Dr. Dan Clauw, the Director of the Chronic Pain & Fatigue Research Center at the University of Michigan, discusses pain that stems from your brain. He presents an overview of what is currently known about the underlying causes of chronic pain and discusses the rationale behind a variety of different treatments for chronic pain. He also shares information about a self-management tool called Fibroguide (fibroguide.med.umich.edu/) that has been shown to be helpful in improving the symptoms and function of people with chronic pain.

Unfortunately some 'Pain Management Services' are assuming if pain has existed for longer than 3 months, it's chronic pain therefore promoting, medications don't work, investigations are of no benefit and exercise is the 'prescription'. This is creating much distress for patients and a culture of hostile therapists has evolved who fail to identify or respect their patients need for appropriate diagnosis and interventions for pain creating conditions....It's a very dark time for many patients in pain.

And, for the record, I did every single one of the non-opiate modalities before I had ALIF surgery, and after. Not one of them was effective in controlling my pain. Having fibro makes it impossible to be stabbed with "tiny" acupuncture needles when it feels like samurai swords, and having IBS and ulcers makes it impossible to simply take over-the-counter anti-inflammatories. So, some people take opiates out of necessity and because they work without side effects. It's not a one-size-fits-all world!!

Sadly, fibro is a 100% brain generated pain. Your medication isn’t necessary, because feeling the pain isn’t necessary. This one is a one size fits all. If you have fibro, you can turn down your pain and ultimately recover. I know. I did. Look up dr Sarno.

@TheAliKat I totally agree with you! One-size-fits-all has been a lie from beginning!! I have chronic pain & quit opiates because of constant judgment by High School MDs! I’m alive but have no life because of the pain & am screwed because of govt interference of pain meds being prescribed by licensed, trained Medical Doctors. All now sent to Pain Mgmt who want to do “procedures”! Proceeding only to cause more harm! It’s ludicrous! I’m fighting back! I’m struggling, but at least I’m trying! I did want to share something I’ve discovered from experience & research…..pain causes anxiety, anxiety will cause the stomach to produce more acid. Excessive acid causes ulcers!! All fruit & many vegetables are highly acidic! Pain causing acid production & consuming foods with high acid content is creating stomach ulcers, I’ve deduced from connecting the dots! Unknown & unchecked, the ulcers will bleed! WTH is wrong with trained professionals ignoring this? If a non-medically trained individual can determine this, why aren’t doctors addressing the entire situation of pain effects on the body as a whole? I had a wonderful, caring, compassionate pain mgmt dr who passed away & I’ve been a “lab rat” for these other Frankensteins! It’s like living a nightmare! Keep up your strength & spirit to stand up & speak out! We are being pushed, it’s time to shove back! 👍

It doesnt totally take away the pain, but definitely does help me. One pain doctor told me that I should be taking a higher strength, but my doctor wont give them to me. I am taking 2 - 3 a day but my doctor has told me that she doesnt like me taking it as it is usually only used for people with severe pain like Rheumatoid arthritis or cancer pain. Over the past 25 years I have tried every therapy and exercise and diet, to no avail. I have been to so many pain clinics and therapists without being helped. I am sure my pain must be as bad as both of those diseases. I have told her to give me another solution, which as yet she has been unable to do. I feel I should be able to get whatever pain med that I need to take me out of this crippling, debilitating pain. I am 76 years old and can honestly say that at this stage in my life I should be helped with whatever treatment I can get. I would just like to have a few hours here and there were I dont feel anything. just give me a break from the pain cycle. I swim on a regular basis and find it gives me about an hour of relief. We put our animals out of their misery when they are in pain, so why not humans. It really is a horrendous illness . Please put us out of our daily misery. We are not living, we are just existing. Chronic pain is not all in our heads. I have nerve damage and neurological conditions that causes me pain. On top of fibromyalgia. There's no cure for what I have and treatment is limited for me. I am use to pain so I am not aware I am sick with infections until it's in my blood. Also, I was a yoga therapist for 20 + years before my chronic pain and neurological conditions started to take away my ability to walk and function. I am allergic to everything because of a rare condition too. I started having severe pain when I was nine years old. I wasn't aware that I was born with nerve damage until I got a skin biopsy. I also always have inflammation in my blood and urine. I have chronic UTI and kidney infections too. I had seizures in my childhood and they went away for awhile but returned in my mid 30's as well. Also, Fibromyalgia can cause depression and anxiety, so it's important to see mental health services as well as pain management if you have fibromyalgia. It can help deal with it but it's not a cure. I meditate a lot to help deal with my pain. I also have tried nerve blocking medication but I am allergic and ended in the hospital. The only nerve blocker that helped me is Botox in my head neck and upper back. I have fibromyalgia and I don't have tender spots it's my whole body that hurts and swell's up.

