In this video, I discuss some hard truths about the ResMed MyAir App and the AHI index that contributes only 5% of the MyAir Score. Imagine if you were a diabetic and your blood glucose levels made up just 5% of your daily diabetes score. It doesn't make much sense. Link to the research paper - cpap.review/ahi . Great video but please can you reduce the amount of AI generated imagery used in your thumbnails. I think it has a negative impact on your credibility and makes your videos appear as clickbait rubbish rather than good informative content.

I am a 72 year old and have been using Resmed for 15 years give or take. I started on an S8 or S9 prescribed by the NHS in London, St THomas Hospital, the premier sleep hospital in London. I was diagnosed with about 300 ahis per night. The NHS reviewed my machine numbers every 6 months and were very happy with 16 ahis per hour. My machines were free at that period. When I left the UK when I retired, I had to purchase my own machines. It can become expensive. I bought an S10 auto in 2019 and it lasted for 2600 hours. I took it apart based on your instructions, thinking that it would be very dusty and that this had caused the compressor to malfunction. It was as clean as a whistle. There is no place here in Thailand that can help me to trouble shoot the problem. There was a video on youtube which was pushing the idea that you should try and breathe through your nose as much as possible. It sort of made sense because the people pushing it said that your nose filtered and heated the air you breathe naturally. It made me think of taping my mouth shut because I was told I was a mouth breather in sleep. I was using a full face mask which was giving me 16 ahis an hour. When I taped my mouth shut the ahis reduced to 6 or 7 an hour. I told my brother about this and he told me that his ahis were 2 or 3 an hour using a nasal mask. I changed to a nasal mask and some of my ahis went to 0.5 to 1.5 an hour. The tape I use is a 3M product with Nexcare and Transpore as the registered trade marks. I dont know why it has two, but who cares. I have written to St Thomas Hospital by email to advise them of this but got no response.

If I understand you correctly you are saying that Resmed have programmed their machines to ignore apnoeas or hypopnoeas because of some arbitrary threshold, even though in reality they are quite obvious events. Does that cause the auto machines to not respond to apnoeas the way they should. I started to take oximeter readings recently when I get up to pee at night. If my oxygen levels were being affected by my apnoeas, how long after waking up would my oxygen levels take to return to my normal waking levels. I noticed that you commented to a user that said he relies on his O2 reading and that as long as it was above 90 he was happy. Your response was "Good on you mate" This is incorrect. See the item below taken from "emedicine health" Oxygen saturation levels (SpO2) between 95 to 100 percent are considered normal for both adults and children (below 95% is considered abnormal). People over 70 years of age may have oxygen levels closer to 95%. Normal oxygen saturation levels (SpO2) are between 95 to 100 percent for both adults and children. My waking level is 97 and that is what I am seeing when I wake up to pee. How can I be sure that this is also the level when I am sleeping. I cannot afford a recordable meter. Or how do I get a meter that is recordable and not a fake. So much stuff in Thailand is sub standard.

I think part of the problem is that so many medical types are more interested in Compliance - if you used it for the proper number of hrs each night, than the number of events one has.

Compliance is all that matters for most medical companies. Even pharmaceutical companies are wanting compliance for their users (WHO/WEF). They don’t care about your health, just profit.

Incredible job you (and team) have done on this analysis! One thing that I'd like to confirm, as it's not 100% clear to me, that is, using the ResMed AirSense 11's data, are you saying that only what we see on MyAir is the problem, but the actual data saved to the SD card (raw data) is good? When I saw how your system seems to be able to display accurate info, I have to assume that the AS11's raw data is accurate, in and of itself. Only when it's sent to the ResMed servers does their MyAir software "dumb it down". Am I right on that? ( I hope I said that clearly. LOL )

You make GREAT videos! Are you planning to make a 'detailed' video for us lay CPAP users on how to read the flow rate and identify which are actually OA, CA, H, RERA or just happen to wake up in the middle of night (breathing while awake)? This way we don't look at MyAir results and somehow think we're doing well. Would really find such a video useful and sure others would, too. Thx! Don't think any other "CPAP discussing" KZbinr have made such a video.

Another informative video. I wear a Pulse Oximeter every night. I find there is little correlation between my Ox saturation and the Resmed score. I get the "excellent" every night. How long I'm on the machine also seems to be part of their score algorithm. Thanks for these videos.

One problem I see. As one ages, especially if they have Asthma or COPD, they have more difficulty exhaling against that pressure. Thus, we end up with O2 support.

The nightly rating system is directly tied to the length of sleep youve had. I have slept for 4.5 hours and had little to no events and my score was like 70%. Then I slept for closer to 8 hours and had significantly more events and got a 100%

I've never worried about the % score and looked at the individual categories because I want to know how many events I had, my mask seal and if I pulled the mask off during my sleep. Oh and how long I used it.

