I had my tkr 3-1-21. STILL IN PAIN 24/7. I’ve done everything possible for a good outcome, but it was horrible. I’d NEVER do the other knee. I’m 77 this yr and it scarred me forever. I have PTSD from it.

I'm 2 years out on the left knee and 1 year out for my right knee. Both knees have pain on flexion and both knees have the feeling of tightness across the top of the knees. It feels like a tight ace bandage. The pain is sharp on any flexion over 100 degrees. I'm very active and I don't let this pain stop me from walking, biking and golfing. But I do need to take frequent rest due to the pain. I'm glad I had them replaced but I think I have substituted one type of pain for another pain.

I had a TKR 10 months ago. I have good days and bad days. With pain and inflammation. Walking feels better than standing on one place for long periods of time. When sitting and to start a step it takes me a second to be able to gain my balance to walk. This also happened to me when I gave birth to my second child 19 years ago. Due to a dislocation of my pelvic bone. It was a terrible pain I felt like my hip was off socket and couldn't walk. OB send me to therapy to get pelvic back in place. I feel the same problem when I start to give my first step. It's like a freeze I just can't get up and walk normal. It's very frustrating! I don't know if the issue with my knee has to do with my past injury. Plus a bad fracture on this knee reason for the knee replacement.

It will be 2 years for me in January with a TKR. I am truly blessed to have my life back and it feels like my real knee. I do not have any pain, no swelling and have full range of motion. I am back to doing the things I love like hiking and biking and have zero issues. I worked hard in therapy and continue to do strength training. To be honest, I rarely think about my knee anymore which is a great thing.

Hey mike, I just had knee replacement surgery three weeks ago, how long did it take for you to get back to work, I work outside, i deliver construction equipment and plan to be out for 8 weeks, but I am nervous that I won’t make it. I do my exercises every day and go to PT two times a week, what do you recommend? Thanks

I'm praying, that when I get my right knee replaced , on the 13th of this month, I'm not in too much pain, I had a brilliant professor Doctor Mr Kay , and his team in Wrightinton who operated on my right hip, and done an excellent replacement op ,and it didn't hurt for long ,I know that getting a knee replacement op, is a lot different and a lot more painful, but I'm praying I have a quick recovery, as a few months after that I'm getting my left knee done , by the same team at the same hospital.

You are like me I had really confident Orthopaedic Surgeon, Anaesthetist & Team so trusted I was safe & in good hands. Keep goal in mind plus must be very vigilant di all exercises give. Be patient do as instructed then will be pleased with ourcome. I have been home 9 days today after L/TKR couldn't be happier. Painful part is pushing self to do exercises but can see definitely worth the effort. Good luck keep as fit/healthy before op start exercises before op that you know will have to do after op. I had a knee revision to replace a knee replacement I had in 2000. I had the op 10 weeks ago. I had mild pain for 7 days then nothing no pain. The knee is recovering really well. We are all different don’t be put off a replacement

Update: 3 years since TKR. STILL in pain. Stairs are worst but it just aches too. I fell on 5-31-22 and fractured the femoral neck same leg. That involved 3 huge screws. I refused to do surgery until I was promised it would be as minimal as possible and NOT a replacement I was told hip replacements often happen after knee replacements because your balance is thrown off. In fact this is very common !!! It was a different surgeon and he did such a great job. Tiny scar NO PAIN after 2 days. If considering a TKR, look at stats for future problems. I wish I’d never had that replacement. My other knee gets injections. No more surgeries.

I’m 14 months out for my right knee and it hurts worse than it did prior to surgery. My left knee is 2 months out and I’m 3 weeks out from MUA on it it and it’s complete garbage. I regret doing them both

I'm just under one year post op on my second knee replacement. My first knee (right) was more difficult to recover ROM, but now that it's been two years it feels great, except for some lingering tendonitis. Left knee was at 0 and 120 about a week post op, BUT is still have some minor stiffness and swelling. It's nothing like the pain I had pre-surgery, but definitely does not feel like a normal knee. I'm functioning fine, and have returned to my normal activities. I think after years of pain, my BRAIN needs time to realize these activities are no longer painful.

I fall into the 20% with both knees! It is so difficult to find the person who can help you. We moved, my new GP reluctantly referred me to a new ortho. The pictures are perfect, but I still have a lot of pain, and swelling. All they can suggest is PT. But three PT’s and all they care about is range of motion; finding a good one, is nearly impossible. I’ve had compression therapy, deep massage and shock wave; nothing has helped. I walk, bike xc ski, and walk. I live in a small rural community. My GP, the gatekeeper won’t go beyond “my X-rays are fine. My knees aren’t getting better!