Maybe a better title would be something like “Retrain the brain to feel less pain”. Based the title alone, chronic pain patients will not receive this video as well as they could. Chronic pain suffers already spend so much on the defense. The title of this video could be received as downplaying someone suffering.

Sometimes pain can be like a leaky roof. You can see where the water is dripping through the ceiling, but that quite often is nowhere near the place the water has entered the roof. The water may have traveled down & along a distant beam before making it’s appearance. I agree that when the source of the pain is not readily apparent their should be further investigation. A patient’s pain should not be rejected because ithe available diagnostic tools are inadequate.

It IS in your head, if that's where your brain is! Your BRAIN controls PAIN receptors. Please re-watch the video with a more objective heart and mind. As a side note, take down the info other commenters offer: names of doctors like Sarno, Forrest Tennant, Shubiner, Alan Gordon, people named like Jon Cabot Zin, Nichole Sach --- just to name a few! I've never heard of these people, but I will be checking them out!!! Also look into your gut health -- it's your second brain. So much valuable info to be learned on that subject alone. BTW, I'm a 30+ year CPP who is still fighting tooth and nail to get answers. Everything, even the laughable or seemingly wrong, is taken into thoughtful consideration before dismissing. New "non-addictive" treatments suck. Pregabalin Gabapentin and antidepressants have worse withdrawal than opioids, are much more damaging to the body (and can cause deadly side effects on any dosage aside from often causing lifetime harm on the nervous system, with common problems being twitches brain zaps and a mental hell that can last for years) while being very unpredictable (the brain chemical imbalance BS is busted...they change the way your brain adapts and develops and that's why they take so long to take effect) and have been proven to have a marginal effect at best in most cases. Plain old opiates just work and on the right dosage and proper side effect monitoring they can make the biggest difference in life quality. Their only problem in the eyes of doctors is that they won't mess you up as much as the aforementioned (so you won't be visiting the doctor that often), and they are very cheap (compared to antidepressants and Lyrica/Neurontin that make Pfizer BILLIONS per year...in fact more than 10% of their multinational medical company's income is attributed to these which are addictive/dependence causing AND harm causing)

It's further interest of mine to know they don't have chronic pain to know what we live with. We know that pain is a signal from the body to the brain that there is something wrong going on. We also know that there isn't ONE "right" answer on what will either calm down the body or brain to meet the doctor's assessment that what we are doing is the "right" thing. Pain maybe subjective to those who don't experience it, but for us that live with it, realize that it needs more than glib answers to help us.

Reading your comments on your own pain experiences it really makes me sad to see so many that have fallen into this vicious cycle of 24/7 pain, that all of the treatments, lifestyle changes and drugs are still struggling. I’m So sorry to all of you that you drew the short straw and you were picked to be the ones who suffer from constant pain. It does make you a better person, it helps to keep you real and you sure do have a lot more empathy for others, but it is also hard work and life isn’t easy, yes we still have things to be grateful for and we are blessed with many things in our lives, but it’s tiring. Having been diagnosed with Fybro in the past year, after years of pain & what seemed like the lack of interest by my Doctor & Only had x rays over the years in a couple of places that showed nothing. My doctor told me to read "its all in your head" Suzanne Sullivan. I was upset & felt humiliated, but read it, Its exactly as this man is saying. Its not each joint it comes from the brain. It made me understand more. I am not one for meds, having chosen to try alternative methods. Hot wax for my hands, hot tub for general all over pain, cold water therapy "which works really good "for a break from the pain. I have anxiety which my Doctor said exasperates my pain. He told me there was nothing that he could do, until I reached a point where I couldnt live with the pain I am in daily, affecting my memory as I couldnt function properly. now I am on a low dose of gabapentin, which does not touch my pain. but I have hope it will help as it increases. This is what I do, 1/4 teaspoon of MSM crystals, 1/4 teaspoon vitamin c and 1 tablespoon of kratom. Don't be scared, western medicine has been lying about kratom, it's related to the coffee tree and in combo w the MSM it detects inflammation. Complex regional pain Syndrome has the highest pain rating in the world, it's known as the suicide disease, because there is no relief from the the constant pain. I've been in pain every second of the day since waking from a coma in 2014. However, this combo has given me my life back. Please try it. I didn’t get up to the dose, I got serotonin syndrome instead. Which knocks out my ability to take 85% of the treatments recommended also to include cannabis in any form because of how low a does the serotonin syndrome occurred at. My sleep study showed a form of narcolepsy which was the reason for my sleep issues not my fibro. The Neurologist said I have too many serotonin and gaba receptors to ever be able to take any drugs that effect them. Also the opioid epidemic has nothing to do with RX pain therapy. Only 1% of these patients are in those statistics. It’s a fentanyl and heroine problem by the FDA and CDCs own admission. Also many countries do and the reason the numbers aren’t there is because a lot of them don’t need a prescription to attain them cause not all governments are so intrusive into the patient/physician relationship. The fact that thousands of patients have been stable on long term opioid therapy at therapeutic doses for 3 decades makes your argument null. 8 years ago my pain level was 10, I was bedridden and dying of sepsis. I’m a unicorn I guess cause now I actually have a life and function 70% better than before. No autoimmune patient will ever be as functional as a healthy patient. The best we can hope for is 60% functionality vs a healthy patients 100%. Dependence is very different from addiction.

