Community Collaborations: Addressing Health-Related Social Needs in Indianapolis

In 2017, Community Health Network Foundation received a $2,560,494 grant from the Centers for Medicare & Medicaid Services to address unmet health-related social needs on the east side of Indianapolis. Under the leadership of John Kunzer, MD, and president of Community Physician Network, we’ve used initial funding to start tackling housing instability, food insecurity, utility limitations, interpersonal violence and transportation access.
