hello, my doctor doesn't do intraarticular injections , he just injects into the ligaments and maybe some tendons. (he injects at 7-8 points at one knee for all i see) When I asked, he said that 25 % of the whole solution injected goes to the intraarticular area. Is that true and enough?

Hi, informative video on this case and treatment. Why didn't Dr. hauser use imaging guidance for this treatment?

Hi Daimion, The knee is one of the easiest, safest joints to treat via palpation of anatomical landmarks. It's the joint that most providers begin injecting in their earliest days of learning Prolotherapy (before feeling ready to move on to the spine, for instance), and Dr. Hauser has treated tens of thousands of them over nearly 30 years. Prolotherapy has been done since approximately the 1950s and doctors who have been doing it for decades originally learned it through dedicated study of palpation of anatomical landmarks and later incorporated ultrasound, fluoroscopy, etc. as those technologies became available. This patient has received Prolotherapy over the course of 20 years for numerous injuries, having experienced the treatment years before imaging guidance was used every day in Prolotherapy practices, and she prefers it only be used if truly necessary, like an upper cervical spine treatment for instance. Based on the specific history of her knee pain, there was not a suspected tear or anything that required cells to be placed in the exact area of a tear, required guided nerve release, etc. where ultrasound would be a preferred tool to use during the procedure.

Hello.. I am having grade 3 chondromalacia in my Right knee.. its was shown in my MRI on 27th Jan 2021..Can you please advice if prolotheraphy can help me out on this?

Hi Vipul Dere, Thanks for your question and sorry to hear about your recent MRI findings. Yes, we help a lot of patients who were diagnosed with chondromalacia. In our office, MRI findings are only part of trying to find what is really wrong with the joint and more importantly, how to resolve it. We do motion ultrasound to see the level of instability and other findings when the joint is being stressed, and physical exam testing to mimic real life, as well as an overall review of a person's medical history. Once we have a more through knowledge of these things, we can develop an individualized treatment plan of what type of Prolotherapy and exercises to help someone get back to where they want to be and resolve their painful symptoms. How effective and how long does the treatment last? How long is the recovery from the injections? When can she return to normal activity? When can the person begin to see results?

Hi DX GI, Great questions. There are many Prolotherapy resources online that go through this and more. In general, the treatment series is usually 4-6 total and the goal is to stimulate the body to repair the painful areas/damaged tissues. The person can do the normal activities of daily living but exercise or intense physical labor would not be recommended for a few days after the treatment as the person would build back up to what they can tolerate, letting pain be their guide and possibly working with a physical therapist or other trainer or fitness practitioner who can assist them with better form/posture and optimizing the results. Some people begin feeling better right away after the first treatment, and others may actually take a few treatments to start seeing a difference, depending on the degre of the injury and their overall activity level, etc. It is not an overnight treatment. It triggers the healing response in the body and that works over a period of days, weeks, and months, just like healing any other injured tissue (such as a cut on the skin). Hi Dr. Al-Abbasi, the same traditional dextrose Prolotherapy solutions that are used in our research and books. That is the base and we individualize solutions from there.

Would this exactly procedure that was done be more successful if you used prp vs dextrose for knee stability ? I’ve asked around and many clinics say prp is superior to the dextrose injections. What’s your take ?

Hi Jessi, Thanks for the question. We get asked this every day by nearly every patient. We address it in other videos and webinars and will have some more updated ones out in the coming months to try and address it further as well. We use PRP here, but it is always in ADDITION to dextrose Prolotherapy. We call it PRP Prolotherapy or Cellular Prolotherapy. Thus, in a treatment to a knee, the PRP and/or stem cells may be injected into the joint and/or a ligament or tendon tear, then dextrose solution is done at the supportive attachments around the joint. We find that doing it this way is more comprehensive, especially for more complex cases. While we are encouraged and happy for anyone who has success with one-shot PRP or PRP-only treatment, our clinic takes on many patients who tried these techniques already but found it wasn't having a strong enough healing effect to get them back to where they wanted to be. So, again, if it works for someone, wonderful! If not, unaddressed joint instability is likely the reason and a more comprehensive provider technique might be considered. @@CaringmedicalProlotherapy have you tried injecting prp at the supportive attachments instead of dextrose ? Does the dextrose injection elicit a different immune response vs prp? Let’s take quad tenindosis for example with no tear (based on a 1.5 T mri). You would inject dextrose into the supportive attachment and because there is no tear based on mri, there would be no need for prp ? Thank you !

Hi @rogerkim5994 - we do both stem cells and PRP treatments in our office. The type of treatment really depends on the patient and their needs. Not all patients need PRP or stem cells, many are cured of their instability and pain with dextrose solution. Hope this helps!

Hey Ross, I'm really but I can't do the activities that I used to do, yoga class, man's time, sort of. I really need need to restore my body and need to get back in the activities like I used to, I'm so active and my body hurts, I need to release pain