I'm having instability and pain issues as well, 16 months post TKA right knee. I can't play tennis or ski as I had anticipated... My Ortho and a 2nd one both say that X-rays are good and that perhaps my PCL is torn, so they suggest revision surgery with a posterior stabilized femoral implant.... I don't want to go through the pain and downtime again, so I'm holding off and dealing with it for now. I read about cases with the wrong implant sizes rubbing against tendons and various other horror stories, so I think I'm done with any future TKAs 😟 Have any of your doctors offered any possible causes or solutions?

I'm 17 months out from TKR -imaging looks great as well as positioning of the replacement. It hurts when simply placing my door on the ground from walking. My surgeon thought it had something to do with my back...it doesn't. I'm so frustrated and depressed because I was a very active center fielder for a traveling softball team. What do I need to do at this point? Can I get a cortisone shot?.

I wouldn't recommend cortisone. I've had lots of them in my back and twice in a knee. They never worked for me and they are not good for your joints - they can actually cause damage. If they do help with pain, that effect does not last and the length of time that you get pain relief shortens with each subsequent injection. For instance, my sister had cortisone in her coccyx and was delighted to be pain free for 8 months. The second injection relieved her pain for 3 or 4 months and the third for only a week or so. You can only have 3 a year because they cause damage too.

Tony, I'm 70 years old. I am 11months post-op on my right knee and 3 weeks PO on my left. I have complete function with my right knee with only slight pain medially and laterally where I guess ligaments once were. I can kick start my classic motorcycles with that leg which is why I had that one done first. I kept a logbook on my progress and I had distracting pain up until 5-6 weeks. By 8 weeks I felt almost normal with 135 degrees of flex. By 12 weeks I was riding my bikes again. Compared to that first knee, my left is a little behind schedule with the pain and stiffness. I only take naproxen if I think I need it to sleep for a few hours at night. Today, with swelling still present, I had PT and I now have 121 degrees of flex in the new knee. That came with great "discomfort" but no screaming. I am putting in the time with home exercises. I will give this knee a full 8-12 weeks before I pass judgement on the success of the procedure. I only know what I know from following you for over a year and knowing what to expect. Last November, I was just an ignorant TKR candidate without a clue. Your videos gave me the knowledge and patience to give it all time it needs and to put my head in the game through recovery. I also want to give credit to my therapists who really know their stuff and pay attention to what I'm saying.

9 weeks in - left TKR. Doing really well with flexion and extension. Walking stairs well but now developed pain in outside of knee just below the patella to the outside - feels like bone on bone again and is affecting my walking due to pain. Very frustrating. PT is doing painful extensions by pressing down above AND below knee. I'm convinced this has affected the new pain I'm experiencing. (TKR patient from the UK)

I am 2 years in now with right knee replacement. Although original pain is gone, I still have some pain and tightness on the right outside of my leg (not my knee joint), I have full use and movement, but the tightness really bothers me sometimes. I have thought maybe more PT would help with easing my movement and getting rid of this tightness. Got to do something=getting left knee done now in August.

When I woke in the hospital I stuffed the sheets in my mouth so no one could hear me screaming. I’m no baby about pain either.

I am 7 months out from TKR on my right knee. Surgeon said he could only get me to about 50 degree bend pre-op. After 5 months of rehab, I was able to get full extension and 115 degree of flexion. I have good strength and endurance, therapist said that it was as good as any non surgical knee… I do not have joint pain, I can stand, walk , do stairs etc…the pain I feel is more muscular in nature. It goes around the all around the knee, with that tight band people are talking about. The only thing I can describe it as being like, is when you do a heavy workout and the soreness and pain you feel the next day, is what I feel in my knee all the time. Massage helps as well as moving and bending the knee. I can only think that because my knee was in such bad condition prior to surgery and for years, it’s going to take my muscles in my right knee time to be reconditioned and to work out that stiffness and soreness… Does this make sense?

I ended up being diagnosed with complex pain syndrome. After 7 years, draining bloody synovial fluid off my knee several times, and the constant swelling it hurts a bit less. Now it’s getting loose. Docs claim nothing is wrong.

I have occasional pain and cramping, but there is better support for the knee. I am a year and two months in. A Tylenol helps. The next knee is slipping while lying down. I am dreading that severe pain after surgery while healing. I can treat the pain, but that severe cramping twists the legs from its normal position.

Thanks Tony for a very informative video. I am 5 months after the left knee TKR. I have 120 flexion. Not 125 as I respond wrong on your other video. The surgeon told me it takes a yr to fully recover from TKR. I am seeing my deep tissue massage therapist once a week, which is helpful. (paying out of the pocket though). I need to be on the bicycle though to have strong knee as I want to walk. I am tried after a little walk in the stores. My heel hurts as well my knee too. Any suggestion? Love to hear from you.