In his opening line talking about the volume nob being turned up is very accurate. Before the injury that causes chronic pain for people happens, all the volume settings for pain were default and worked well, but if those volume settings get tweaked during the injury (brain and spine) and your injury isn't healed relatively quickly the constant pain can begin to appear like it's affecting other areas a lot more than it might be. Other conditions or underlying conditions on top can exacerbate the problems to extremely high levels. But if you are suffering from chronic pain your only goal should be to at least understand or find out whats wrong, because you didn't feel like this before the injury and now it's been months maybe even years of suffering.

Pain is in the brain. I suffered from chronic pain for over a year. As Dr Sarno says “you have to think your way out of this”. I am no longer in pain. You have to overcome yourself and have belief. I know it’s hard, you don’t need to preach to me. I have been there. No one can get you out of this but yourself. Trust me you will. Stop caring about it, distract yourself, do what feels good. Don’t let it bother you. You know it’s going to pass. It’s not even real. I typically hear all bullshit of the day -“how would you know” - because I’ve been through it “oh but how can I not care when I am in so much pain” - have you not been listening to me - I have been there and you just have to do it. There’s no easy path here or no shortcuts. If I can do it, anyone can. I am only leaving this comment to give others hope and let you know it is possible. No need to contact me. Thank you so much for this lecture!! I learned so much (especially about injury pain vs brain pain, and that you can have both, and both need to be treated), and I finally "get" it now, about the recovery process. I will try not to overdo when I'm feeling better (some chronic pain has crept back in, and depression never left), and will try to do some activities that make me happy, and see if I can crawl out of this ravine. -- Yes, my Drs have all told me this before, but having two hours of the whys and wherefores finally got it to sink in!

It a very imformative video about fibromyalgia, I am not sure that he is right about the cause, research is still being done but he does describe the intense pain correctly. Many patients and research studies show the medications he is talking about barely do better than a placebo & the side effects are very concerning. I , and many, chronic pain parients do not understand why doctors are saying opioids do not treat our pain, when we clearly say they do. Antidepressants do not help pain. We do not even know if they help depression! There are too many overdoses, in general, yes but I don't think doctor's and chronic pain patients are to blame. The FDA & DEA are pitting doctor's, patients and pharmacists against each other. Too many pain patients have now been lost to suicide or quality of life dramatically decline. Cannabis is illegal here & if it were legal, I would only use it to sleep. I don't know.

