Kneeling after a total knee replacement can sometimes be problematic. And it was for us until we came across a fantastic surgeon who showed me the way. Now, many more of our patients can kneel after surgery with this one small change.

I have had both knees done several years ago ,very pleased with the result but definately cannot kneel. Also if you fall as I have twice it is impossible to get up because you are unable to roll over on to your knees.I have spent hours trying to get up as I live alone.I am really glad to hear things have now improved and I hope that will become the norm in all hospitals.

You could try to desensitise it slowly. For some patients, not being able to kneel also won't bother them a great deal.

I've recently had a full right knee replaced. The Doc. told me to kneel asap to get used to it. I can and do. However it is the weirdest sensation as I've never gained full "feelings" in that knee. I started by kneeling in bed, thinking if it hurts, it's a soft landing.

5:00 PM central time I just had double knee surgery, September 18 of 2023 My doctors PA said I can get on my knees and do whatever I want as long as I can handle the pain. She told me do not run unless bear is chasing you because you do not want that hard impact on your knees. My therapist today when I got on my knees for the second time, suggested that I not do that no more. I’ve watched your video games a lot of information and thank you very much for your videos. Have a good day.

I am 84 years old, weigh 220 lbs, and am 6ft,1inch tall. I have had both knees replaced with the incision down the middle of the knee. I can and often kneel on my concrete shop floor, while woodworking , for short periods with little or no discomfort. I am careful to support myself during the kneeling process so as not to plop down on the concrete but a 3 or 4 minute kneeling period is not uncomfortable.

That is absolutely fantastic! We found that about 15-20% of our patients could kneel with the standard, midline incision that was performed previously.

My surgeon was amazing but he used a midline incision on my right full knee replacement. Now, through time and much practice, I can put some light weight on the scar tissue. After thinking about it I wondered why the incision wasn't to one side of the knee. Now I know that it can be! Eventually, I'll need my left knee done, and when I do, I'll request a surgeon who does the side cut. Thank you for this informative video!

Oh my, I didn’t know about not kneeling after knee replacement - - when I had one done ten years ago. I fell a couple of years ago and tried to get up by kneeling, the excruciating pain was so severe that I screamed. I would not get the other knee done because of this, yet it too is so painful that I can not kneel at all now! I always wondered why it is so dreadfully painful to kneel after a replacement - - WOW, I’m glad that I clicked onto this site!!!

Im a 57 yo male and have both TKR 4 years apart, with recent surgery 6 months ago. Like most on this chat, i can kneel on my first TKR with pain, however surfaces like carpet is easier. Not quite there yet with second surgery however with my experience with first surgery its a much easier recovery as from my waking moment in hospital i straightened, flexed and stood 40% weight on it within hours. First TKR like most i assume, was hesitant to move at first and have no doubt recovery was not as quick as i had been told. Second TKR is awesome for 6 months. Every moment i remembered i would rock toes forward and back, raise knee and extend to establish a flat leg. Physio was very pleased that he could not slide his hand under my knee within 7 days. While not as good as natural knees, im pleased with improvement from severe osteoarthritis i endured for far too long. For any one due for surgery in the future, my advice is move as much as you can when you can, with the goal of stretching any scar tissue to oblivion. Currently working with PT at local gym and am able to leg press my weight (125kg) plus 100kg added weight and knees are great. Thanks to all surgeons, you make a hell of a difference for us all.

Yes, I am making good progress on kneeling following Chien-Wen’s replacement of both knees. Like everything it requires consistent attention to the rehab, moving from very padded through soft through to harder surfaces. It’s especially useful to be able to kneel on exercise mats in Pilates class. It has worked for me1

I had 2nd knee replaced in Jan 2020, (1st one 18 months before). because of Covid I was not able to finish my PT or resume water walking and swimming right away. I did some of my suggested therapy at home but not as much as was recommended. 6 months after my surgery I could not kneel, I would hold on to my walker and slide down onto a thick pad and then sit to do my gardening. I have about 200 ft of roses and other perenials around the edge of my yard which need to be cut and weeded in early spring and mid summer. I have kept working at it, I am now 76 and after 3 years i can just kneel down and get back up with no problem. last winter I had to kneel on concrete to clear my snow blower before putting it away, a little uncomfortable, but doable, I am in US,

I had my left knee replaced 3 months ago by Dr. Liew from orthopadeics 360. I am very happy with the results so far and apart from some minor swelling and some minor heat, I have no ongoing issues. I can kneel on my left knee on hard surfaces no problems at all, although I probably use my right knee to support a greater portion of the total weight at the moment.

1) I definitely can’t kneel after bilateral replacements with midline incision 2) for those who can’t my PT helped me figure out a way to do it ( need to know in case you trip and fall) 3) when on the ground, lay on your tummy. Place a bulky rolled up towel of lap blanket so that it’s firm enough not to squish all the way flat. Place that horizontally across your shins ( between your knees and ankles) closer to the knees though not directly on your knees. Doing this gives your knees enough elevation from the ground so you are not directly kneeling on the ground itself. As you go to get up all the weight and pressure is directed to your shins and your knees ride free because they are neither on the ground or the rolled up towel or blanket. That’s why it’s important to have a firm roll or pillow so your knees don’t end on the floor as you get up. Hope that makes sense. It was brilliant and was the first time I could get down and up from the floor in years ! Going to tell my surgeon about this new lateral approach though - brilliant!

I had a total knee replacement - right knee. I have the usual scar down the front of the knee. Not bring able to kneel is very limiting so I was worried but determined to try. It was very uncomfortable at first but I kept practising and it became much easier. Now after three years I have no trouble at all. My advice would be to persevere starting with soft surfaces and progressing to harder ones.

Absolutely - desensitising the skin is the name of the game. With an anterior incision, I would say that only about 10-20% of my patients could knee. This is very different now with the curved incision around the knee.

For those of use who have had knee replacement/s (2005 on left 2010 on right), is it possible to have the surgery done over so that one can kneel? Am I correct in thinking that would involve removing the original appliance and then have the knee replacement done again using the cutting method described? I am also thinking insurance would not cover the expense. Comments please!

No. It's like I can feel the edge of the device sharply pressing the patellar tendon.
@orthopaedics360

you shouldn't be able to really feel the edge of the prosthesis that easily, unless you are extremely thin. Either way, it should not result in any limitation in kneeling - it is the soft tissue on top of the prosthesis that causes the issue.

Often patients find it hard to kneel even before their knee replacement. We hope to enhance that with the curved incision and removing the underlying inflammation through their total knee replacement

4 years since left knee done. Absolutely NO, I can not kneel. Flesh and metal pinch, while bone and flesh do not pinch. Small sacrifice for a good functional knee. My new knee is my second favorite body part.

That’s not the question at all , we want to know does kneeling damage the parts or the changes that the operation causes . Of course we can kneel but should we ? Are we risking damage to the artificial items in our knee ? I have one unipolar and one full replacement. The incision is 15 years old on the unipolar and 5 on the full . So yes we can but should we ??!!

I don't know about this guy. I had both of my knees done and I can't kneel down. Pain, maybe. The main reason is loss of range of motion and the knee won't bend like before. I just been at my doctor Dr. Sah in Fremont ca. My last Opp was exactly 5 years back. 130 degrees of bending. The one before 125 degrees only. If your knee bends only that much one can't kneel. By the way Dr. Sah thinks that I am doing very well. Never had any kind of pain.

You have to keep the knee bending and unbending, repeatidly straight after the operation (with attached machine if available, otherwise by own efforts,), in order to preserve a full range of movement. If this isn't done, you lose bending ability.