Doc i am 5 month post acl surgery i still have pain going down the stairs and it still feels unstable i dont trust my knee and when i go to the gym is it normal feeling pain doing the exercises? I have no problem with pain i am just afraid that i making it worse or that i am doing something wrong

Be careful. Read the statistics on falling after knee replacement. I fell a year after and fractured femoral neck of same leg. The repair of that was nothing compared to the knee. Operation was 3-1-21. It’s 9-30-24 and it still hurts. I have chronic insomnia from pain. I’m afraid to go anywhere alone in case I fall.

Hi Tony, I’m 15 months out from a total right knee replacement following a repair from a severe tibial fracture the year before. I’ve had pain in the medial side just below the knee capsule for the last 6 months. It’s gotten worse over the last several months, with swelling, warmth, and pain.I sometimes get stabbing pain in a very localized spot just above the anserine bursa. I have full flexion and extension and can walk normally- I’m quite strong. Just persistent pain. My x-rays look great. My surgeon thinks there is some instability in the joint and I may need to have a larger spacer put in, but we’re trying RICE first. Second line is cortisone. My question is can i continue to heal and recover well into the second year? Or is this as good as it gets?

I have no medical expertise, and no magic bullet solution but my suggestion is look at the content put out by KZbinrs like Dr Kelly Starett and Dr Aaron Horschig. Dr Kelly has done a post surgery series. Perhaps some specialist physical therapy may help. The tools Dr Kelly used early after his TKR for pain relief like the H-Wave electrotherapy may also help and give you tools to manage until it’s sorted out. Wish you all the best.

Thank you for this information. I am 18 months out on L TKR. I have less function and more swelling than I did with my badly deformed original knee. Recently, My physician checked for infection because of swelling- and although the fluids had higher levels of RBC and WBC- they all were within the "norm." The recommendation and diagnosis if flexion instabiity and my options are a revision to add a thicker spacer or just live with it. My flexion is limited and it seems to get worse rather than better. Some mornings riding the stationary bike is harder, at first than when I was 2 weeks out. My description- my knee feels like a stump. Skeptical about the next 18 months- but a revision?? I may try a lymphatic massage and treatment and keep up with my exercise plan.

Thanks for the comment. Stiffness after prolonged positioning is common. I haven't honestly seen a way to eliminate it other than staying active, but when you are sleeping your options are limited. 4 months is still early so keep working on your exercises without overdoing it. Sometimes patients are actually doing too much and feel better when they do less for a day or two.

My doctor doesn’t give a shit.. he told me at my 2 month check up when I couldn’t straighten my knee or even bend to 90 degrees.. he couldn’t do anything else for me. I like my physical therapist .. I’m 4 mos out and still in physical therapy… but he has no answers to my questions

Thanks, this was helpful to let me see that it’s not all in my head or that I’m not “working hard enough”. 😂 I think the surgery did what it should as far as the joint goes, but the muscles are another story! I’ve had one manipulation and it didn’t really make any difference. There are several concerns that I have, but I’m interested in your opinion on something that is just happening…. Within the last week or so my range of motion has gotten smaller and there is a thickened area underneath the scar at the top of my knee into the thigh. The thickest part is an area where three different times stitches popped through the surface and had to be pulled out. The skin around the scar is actually looking like a little valley if that makes sense. Have you seen this before? Can you suggest any thing I should do?

Great question. Usually anything between 120 and 135 is full flexion. 0-degrees is full extension.

I was 35 when I was in an accident shattering my tibia plateau in two spiral fractures on my tibia fibia and destroying my ankle after 4 years of surgeries and rehabs the doctor finally replaced my knee waking up after the surgery I was rushed to sit up where Hospital staff let me fall out of the bed landing on my left knee and knocking myself unconscious the staff rushed me back into bed x-rayed and called the doctor immediately I was put in the demobilizer and was told there was no damage done from the fall and was sent home it was a Friday Saturday morning I woke up and more pain than I'd ever felt before it was it was my 13th surgery on my left knee alone I called the non-emergency number for my doctor were they informed me they would see me first thing Monday morning encouraged me to take my pain medicine and stay rested when I got to the Orthopedics office I was greeted by my doctor only to yell at me telling me I should have never worn the demobilizer the hospital had put it on me and I'd left it on the whole time after discussing it he took some X-rays and said that there was nothing wrong it was just post op pain after burning through the pain meds in 3 days he had given me five more days worth of pills and sent me to physical therapy in physical therapy I could not perform any duties let alone bear any weight on my left leg I went back to see my doctor a total of 14 times and one month where he finally told me I had a low pain tolerance and I needed to seek mental help 8 months later still being an excruciating pain leg was swollen was hard to walk more than 5 minutes so I seeked out a different doctor told that doctor everything that had happened he had taken some X-rays and reviewed them immediately pushing a wheelchair into the room demanding that I had sat down and he did not want me walking in his office he had told me that my leg was fractured and my knee implant was loose that I needed a surgery immediately within the next two weeks I was having my replacement done less than a year after my first knee replacement I was having my second after following everything I was instructed to do by my new orthopedic doctor it has been a year and a half I'm still taking pain meds my leg is swollen and I can only stand for about 5 hours a day I have tried everything from medications massages the pain in the leg is unbearable I am now seeking a doctor to give me a final opinion on removing it I've been in pain for the Last 5 Years and struggling to support myself my quality of life has diminished and I am desperately trying to find some sort of help so if anyone in here has any advice any types of treatment I just want to live somewhat of a normal life I am only 40 but yet I feel like I'm 80