I've had fibromyalgia back and neck pain since my 30s. I also have a curved spine. After an incident at work where I caught a box of six-part computer paper from a stack a boxes workman's comp sent me to a physical therapist and I'm in a real physical therapist and I already had a great chiropractor. Of course I was working out at the gym anyway, but had to stop for a while because of the injury. Once my chiropractor acquired what they call the Nautilus back machine, which you used to have a prescription for, that's when the real healing started. Between the real physical therapist and my chiropractor I was out of pain within two or three months or less. Today you can't find a nautilus back machine and the physical therapy that my back doctor prescribed for me slaps me for $40 copay a visit. The physical therapy they give you today is so damn slow that you cannot even feel the effects of it. Maybe for knee injuries blah blah blah, but now I have osteoarthritis in my hip, osteoporosis and degenerative disc in my hip. My joints in my fingers and my toes and my wrist feel like they are on fire sometimes. Yeah it's all in my head. Now we all know I think that the brain and the pain is all connected and I had the pain under control for years with just Advil and clorazepam . seldom did I use opioids. I am a mental health counselor and when Bush and Obama destroyed our economy in 2008 most of my jobs went away. I live in a part of Florida where the pay is too low anyway and I ended up walking into Marshalls and asking them if they were hiring. That was 8 years ago. They have kept me standing in a dressing room for almost 8 years. I have a stack of ADA very specific paperwork accommodations. The accommodations would be very simple. Just put me somewhere where I'm moving around and they wouldn't do it. I started having pain after the first year on my left side which is my bad side. It got so bad I would leave work and go to the ER. Eventually workman's comp had to do something. So they sent me to physical therapy, which is what the orthopedic doctor prescribed after a million x-rays. One look at my back and he was amazed that I wasn't in worse pain. I only saw him like three times and the physical therapy was so slow. I would return to work the next day the pain would return and the next day I would go to physical therapy. It's a joke. I finally got in to see one of the best pain management doctors in Florida. He gave me my life back prescribing me opioids and some gabapentin. No mention of epidurals or anything like that for me. He was the kindest man I have ever met and often asked me why I haven't given up. This was in 2019 and in 2021 he passed away from cancer and the other doctor discharged me from the clinic without a prescription. Without the pain medicine I can't work because I can't stand or sit because I have spinal stenosis and most of the back problems were triggered by standing and this damn dressing room, my house was destroyed by hurricane Michael when a tree fell through it and of course the insurance company delayed everything. I owned my town home and I am retired living on social security and what I make it Marshall's. I kept trying to work and then one day I walked out of Marshalls to my rented efficiency and went to bed. I went through withdrawal, IBS and skin cancer. The antibiotics destroyed the good bacteria and even though I got into another pain management doctor. I was in such bad shape physically and mentally that I did not crawl out of bed except to take my little dog out. It has been an absolute nightmare. I was only working part-time but on medical leave of absence for 7 months. The manager promised me he would move me but when I return he schedules me again for the dressing room. After almost 3 months I called human resources and the next day I was on the floor in a different department. As long as I was moving around I was fine. I didn't even need pain medicine, but after Christmas this manager was so pissed at me that he stuck me back in the dressing room and would not move me. In 2021 after my doctor died I went to bed and did not get out of bed except to buy some groceries and take my dog out. My antidepressants did not work any more probably due to the opioids. So I took the opioids and hope of getting some dopamine in my body. I really wasn't in that much pain except for my stomach. Now these doctors want to tell you that you really need to map your body and your brain. What they don't remember or think of is that when neither is functioning well for you, it doesn't matter. My psychiatrist did not have an answer and the pain management doctor I signed up with I saw one time. The rest of the time I just see his nurse. The only thing he does is basically injections or epidurals. Now I've been back at work for several months and something happened in my hip that caused me so much excruciating pain I had to hold on to my hip to make it to the car. Now the doctors have done nothing but take an x-ray and refer me to an orthopedic clinic that told me the last time I was there they couldn't find a reason for my pain. This is what these people do and then they write you a script for physical therapy. I still haven't called the physical therapist. Yes it's to strengthen my core, but I've lost so much muscle mass it'll take forever. Now I'm searching for a nautilus back machine and a decent chiropractor. I don't know what my point is here except that this doctor is giving a lecture about the research he did 5 years ago on managing pain, specifically fibromyalgia and different forms of arthritis. What the hell are you supposed to do with the deformed hip? Tell your brain that it doesn't hurt anymore and that the degenerative disc and the bulging disc aren't hurting anymore? I'm laying here at night praying I can go to work tomorrow and work for 6 hours, because if I don't have a reason to get up I might as well not live. The only thing that's keeping me alive is my dog. The constant chronic pain and the BS that these doctors feed you lead you many times to hopelessness. Who the hell wants to live like this? I have prayed and still pray for God to send me some blessings. I've never been a couch potato. Fortunately I'm still tall and slim. Unfortunately, the insurance company didn't give me enough to restore my town home and I live in an efficiency in a high-rise that rents to people that need to be in a nursing home. I look around and just want to cry and ask God why me. I've always been a fighter but sometimes you just reach the point where life is just not worth living. I hate to sound so morbid but that's how a lot of us feel sometimes. I wish there was a pain support group online for us because many of us are suffering and this guy on this KZbin channel is not helping,. If anybody has any ideas I'll be willing to leave the group. I've led many groups in the past where I worked with trouble teenagers and juvenile delinquents. I think we could all use some support from each other. My close friends don't live in town and it's hard to go to church or make friends when you're in excruciating pain or you're so depressed you can't get out of bed. Sometimes we just need a friend.

Dear @gawebn, It is so easy for our medical teams to miss something fairly easy simply because they are looking (in pain case) for a "needle in a haystack', but it doesn't mean you aren't having issues. Another point: sometimes doctors are "so busy" especially in the ER they miss doing simple tests that can show them where the problem is: case in point? Our neighbor (it was finally found) had a heart attack, but no enzyme tests were performed. This is crucial, as that you only have so much time with the heart to help it along. Sort of drove us nuts, they said to him; "We see you're having a problem, but we don't know what it is." No one needs to be waiting three days to have a simple enzyme test done.

