If my S1 nerve is not activating my glute medius/maximum, how do you get the nerve to wake up and activate the glutes? I have L5-s1 fusion with no nerve compression and no issues with discs/joints according to imaging. I would greatly appreciate any opinion on the matter.

If no S1 nerve compression then doubt the Gmax activation is an issue with the disc or nerve root as the S1 also supplies eversion of the foot and plantar flexion of the ankle - if these movements are weak then a possibility it is S1 - regards JG

My thigh and glutes are athropy (and uneven glutes) because I'd been immobilize for 2 months after knee surgery. I also have disc issue on L5/S1. Due to weak glutes, i feel more instability on my lower back and numbness on my leg more frequently. After that, I've been trying to streighten and build glutes for almost 5 months, but the glutes muscle still flat and won't grow. What is your opinion? Could it be fixed?

Hard to say without looking at you as there are many causes for the weakened gluteal muscles. The S1 nerve root might be involved as well if you have a disc issue. See a therapists who can assess and advise correctly, regards JG

Hi, I have been suffering with body pain for many years now but the last 2 years have been excruciating, I am a labourer and work long hours and 7 days a week and walk long distances daily, so during the lockdown I've been massaging problem areas on my body and managed to release extremely tight muscles on my neck, back, wings, shoulders but on my hip, behind my knees and on my inner souls of my feet under my ankles the muscles or tissue there has become like hard balls with little soft lumps in them, so I want to work them out slowly but not sure if it could be a nerve problem or just overworked muscle, I had those little soft lumps around other sore muscles even my neck but they have gone since I started massaging. And if the nerve at L5S1 is not supplying the glute, how do you remedy that? I have a herniation at that level. Despite an EMG showing nerve root irritation at L5S1 and radiculopathy , multiple surgeons all said they see no nerve compression on my MRIs. Therefore they say I’m not a candidate for surgery. Yet my glutes, especially the left are basically shut off. No PT has been able to activate them and they are totally atrophied. The left being the worse. If I do clam shells on that side, it starts shaking on number 4. If I go to do the prone leg raise like in your demo, my thigh won’t even come off the table, the hamstring just bends at the knee. Something is wrong I know it. I need help. My body is now compensating in the worst ways and other things are now hurting. Can I get the glutes to work again, if there is in-fact a nerve problem?

I have the same problem, in my case only my right glute down my leg. MRI shows nothing but electromiography shows iritation in L5/S1. Nothing is working (exercice, PT, relax)

My right glute and calf muscle are atrophied, so it sounds like this may be what I have. I think it came from placing my wallet in my right back pocket for years. I found a clip on youtube entitled "L5 S1 disc bulge exercises" and have been doing the exercises prescribed for a few months now and am still atrophied and noticing little if any improvement. He says to move your feet to the right if you have a right side herniation, I'm assuming if my right glute is atrophied, the herniation is on the right side? Also, I have no back pain, just gluteal and calf atrophy, not sure if this matters.

My right glute has been atrophied for 4 years now, I took an ultrasound a while back, confirming I don’t have a herniated disc, but I’ve tried all kinds of rehab/glute activation and so far, nothing has worked in a noticeable way but I’m not giving up!

I’ve had the injury for now 4 and a half years. At first I thought the problem was in the glute max so I looked up all kinds of glute activation rehab that resulted in minimal improvement. After a while, I thought the problem could be in the spine but I had no back pain and an MRI confirmed my spine is fine. A bit before last Christmas, I was feeling pain in the hip near the greater trochanter, did some research on this and found out this pain is usually caused by glute med tendon strains or pulls, so I started doing rehab for this, now after 4.5 years of frustration, the muscle that was lost in my glute is coming back, the problem was the glute med. reason I was so frustrated is I visited some doctors, physiotherapists, chiropractors, did all kinds of research online and nobody could tell me what was wrong, I strongly suspected all along it was probably relatively simple to fix if only I knew exactly what the problem was. As for the atrophy in the right calf, I don’t know if it’s related to the glute med thing, it’s just that when I put on my knee sleeves, the right one always slides up more easily than the left. As a powerlifter, I don’t even do any direct calf work in the gym.

I’ve ran across this video because my left glute is weaker and smaller than my right. Lately I’ve been doing secluded exercises and I feel a tight strain in my left glute/hamstring every time I stand or bend. It’s been almost two weeks and is still slightly painful with a shock sometimes running down my leg.. will going to a chiropractor help or should I see another doctor? It’s frustrating because I want to get back in the gym.

Hi there Since a bad deadlift I have had patella tracking in my right leg and neck and hip pain. I found when I squat my hamstrings on the right side is more dominant whereas my left side my glute is dominant. Do you think it is the S1? Thankyou

Thanks for the comments, If its a disc pathology that is the problem then time will help as normally takes 3 months to settle down. If the nerve is still compressed then you might need surgery but pretty rare. Sometimes an Osteopath or Chiroparctor can change the position of the vertebra and that can help to reduce the pressure to the nerve root. regards, JG

@@JohnGibbons Wow! Thanks for your prompt reply, Doctor. Actually, I'm a national level cyclist, and I've plateaued at this level because I'm riddled by muscular imbalances. I was lead to this line of thought by Mr. Steve Hogg from Australia. He is a bike fitter who believes in making the rider functional first, and setting up the contact points second. So I've had an Osteopath look into the matter, and many physios for over three years now, and they've had me do glute strengthening exercises to their hearts' content with zero results. There's a 'weak' left glute, a hypertonic right psoas, twisted pelvis, tight left ql, and so an ad infinitum which is simply disabling as my body has run out of compensations to cope up with the training load. The most obvious fallout is knee pain due to ITB friction and general discomfort and inefficiency. Coming back to the question, what are the procedures to really pinpoint the cause of inhibition. For example, how does one ensure labral tears, through imaging? And so on for pinched nerves. Thank you for reading this, by the way.

If you watch my video on Hip assessment and get someone to follow the techniques I show on you then you will know if you have a hip issue. A steroid injection into the hip could then reduce your symptoms and then an MR Arthrogram will be the Gold standard test for Labral tears even though an X-Ray will show a cam lesion for an FAI. Typically it is an inhibited Glutei that might cause an over activation of the hamstrings and the triceps surae....and most S1 nerve issues are disc related between L5/S1, regards JG
