Spad risk 1 can be avoided by everyone in the cab following the ‘clear to stop talking’ rule. This applies to North America too.

Generally speaking it's 3 strikes and you're out. 1 SPAD means pulled off active duty and a serious talk with your driver team manager. 2nd is same but perhaps a more serious talk and 3rd would be demotion or getting your books. Obviously individual circumstances of the SPAD play into this. If there are a number of mitigating circumstances such as lighting, sighting, technical failures or error on the part of others then the consequences may be less severe. If the SPAD is down to gross professional negligence (such as intoxication for example) then you can bet you will be sacked there and then.

Depends. If the driver didn't know about the danger signal, (e.g. if he knocked down a signal and while it was red another train entered the block), and continued into the next block with the other train still occupying, there is a big risk of collision. Otherwise, you could be fired.
