@@michelegiannone7403 Well, it progressed from street skating as a kid then pounding on it wrestling (when first diagnosed) in high school, then heavy weightlifting for years (worst part), then skiing (also worst). Don't aggravate it too much, and do everything you can to be in alignment with well-tracking knees, and make sure you always have strong vastus medialis muscles - and get plenty of collagen and protein, for your whole life. Learn all you can about it - and do what sport/physical activity you love, just because you love and need to do it anyway. I can't imagine not snowboarding - f that.

Great explanation as usual! This seems to be a "knee topic" not discussed very often, so thanks for adressing it. I hope there's enough interest in this topic to go further in depth on causes, such as prevalence among certain professions, injuries like repeated patellar subluxations, and Q angle variations--both genetic and between the sexes. And of course, the surgical approaches like chondroplasty, tendon releases, retinacluar imbrications, maquet procedure, etc.

I’m stage 2 and got prp and have done physical therapy for nearly 2 years working with multiple specialists. I have bad pain and swelling even though mri does not look bad

Thanks for this video. I've read a lot of explanations about the different stages of chondromalacia patellar, but this is very clear about the mechanics. I'd love to know more about the long-term management of chondromalacia patellar. I can't help feel like I'm on a long-term track to osteoarthritis and then a knee replacement. But I hope with proper management I can avoid that for decades. And I would be interested in reading your book about sport injuries if you make an English translation! Maybe just a digital release?

4:18 thank you for saying that. I was scared that my stage 2 chondromalacia was wrongly diagnosed because of how much pain it has been causing me. I was afraid it was stage 3 or 4, but the MRI luckily showed it's only stage 2.

Great video , thank you dear . I need your advise: I experience a tingling and warm sensation when I sit in a chair at the office. Let me elaborate: from 8 a.m. to 12 p.m., I don’t have any issues. However, after 12 p.m., this warm, tingling sensation begins in the middle of my knees (around the patella). It doesn’t go away unless I start moving around. Last year, the sensation was different, as follows: whenever I felt sleepy, my knees would start tingling, sending signals to my brain, which would cause my adrenaline to spike. I’d wake up frightened due to an accelerated heartbeat. I visited several doctors at that time, and they said, based on their assessment, that everything seemed normal with my knees. Some suggested that the sensation might be due to wear and tear, as I am 47 years old and have partial tears in my ACL and meniscus, but nothing major. I’ve read many articles about knee problems, but none described the symptoms I have. I run three times a week (5 kilometers each time) and do light leg exercises. I also tried physiotherapy, but it didn’t help much. In Aug, i visited a good Dr and he diagnosed my case as apears in the MRI as Chondromalacia in both knees , he suggested physiotherapy and then PRP if did’t work.physiotherapy did’t work so i had to take PRP,with that being said i did’t see any improvment, therefore, the Dr gave me another PRP .so lets see. He said i’m in stage two, i can share my MRI.

I have stage 4 in both, and the specialists tell me just do physio. Makes sense as i need to move my kneecap so it sits in the groove better.

i have incorporated an exercise program that includes a stationary recumbent bike that i do everyday for 5 minutes...not a lot of resistance but just enough to get the knees moving and lubricated...is it true that Motion=Lotion in regards to knees???? and cartilage possible regeneration????

I developed cmp when I was 10 yrs old, dislocated the knee when working so had my 1st surgery at 19 and then again at 29yrs old. It's been really bad for the past 3yrs so had an x-ray for the first time since diagnoses. I have been told on the last surgery that it'll be a reconstruction if it plays up again and not even 40.

In patellofemoral pain, the cartilage is the first thing checked but, in fact cartilage isn't innervated ... Why this paradigm ?

The underlying subchondral bone contains sensory nerve endings and gives pain sensation;

@@shahemabb7373 yes , it will get worsen. Good exercise is swimming and no load bearing exercises . I am suffering with same problem and it took me more than 8 month to reduce my knee pain, I stopped climbing stairs and cycling , I am using ayurvedic medicine as well . Hope you will recover soon

Yes swimming help alot , I am suffering since 2023 and started swimming, during swimming I use life jacket so that I can walk in water , I do it for 2 hrs and my pain is very minimal now . Note - don't do cycling and stairs climbing for 3 month you will get better.😊

I'm a Bharatanatyam dancer battling with the same issue. Last year I got an MRI done and I have Grade 2 CMP. Every time I prep for a program it gets worse. I have a program coming up and around two months of regular practice the pain started aggravating. It's so sad that I'm not able to do something I absolutely love doing. I'm so scared to do it now. I feel something worse will happen.
