This is actually Allison's mother. I am 60 years old. I was diagnosed in 1995 with Stage 2A Breast cancer with 2 lymph nodes positive. Infiltrating ductal carcinoma estrogen positive in my left breast. I had an MRM. In 2011 I was diagnosed with Stage 4 breast cancer in my right breast, infiltrating ductal carcinoma, her2 positive. Metastisis to my lungs. I have been on herceptin during my entire treatment. After approx first 18 months, I started on chemo, Xeloda. It did not work. I was then put on Abraxene and herceptin. In December 2016, I stopped chemo and herceptin. And as of this past week and scans with no change I am officially considered to be in remission! How long I will be inremission is unknown, But I will be thankful for as long as it lasts.

Allison Shrum *I hope that everything is going to be good for you in the future, breast cancer took my mom when she was 62 years old after a heroic battle, so I know how tough it is. BIG hugs! :) 💜*

Great lecture, as always. One correction only. Treatment for LCIS is not the same as DCIS. In DCIS the lesion should be excised followed by consideration of radiation and Tamoxifen. In LCIS excision may be done for diagnostic purposes after biopsy (this is controversial) but observation with risk reduction management (medication, surgery) is generally considered. This is because if you try to remove LCIS you will likely not be successful since there will be many other lesions in the breast. That is why it is a risk factor for breast cancer (ductal or lobular) in either breast, not only the one you found the LCIS in. DCIS on the other hand can and should be completely removed, as the lesion itself may progress to cancer. You certainly would not do a lumpectomy with radiation for LCIS. I don't know if such a thing would be tested in the USMLE exams but if they do this would be a major difference between DCIS and LCIS.

So, to summarize: DCIS = lumpectomy +/- radio & tamoxifen OR MRM? LCIS = watch & wait with meds (what kind of meds?) and then MRM if needed?

I've read that for Inflammatory Breast Cancer surgery is part of treatment, but is placed differently in the treatment algorithm. 1st chemo then if tumor shrinks surgery can be tried but with no conservation of tissue. Then follow with radiation.

Another great video, this will be my third set of exams revising from your videos so thank you!! It is interesting that African American women are more likely to die from BrCa than Caucasian women, however I'm not sure this is due to the aggression of the tumour. There is evidence that Oncologists are less likely to treat black women with a post-lumpectomy radiation than they are white women, and black women are also much less likely to conduct breast self-examination than white women. There are also access to healthcare issues such as time off work, transport, and over in the US- cost, that contribute to disparate breast cancer survival between ethnicities.

This is exactly what i was thinking. And I don't think it's nice in any way to say that" sadly white women are more likely to get breast CA" its sad that any woman should get it no matter what race you are.

It’s almost certainly health care disparity, availability and cultural doubt just like with COVID vaccine (think Tuskegee) My Asian friend died from triple negative 6-7 years ago. The treatment options sucked and she was a physician from a family of physicians, so she had access to care. But guess who gets triple negative? Not white women.

She had a mastectomy of the left breast 10 years ago and has chronic lymph oedema of the left arm. What is the most likely cause of the malignancy? Lymphangiosarcoma Lymphoma Myeloma Angiomyolipoma Giant cell tumour

Actually, this sounds like a Stewart-Treves Syndrome. A rare, cutaneous angiosarcoma from long standing lymphedema. I studied this under Surgical-Dermatology.

+Hanah Detalla you are right but the most commonest cause for malignancy wouldn't be a lymphangiosarcoma; it's still the lymphoma. And yes long standing cases causes ST syndrome but then the propensity is less nowadays due to more aggressive adjuvent chemoradiotheraphy following MRM😊

+Hanah Detalla And post op arm exercises and chest physiotherapy with incentive spirometry most commonly advised nowerdays.So it's less likely to see unless we address patients with poor compliance towards treatment following surgery 😊

I agree. ST Syndrome is a rare case, and post op rehab avoids that. However, I feel like if this was an exam and the data presented above was only what the student had, I still feel like ST would be the best answer. However, had the question been constructed with an additional :"Patient reports of compliance to post op arm exercises and CPT", lymphoma would be the better answer. =) But yey, thank you for the learning!

Something my attending once told me is that the reason why nulliparity is a "risk factor" is because women who have children breastfeed- they're more likely to "check their breast" compare to women who don't have children. I don't know if there's any study that supports that statement, but--- it kinda makes sense.

women who are nulliparous tends to have more ovulatory cycles in their lifetime hence more expose to Estrogen.More pregnancy equals more anovulation = less Estrogen exposure