This definitely makes sense to me. My Dreamstation would give me around a 3 AHI and when I replaced it with an Airsense 10 because of the recall, my sleep has felt worse ever since even though the Airsense 10 also reports 3 AHI.

Hey mate, that's interesting and i've heard similar feedback from a bunch of other DS users. Check out that vid i did on the auto algorithms. Your sleep likely felt worse because the ResMed devices generally provide too much pressure which can cause a lot of sleep disturbance

@@CPAPReviews (so sorry, this got long winded)... sigh.... I was about to ask a question on AS11 pressure, but your statement may have just answered that! Here's what I'm noticing here with my husband's setup. He uses the N30i (nasal cushion). He did try the nasal pillow, but he found that it irritated the inside of the nostrils). So what I'm noticing, is that as he repositions himself during sleep, sometimes one arm of the mask moves a bit, so the nasal pillow moves out of alignment a bit and air is escaping. As this happens, I can hear the AS11 crank up the pressure. I'm assuming that the AS11 "thinks" that breathing has stopped or gone shallow. I don't know for sure. But what I'm curious about, is, that if it's possible that this could cause incorrect numbers to be observed by the machine? When this happens, I'll try to adjust the cushion and hubby usually just repositions again and the cushion is back in place. The AS11 then reduces back to normal pressure. Could this be part of the problem with inaccurate numbers? Just something that you might find interesting and probably are already aware of. So for him, his numbers (as fake as they apparently are) are usually .5 to maybe 1.5 ish per hour. HOWEVER, on 13Dec2023, he has a pacemaker implanted. (left side) Of course, until 6 weeks has past, he has to keep his left arm inside a shirt at night and mostly sleep on his back. Sometimes on the right side. I've noticed that his numbers JUMPED up to as high as 4.5, which never happened before the pacemaker. I'm not suggesting that the device is 'causing" more apnea, but perhaps it's being on his back more that is causing more apnea to happen. ?????? Does that make any sense at all?

What myair needs is 2 "scores". A score for usage and a score for AHI. Not make the AHI part of the usage. As to machines not recorded events I have a day where I had the settings wrong showing like 1 AHI but there were 90 events per hour. But all the events were to short for the machine to read as events. Like my breathing chart was scary

Thanks for your feedback buddy. I think they need 2 modes! A beginner mode and a Advanced mode and it just changes the category score weights. IE the 'events per hour' changes from 5% of the score to 50% etc. Cheers

I can't stand the CPAP medical cartel. I've been using CPAP for 5 years, I'm in excellent shape, don't drink, don't smoke, use excellent sleep hygiene and yet I've gone from mild to moderate apnea and all I hear from my myriad of doctors over the years is it isn't unusual for apnea to get worse even with good "compliance" and I'll probably be rated as "severe" My last doctor asked me what I thought I was doing wrong as to why I wake up tired. I think the majority of the CPAP industry found a "solution" to a problem that doesn't exist.

The MyAir data is only good for taking to my GP every couple of years to renew my medical certificate for my license. That's all I've ever used it for.

Thanks for the Facts. That exclamation of: "Excellent Results, John!", and the S10 (with it's 6 or so bias) gives me a score of "83". I wake up feeling like an underfed oxygen vampire. From now on I will stick with Sleep HQ readings. Is there no relief from these CPAP manufacturers? Maybe it's rocket science. And maybe we walked on the moon.

Yes we could. It might cause some regulatory issues for us when we do though so for now we just display the raw data

Strange given that what I have read(though granted internet research is hardly 100% lol) is that sleep apnea is massively underdiagnosed/untreated and its very unlikely a cpap machine can do someone any harm, seems like they should be handing them out like candy.

When I used the scores from MyAir I just gave myself a gold star because I thought the circle graph was an accurate way to represent the success of my therapy. I’m afraid if there was just an overall score like MyAir app some of the users of SleepHQ would not take a deeper dive into their health.

Okay, so I am legitimately worried. I tend to get 0-.3 events per hour, but I wake up feeling awful and it has been almsot a hear. Two sleep doctors have told me I am doing great and keep going.

So you are saying if my blood oxygen is at 94-98% while I sleep my 2.1events per hour would actually be between 5 and 7? If this is so far off and this information is public knowledge why aren't our sleep consultants telling us stuff like this?

Hey mate, Thanks for the great info.I have a F&P Sleepstyle machine.Any idea how accurate these machines are?I have had a situation where the Physician and his staff have been very reckless and uncaring. Saying basically that I shouldn’t be concerned about the high readings and review after a year. When I asked to have my levels recalibrated I was basically ignored and when they did adjust the airflow, they took it from 9 to 20🤬. Since then I have experimented and adjusted a few times to get my AHI down from 12 to an average 6.