For the last 6 months due to my thalamic stroke my nerve pain is like being tortured, electrical, burning, fire, ice, needles, and I'm not able to use left hand. Most days to be able to "function" is i put myself in my brain. My left side of my face including my nose, eye, forehead, and top of my head are numb and same pain as hand. My left leg too. This is zero quality of life. I never knew when you had a stroke that pain was involved. I could have deep brain surgery and then brain surgery but the percentage of success is not good. I've been told, sorry you having a stroke in your thalamus and it's damaged, it's a bad outcome. Life expectancy also changes due to this horrid torture pain and zero functioning of life. "Faulty brain processing" is laughably close minded, ignorant and imprecise. If you are GABA-deficient, it's no wonder it is going to be the case of overexcitation and cross-wiring to code for widespread pain in the primary somatosensoric cortex and thalamus. I'd thus expect benzos and gabaergic drugs, perhaps even low dose propofol to have an effect. If you lack electrolytes (hepatic/nephrotic insufficiency, happens quicker than you think), muscular pH decreases, and acidic muscular metabolites and prostaglandins excite sensory neurons. Autoantibodies, even antinuclear ones, may act on neuronal surfaces, causing cytokines and COXs to irritate. Finally you are left with structural abnormities, like whiplash, ankylosing spondylitis, or slipped disks. But these are rather more obvious.

This is one of the better presentations I have seen on the increasing understanding of chronic pain. This doctor speaks with an experienced common sense as a doctor who has dedicated himself to treating this disorder. I have struggled with this for nearly 20 years now and am finally understanding it. It has taken its toll on my life but I have stayed positive and hopeful in spite of it. I am finally getting improvement after a very long disability and much frustration at the hands of perhaps well meaning doctors ( I am being generous and charitable here ) and this doctor is one of very few who after listening to him I would strongly recommend that you take his advice to heart. Cannabis is a crap shoot. I don’t enjoy the side effects but now and then it is a short vacation from misery. The problem is that their is much experimentation required and its an up and down thing and you end up stoned. This isn’t good if you are raising small children or need to drive or operate machinery. It can be helpful to reduce the intensity of pain and relax spasming muscles and lift mood but it is just one tool and it can kick your butt if you aren’t careful. The Mayo Clinic video is good and there are some from Australia that are good too, (Moseley?) More and more people are being enrolled in pain programs now which is good. But we have a ways to go yet before there are effective treatments for most people and many doctors don’t want to deal with pain patients at all especially after they were blamed for the opioid epidemic. I have no doubt that the stresses of our modern lives are causing many of us who have a predisposition to chronic pain either by genetic predisposition or significant traumas or long term stress or stroke or spinal cord injury or head trauma or abuse as children or sexual trauma or combat trauma or, or.... that there is a rising epidemic of stress related neurological disorders in our culture. Actual successful therapy is still very much unavailable and a type of meditation technique is mainly what they now offer. Good luck getting a brain MRI to find out though if there are any obvious brain abnormalities. In the 80’s and 90’s people who were diagnosed with fibromyalgia were called everything from hypochondriac to malingerers. A lot of people have suffered a lot with little sympathy from friends, family and doctors. Hopefully this is changing now. To get peer reviewed studies and articles as a non physician is still difficult and even if you can get access it is expensive. Also doctors turn their nose up when you suggest a diagnosis or test or internet article. So if you are experiencing symptoms of chronic pain and other unexplained symptoms I suggest you first google Dr Sarno and tension Myositis. Then google Mind Body Syndrome and Dr Shubiner and check out the symptom list of over 30 associated symptoms. Then Google Central Sensitization and find the symptom checklist for that disorder. It will take a little time but you will begin to see that you may have a “wound up” central nervous system which is the main culprit in your multi symptom sufferings.

I never got to a dose that allowed me to have a life but I did get some relief for a few hours out of the day. I don't expect to be pain free but I do expect some relief which medication can provide. I don't want to live in constant torture for the rest of my life so I will be one those pain patients that was driven to suicide by the government,FDA,CDC and all those who supported them in demonizing opioid pain medication.

He’s only creating more misunderstanding and confusion in the medical field. I’d love to see this dude make it through one day of intractable pain I bet his position would change real quick. He’s causing inhumane care and neglect of pain patients who need help.

I had it my entire adult life and it was never treated. I suffered terrible menstruated cramps. At age 61, I required a laparoscopy and the gynaecologist found endo on my right ureter.