Jon so sorry for what you e been through. Sure hope by now you’ve gotten some improvement and hope for a better future with that leg. I’m 70 and had a TKR 1.5 years ago and it’s not good, huge mistake having it done.

I had my right knee done 5 years ago no problems everything went well I had my left knee done 7 months ago and I'm still in pain and I don't even sleep well at night

In 10 years I have had 3 full replacements and 1 revision on left knee and 2 replacements on my right. Left knee had the first one put in at a 20% slant. 2nd one mechanical failure. The list goes on with subsequent and beforehand arthroscopics, 19 knee surgeries in all with another clean on the last replacement to be done. No one has worked harder after every replacement etc than me. The surgeons have commended me for all my hard work. I have chronic pain and have a pain specialist who has had me on OxyContin slow release twice a day, Endone when I need it, Norflex for the back cramps and gabapentin when I need it. I have also had 4 radiofrequency ablation for my lower back and PRP injections into the knee. Pain Specialist only wants me doing these fancy things and not getting to the basics of Physio and seeing to the rest of my body’s well being. As I said, I’ve worked so hard with exercises, hydrotherapy and cortisone injections into the knee or sometimes into surrounding muscles. I have been known to be pushing myself too hard at times but I am proud of 110 degree bend in both knees. I have had 3 different surgeons over the course of these surgeries. I have osteoporosis and rheumatoid arthritis badly in my hands now. I can’t get anyone to listen to me!!! Any GP appointment is takes up with pain reduction discussions-even though what I am to have is sent to the GP in writing what to prescribe me. Not the slightest bit of interest in anything else. I see a psychiatrist monthly who has had to put me on antidepressants and anxiety meds. His specialty is not pain but I can talk about that if I want. The final straw was my last visit to the Physio. I told him what exercises I’d been doing both at home and in the pool. He said ‘your bend is great so what do you want me to do’. Apart from the GP, I have a 2.5 hour drive each direction for specialists as we live in a regional area. I’m in Melbourne Australia and I know the medical system in the US is a lot different, however, the take away from your clip is fantastic. You have given me a lot of confidence to raise these points you make with the GP, as I have to tell you I have totally given up trying these days. I am 66 years old and thank you for taking time out to cover this important topic.

I’m a enlightened patient of B/l TKR since 25 May 2015 and pain is like hell and all tests are okay including Bone Scan.Actually this surgery is being done to satisfy certain MNCs ,corporate hospitals and doctors. It should be banned forthwith as it is afailed treatment.regards

More than 6 months after my left knee replacement ( age 79 ) during my recovery I was beginning walking again and still felt a lot of pain in the knee. No infection and xrays were normal for the knee BUT my left hip also had arthritis and I had that replaced and now I am walking 5 miles/day. So go figure. it was referred pain from a bad hip.

You're right, I did all the googling all the reading before the surgery. What to do.What not to do. Now I'm reading all these horror stories since didn't go my way. It's been 4 weeks and I'm f*****. It is definitely my fault 😠

What a stupid response. Obviously no one knows how their surgery will work out. You can do all the right things and interview several surgeons, but in the end it’s all just luck. Explain how an overweight, out of shape 70 year old can recover more easily than an active person who’s only in their 50’s. I’ve seen this scenario too many times. Makes NO sense

I am still having a lot of pain all around the knee area and goes all the way up to my private areas. Also, I lose strength on the leg and fall. Oh, and is making a sound when I move my leg a certain way and you can hear it and feel it if the person is touching the leg. Is very hard for me to sleep or even do simple things. Worst thing I could ever done.

I have a question and I am desperate for some clarity. I had a TKR on my left knee three years ago and I have constant swelling, stiffness and a feeling of heaviness in my knee and leg. Is this normal? I dont see how it could be and I get no answer but your replacement is well placed. Can you give me some insight? Thank you!

Hi Thomas, I wish I had an answer for you but I don't. There are some many systems involved between the lymphatic system, nervous system, immune system, etc. The surgeon can only look at the hardware and say the hardware looks great, but all the soft tissue stuff might be completely disrupted.
