on the bottom and would probably cut your cheek more. The pressure is probably the same.

Agree, I have Damon braces at the age of 27 and although they are smaller they are a lot bulkier than the regular ones. My brother had the regular ones and they are a lot flatter so he had no discomfort whatsoever!

I'm almost 6 months in with braces and so far so good. I haven't had to do a liquid diet. That said I am very mindful of my food. Bread can be tricky because it still requires you to chew it. Think to eat foods that you can more or less swallow without chewing much. I recently started using my mini food processor to grind my cooked meats. Curried chicken grinded taste just the same as curried chicken pieces but soo much easier to eat. And if you have to do smoothies ensure you bulk it up with added cooked oatmeal or protein powder to give it substance so you don't feel hungry so quick.

I've been wearing braces for 2 months now. I only had pain the first 2 weeks, but it wasn't so hard to deal with. But the bite turbos... they are the worst! Couldn't eat anything except for smoothies and mashed food. Unrelated question: Do palate expanders work for adults? I wanted to expand my palate a bit but my ortho told me that it wouldn't work for me as I'm too old. I'm 23 btw. Thank you!

Hi George - I actually just made a bite turbo video so stay clear if you don't want to be traumatized from the memory of it =P Sounds like your braces journey is well along its way! Onto the unrelated question of expanders... Expansion in adults is a lot more "dental" than "skeletal" - I have a video on expansion, but if you want to achieve a skeletal expansion, you either need to undergo surgery or use mini-screws-assisted expanders .. Hope that helps answer your questions!

Gram.gám3s Vaughan you have to make sure the bracket is dry (which is hard bc they are in your mouth with saliva) but try to dry it off before applying the wax and then make sure you are using enough wax to completely cover the bracket and then tuck it underneath the bracket to form a pocket around the bracket. Hope that makes sense!

My ultimate tip to help ulcer pain when I don't have wax: After I notice the area being cut and uncomfortable I pour salt on it in the moment it burns... But after I swish with water it's like it instantly heals it and closes up and bam the next day I forget about it like it's gone.

Generally it takes a few days for the discomfort to go away but you should still try to eat soft foods like mashed potatoes & soup!

Dr Greg! “Gishy Goo!” is much better than wax and silicone ear plugs (like Mack’s) are much better than orthodontic wax. However, you may be limited as to what you can actually offer at UCLA.

yesterday I got my braces on my lower teeth, and rubber bands. popsicles really helped! and soft foods too, such as yogurt, soft carrots. I also put in my rubber bands yesterday. but got so much pain because of it. so I decided to only put in my rubber bands when I have less pain. I have to wear my rubber bands day and night. also with food etc. quite a long time.
