Hi Dr. Brent. I am a DPT student! It was a great explanation! There is only one thing I would like to add that I learned from my professor in DPT school. You are more likely to get a cavitation if you were to log roll her such that her femur (the one that is hooked) is facing directly perpendicular against the floor, and your greater trochanter is in line with her greater trochanter (which I think it was). Of course you have to lock her from both upper and lower side which you did perfectly. Not to say that cavitation is the goal. Even normal lumbar mobilzation/rotation has a great effect, but I am sure you will agree that cavitation always has a good psychological affect on patient.

Hi Brent, I am currently suffering from pain in my upper right leg, from the inner thigh right around to the buttock area. Trying to cross my right leg over my left leg is painful. Would you say that this is a case of hip replacement or could it be manipulated to correct the situation? The fact that I experience pain from my inner thigh right around to the buttock area make me question the fact of hip replacement. I am 70 years old but otherwise very healthy. Your assessment will be appreciated.

What are your thoughts on contraindications of lumbar HVLA manipulations on herniated disc pathologies. Also I think your patient should be closer to you during the manipulation.

Hey Thierry, The research does not suggest that HVLA is more risky for someone with an HNP, there is some evidence to suggest that it may not be preferential if there is nerve involvement (sciatica), but this is not a statement about risk as much as outcomes. As far as position, without a smaller table, I am not getting closer, unless I no longer want the table to stabilize her top lower extremity. Dr. B2

Great question, I would say any evidence of osteoperosis or fracture would be my first thought, cuada equina signs or active cancer in the area, history of nerve damage might be a contraindication, and I might add that acute irritable pain is another quasi-contraindication. When pain is really high and easily triggered I will often default to mobilizations. This is not a complete list, but a good start ;-) Dr. B2

Good video. I have a serious issue and question. I had this done about 3 yrs ago and when he did this manipulation/ technique (that particular day) , he came down with GREAT force , more than any other time which was crazy and caused much pain,could barely stand on my right leg..indeed I was shocked!! And he had a weird half smile on his face...seems like he did it on purpose..he was my ex-husbands friend..he also retired right after that. Strange eh. So. here I am now, I have alot of issues in that spot..dont know if its L 1,2,3 or 4? Im afraid to go get it done again. What damage/affects could this cause and what do you recommend? Thankyou. I really hope to hear from you.

I cant afford chiropractors , and the one time I went , the lower back adjustment was all I wanted and was the most relieving part of it , I haven't been diagnosed but I believe I have a pinch nerve in my lower back on the side deep inside feeling and what feels like above the pelvis or back of pelvis , anyways would I be able to teach like my woman to be able to do this to me or is it to dangerous if someone hasn't been trained? Cause damb chiropractors want to do all this other crap when theres only one adjustment I needed ended up costing 250 bucks just for one little visit way to expensive but it did relieve my lower back issue for a year or two now ita starting to come back.

I've been considering visiting a chiropractor,but I've been hesitant. I'm from Jamaica,I was told by my doctor that I should consider surgery,at the moment I'm not even sure how much the curve has progressed. Any suggestions?

Some feedback. Your upper arm placement won't work for everyone it can really hurt shoulders and graze breast tissue. It's also easy to over rotate someone like this with that much torque. It also won't work well on acute or sub acute patients.

Brent Brookbush Hmm, I see. Thank you for the quick response. You see, I am addicted to cracking bones in my body, literally anywhere possible. Most of the time, almost everytime, I find a way to crack a bone, one would seem impossible to do it by yourself. I find ways to do it anyway. For my lumbar spine, I basically lie down on my side, as shown in the video, press down on one hip and rotate my upper body the other way, that way I am able to “crack”/pop my lower spine (lumbar).

I always try to be as accurate as possible, because as you know it is easy for individuals to confuse ATCs with personal trainers who like to train athletes when "Athletic Trainer" alone is used as the label. I am not saying I agree with this, just as I don't like personal trainers using "PT" as their credential, since that would be "physical therapist". Point being, the use of the ATC credential is really a way for me to put an extra bit of clarity on which profession I am not talking too. B2
