It makes sense with how they characterize the two in the movie versus the musical. In the movie, McNamara is much more of a "right-hand man" to Chandler whereas Duke is timid, reserved, and only comes out of her shell after Chandler is dead and there is a "power vacuum" in the school that McNamara, who suffers from her own demons and an inability to understand what is going on inside her own head because she's just a teenager enduring a boatload of trauma in the span of only a few days, is unable to fill. In the musical, it's the opposite. McNamara is incredibly reserved and timid, save a few moments where she acts confident around Chandler and then during Blue when she goes along with Duke's BS plan to ditch Veronica with the guys and during its Reprise when she seeks approval from Duke and joins in the singing and dancing even though she clearly doesn't enjoy it (though her little swaying and clapping bit never ceases to make me laugh.) Meanwhile, Duke is much more openly bitter and resentful about being the "right-hand man" because she and Chandler hate each other, but stay together because of the power dynamic they provide (and imo its because Chandler doesn't want Duke trying to usurp her on her own so she keeps her contained beside her rather than risking losing her status as Queen Bee.) In my opinion, it's a very good change because of the different ways the creators want to tell the story. Dan Waters (the writer of Heathers) sets up the power dynamic between the girls very subtly and slowly morphs it over time in response to how the story is unfolding. Laurence and Kevin (wrote the musical adaption) wanted to do the same, but made it so the characters were more clearcut in who they would be by the end, namely Duke becoming a massive asshole who tries to fill in for Chandler and McNamara becoming the face of teenage depression and suicide by being more openly confused about her feelings and also being more sympathetic by being the "cinnamon roll" the fandom has dubbed her because of her little moments.