Hey buddy, it's likely a negative bias but i don't think it would be as bad as ResMed. Not Sure if you've seen my vid on the F&P auto algorithm but it's a good one! - kzbin.info/www/bejne/rGfCf3uhjcyGiLc . Just make sure you don't have the 'sense awake' feature turned on as that will cause more harm than good. Congrats on your AHI drop legend! You're not the first person to get shit treatment from their Physician and you'll likely get better therapy and sleep if you monitor your own results and adjust your own pressure levels.

Interesting video. I have an Air Sense 11. From your video I understand that the correlation is not as good as we'd like to think but the facts are I have stopped snoring completely and my partner is not noticing that I am having difficulty breathing during my sleep - not enough to wake and disrupt her sleep at least. So you've identified a problem with the machine missing some H points where it didn't kick in but I'm not sure what the aiim of the video is, apart from pointing out that there's an inherent inaccuracy compared to the 'exam test'. I was waiting for the 'solution bit' i.e. increase some settings to increase the pressure differentiation etc or is there no manipulation of the device settings that can help the machine increase it's air pressure when you start getting airflow issues, earlier than it does now?

Well, yeah I knew this already from using my pulse oximeter ring. My ResMed 10 doesn't catch half the real apneas. I used the ring to adjust my pressures to have a very narrow range and now I have 5 apneas on a bad night and my airsense 10 will show a .3 or something. Surprisingly mask fit and comfort makes a big difference too.

Useful information. I already knew what my CPAP reported was not fully accurate. But comparing the numbers to previous readings on the same machine at least gives me some indication of how good or bad my control is. I wonder if the mask used could have some effect on what the machine senses? I've had my Airsense 10 since late 2015. Do the makers of these machines offer firmware updates with improved algorithms?

Hey buddy, the answer to your question is yes they do which is why you can select a mask type in the machine settings. It helps the device adjust the pressure at the mask end as masks like pillows and nasals have higher resistance and need higher flow to maintain the pressure generated at the cpap end. You can't update the firmware on the AS10 but they introduced this feature with the AS11. Cheers

Here is a problem with your analysis. If someone legitimately scores a 2 or 3 with an Airsense 10, you are suggesting that is probably more than 10. That may be true for some people, but for others the 2 or 3 might be totally accurate. Your conclusion that MyAir is worthless is not changed, only the extrapolation from that to your statement about 0 likely meaning 10.

Yeah with low numbers I see what you are saying. A 0 from the Airsense doesn't automatically mean 10. All you can say is that its somewhere between 0 and 10. But what about a 2?

I don't know about that mate, i did say 'based on the averages' and i also clearly demonstrated that the hypopneas were the cause of the issue etc. I'm pretty happy with the video and the take home message. Cheers

I have a ResMed 11 and have used your site to verify that just like in your example, there are hypopnea flags that aren't being properly flagged, making my apnea numbers appear lower than what they actually are. I was really hoping for a solution here but it just seems like there is none, since even if I increase the pressure, if the machine can't accurately flag a hypopnea then how will it know to increase the pressure. I really hope you do a follow-up on this assuming there is a solution. Currently, I sleep with a neck brace to keep my neck straight and keeps my chin from dipping down, and make myself breathe deeper when going to sleep as I have a habit of shallow breathing which has helped but if there are other ideas/solutions please let me know or make a video. Thank you!

Well Myair and Oscar and sleepHQ gives me the same results so what ahi Reading do I believe . If only they were better reliable scoring charts ...maybe built into a easy to access web site or App ❤

I rely on my smart watch to give me my sleep numbers, cycles and sats. My sats have improved from mid 60 percentage to late 80's and early 90's. Happy with that, which means my apnea isn't as severe since starting the cpap. I use the resmed now only to check the smiley faces at a glance without needing to put on my glasses 😂.

I'm fairly new to CPAP therapy but I've always taken the results from this app with a grain of salt. I work in information technology so I noticed fairly early that the results weren't exactly super scientific.

Great video. Do I understand correctly that SleepHQ just takes the identified flags from the AS10 (in my case) and displays the results? If this is the case we also need to add the fiddle factor to the value shown in SleepHQ.

Hello my friend, and thanks for using my platform. Right now we just chart the raw data from the cpap machines including the CPAP generated respiratory flags and AHI but all this will change in 2024. I have been using cpap for about three months and got my AHI down to about 5.2 unfortunately my airsense 10 developed a fault it was sent to Resmed and took six weeks to repair, the first three nights have been a nightmare with constant leaks and an AHI score around 18+ ! After a six week break is this normal, I feel like all my perseverance and patience has been for naught ? Cheers.

