Yes ofcourse 😊 Facilitating movement recovery during flaccid stage utilizes treatment strategies like Use of Ramistes phenomenon ( resisted contractions of normal side produces reflexive movements on weaker/ paralyzed side). But as the patient progresses upto stage 3 more emphasis is given on breaking synergy and at this moment cross training/ educational is discontinued. My wife is passing through this stage. But her problem is that due to deltoid muscle wastage there is an atrophy. As a result subluxation of GCH is there. Sir, kindly suggest whether she will be able to overcome from atrophy. Regular physiotherapy is going on home since 6 months. Sir, kindly suggest whether any physiotherapy centre is there I.e. your place to treat my wife in such a critical position. I'm totally frustrating. Sir, kindly guide me. I regularly watch your video and it gives me hope and mental strength.

Sir, what is the reason that the synergy pattern have the strong or weak components? And the same patient can develop the three synergy's (initially flexion then extension)as the hemiplegic doesn't get proper treatment ?

Depending on the functions carried by muscles in daily activities, Their representation in the brain can be more or less prominent. For example, the hand being flexor dominant in nature results in the wrist and finger flexion to be more dominant as compared to wrist and finger extensors. Basically it's about how much brain signals are alloted to different muscle groups. The one having more will be dominant and the one having less will be weak. As the patient gains control over these brain signals, he can produce more variety of synergy movements. Patient not getting proper treatment is limited to use synergy patterns based on his post stroke sensory motor experiences.

Yes, We know that flexor synergy predominates in the upper lim and extensor synergy in the lower limb in a hemiplegic patient. I have given the reasoning for this in the next part of Brunnstrom Therapy series. Next video releasing on Friday😊🙏