I have the airsense 10 auto and the EPR feature is an issue. It acts more like a bipap and then blasting me when I breathe in. I think that this feature is messing with the events along with other things. Turning it off actually fixed so much. Thankfully my RT is more concerned about events than air leaking or the actual score on resmed.

Help! I bought a second hand machine as I was desperate and a new heated tube and it doesn't detect that it's been plugged in. Gutted because the way I paid for the machine I paid as much as a new one 😭

Damn I have an airsense 10 and am mostly getting around 2. About the gape widening, I don't see that it matters to much. Turning a 60 into an 80 doesn't seem nearly as bad as turning a 2 into a 14.

Not sure if you've seen my vids on ResMed EPR but this could have something to do with it. If you have EPR on it will dramatically reduce your Apnea control and often increase your AHI. The Philips pressure relief is different. You can check out that vid here - kzbin.info/www/bejne/fZK7cmOQfrydZ7s

No so much a scam but the game is somewhat rigged. 1 The CPAP machines under report the true AHI and then the Apps scores are weighted in a way that makes it easy to score a high number. Cheers

Not really, the AHI has serious limitations and as per the video, the numbers not very accurate anyway. Bloody oxygen is the best! Cheers

Depends on your age, for instance i am 72 and my percentage oxygen saturation waking is 97. For good sleep, that percentage should be maintained. 95 is average for my age.

Please correct my thinking --- if the black line in the graph beginning at 4:55 is the actual AHI, and the red line is the AirSense 10 reading, it seems AirSense is reporting too *high* rather than too low.

Its not the red line itself. Its how the red line effect the numbers given by the green lines when added at 5:15. On the left your air sense says AHI of about 16, you run a horizontal line to the right until you get to the red line then go vertically down to the bottom line and it says your airsense 10 AHI of 16 is actually more like an AHI of 28. Make more sense?

I can't find this Sleep HQ app on android I...normally just use my air and that's it and it shows me normally having like 1 event all night but my doc said I originally quit breathing 3 times a minute

There's no Android app yet but the great thing our platform is it's web based so you can access it from any device that has a browser (ie safari or google chrome etc) . Good luck

I'm no pro on that, but I do understand the technology and what the difference is between one that ACTUALLY works and the ones that FAKE the numbers. After doing a lot of research and not wanting to spend hundreds of dollars, I ended up buying one that is branded *"Braun BPX800"* series. I ran a bunch of tests with it, at one time, using the data from the FitBit watch I had, but the best was comparing it to the oximeter at the hospital! It was bang on! Well, I mean, there's always going to be a slight difference of a digit or two, which for us is fine. I got rid of that horrible FitBit junk and bought a cheap junk smart watch. LOL The oximeter on that is one of the FAKE ones. You can tell, because it'll always read 98% and above, but usually 98%. So obviously, I don't rely on that. I've tested this Braun one too by breathing out, holding for a bit, then shallow breathing. This reduces oxygen. This Braun shows the numbers in real time. In this test, I had it register as low as 89%. Then I just start to breath normally and the numbers move up to where they should be. This was not too expensive either. And what I like about it, is that when you press the button, if you press more than once, it will re-orientate the display so it can be read in any direction. Hope that helps.

That doesn’t look like an actual app report. It looks like it was doctored. I use the Resmed and this app and it never gives me an excellent with these kind of numbers.

My Air Sense 11 only updates if I unplug it, it may connect. Has not made a connection all month. I do look at the LCD every morning. Need to get into the graphs on these programs. OK, the study. Lots of info on the study not included. Sample size, number of people, number of machines and for which company. Medical conditions of participants. I could go on and on on what makes a good study! Was this study Peer Reviewed? How many other studies got the same results? I'm inclined to believe the machines suck at reporting, but I want to see a real study. That means large cohort, randomized placebo controlled and peer reviewed study that is repeated.

Unfortunately, you dont have any clue about how a medical study should be conducted to ensure the results are real and accurate. Did they test one machine on one person each? Not a large sample. Did they test a thousand machines several times. Sample size is perhaps the most important thing to do and to know to ensure valid results.

I had not seen the link and it is helpful. But my concerns remail relevant. Did they test one Resmed 10 machine? Perhaps it was defective or out of spec. Did they test 100 machines and take an average. Many times in medicine results can be very different between in vitro studies and in vivo studies. Someone should design a real medical grade study using real people and have it peer reviewed. So far, in my 5 months on CPAP no one has showed me a real study. Not that these results arent concerning, they are. But we need real studies. @@CPAPReviews
