I hope that this is a helpful video about light-adjustable lenses, my new favorite lens to offer patients in order to customize and fine-tune their vision AFTER surgery! My apologies for the sound quality for this video - my mic broke during filming and I was unable to edit out some of the rustling and background noises. I'll make sure to fix this before the next video.

Is there any evidence that the lense can be disrupted by ionizing radiation? I’m an X-ray tech and there was some question by my eye Dr about it being a problem.

I am almost 75 years old, and have had the LAL lens installed in my right eye over a week ago, and will have the LAL lens installed in my left eye this Thursday (18 days apart, which is common). So far I am impressed with the results. I will probably have new glasses just for reading and looking at the computer screen. The UV treatment after the surgery will be done once a week for 4 weeks. I was awake during the surgery. It did not hurt, since I received a lot of numbing drops for the eye and a sedative through an IV. Thank you Dr. Lee for all of your clear informational videos.

Hi Briane. I'm considering the light justable lens myself. Are they going to do mono vision On you where one Eye is set for 2020 distance and 1 Eye is set to be a little nearsighted so that you can have some intermediate vision for computer use or near vision so you can read or see near?? Thx

Hi otie. Did you have the light adjustable lens? Do you have any intermediate vision for computer use or any near vision for reading uploads? Or have they done a mono vision by setting one Eye for 2020 distance and 1 Eye a little nearsighted so you can see near forreading? Thx

Hi otie. Did you have the light adjustable lenses implanted? I'm glad to hear you have 20/20 far vision. How about your intermediate or near vision? Are they adjusting one Eye a little nearsighted so that you can get some intermediate or near vision making it a Mini monovision so you can see nearer? Or are you just going to have both eyes set for 2020 far vision.? Thank you

I just have cataract surgery on my left eye 5 days ago and I didn't know anything about LAL lense until today and wonder why my surgeon didn't mention it to me. My cataract surgeon suggested Vivity lense (with Toric for my astigmatism) to suit my eye condition as I also had lasik surgery 15 years ago and I'm a competitive swimmer and not taking ANY medication. After 5 days I start to see everything (driving, TV, computer) so crystal clear and I'm surprised I can read without glasses except very tiny fonts. I'm going to have another surgery for my right eye next week and I'll definitely ask him about LAL. Thanks for the video Dr. Lee.

We have to be thankful for Doctors like Michelle Lee. I got Catarac Surgery with Clareon IOL yesterday. This was due to a Retina detachment I had back in March of 2023. My right eye had been foged up for a while. I now see clear. It's kinda of strange since I had got use to the eye with the cataract. Now im in recovery mode.

I guess I am the only one who is not happy with my visual acuity after LAL implants. I was encouraged to get LAL due to my previous Lasik. Post final LAL lock in, I have 20/35 in one one eye and 20/40 in the other. Doc said that's the best he could achieve for me. Close up vision is pretty good, but distance is very soft focus. However, I was seeing better after the adjustment and before the final lock in. The lock in procedure definitely had a negative impact on my vision. I haven't been able to find any reviews out there similar to my experience. But for the huge extra cost incurred for these lenses, I was hoping for better results.

Thank you, Dr. Lee, this is so helpful. I am scheduled for cataract surgey this month and originally chose the Vivity lens, but I have been hearing about the LAL and it sounds amazing. I’ll consult with the surgeon next week to see if I am a candidate. I’m not seeing a lot of downside with this lens. Thanks as always for the information you share!

Thank you for this video Dr Lee. I have had LASIK surgery twice the second time I was given monovision and have enjoyed it. I will be getting the LAL in my right eye on 7/11 for far vision and in my left eye on 7/18 for near vision. I am excited and looking forward to both surgeries. I hope my vision great with the initial measurements but happy it will be able to be adjusted after the surgery as necessary

Hi rx dude. May I ask that instead of the light adjustable lens set for amano vision did you consider a multi focal lens like the PanOptix? Do you know how much they will offset the eyes to give you the mono vision? I have heard anywhere from -.5 to -.75 (1/2 to 3/4) quarters diopter nearsighted is necessary. I've never had monovision and and afraid I won't be able to adapt to it. Was adapting difficult for you when you had it? Thank you for your time and help. Carl

59 yr old male, in great health. I had this surgery July 2023 and my vision has been ruined. My eys will not adjust or work together (one near, one far} and the blur or cloudiness has never gone away. Any suggestions going forward? Vitrectomy? Remove and replace seems very risky. Scottsdale, AZ BDP eyecenter.

Hi Dr. Lee, I customized my Snellen, by going from 20/50, to 20/20. Took me two years. Refractive change, from -0.75 Diopters to +0.5 diopters. Thanks for your advice!

Not sure I understood correctly. With a LAL installed you can not take any of the medications listed? Not sure what all those medications are used for just concerned if in the future I'll face the choice of medication to cure whatever OR loosing/effecting my eyesight.. Also how much UV does it take to effect these lenses. Being on a boat on a sunny day without sun glasses enough to tweak the adjustment of the lens?

Dr. Lee, I have watched a number of your cataract videos and have found them very informative. The one thing I would ask you to consider making is a video indicating what you deem to be the price ranges of the various IOLs. I fully understand this will vary depending upon a given practice, its geographic location within the U. S., and, perhaps even in the same city. Out of pocket costs for most Medicare patients is an important factor. The standard IOL is covered by Medicare but not other newer lenses to my knowledge or related ancillary treatments. Thus, I believe many of your viewers would find this information to be useful. Thank you for your consideration and keep up your informative videos.

Thanks for this info. -I need cataract surgery and you mentioned several issues with LOL lens implants that weren't mentioned by my eye doctor and his office. They knew I take one of the medications on your no-no list, but didn't mention the possible complications of that with LOL lenses.

Hi Dr. Lee, Thank you for your informative video. I went to my eye surgeon today to do measurements for my cataract surgery. She suggested I choose the LAL+. When I went for the consultation a couple of months ago, she said monofocal lenses with blended vision would work well for me. It turns out that she just started to offer LAL like this week. I was wondering it is reasonable for me to ask her if she had training on how to do LAL adjustment.

I am 36 and I have been already had keratoconus on my left eye since 2013. I got used live with my kc with the help of toric lenses and my right eye has already been seeing well but at december 2022 one day in the morning I relized that my right eye has very very cloudy. I made a small research about the sympthoms and I concluded that this is PSC(Posterior Subcapsular Cataract). I also went to a doctor and the doctor also confirm that it was PSC. As time passes it progressed some more and now I almost couldn't read the black writing on a white paper and I almost couldn't read the computer screen in light mode. Since my left eye has already kc up to having PSC I have been using my left eye and right eye was only helpful. But now if I don't put my special hybrid lens on my left eye, the vision is terrible especially sunny days... I don't know what to do and what to choose!! I have doubts if catarct surgery really satisfactorily correct my vision and provide me my previous vision or will I be disappointed again after suregry? I even don't know if the surgeons can completely clear the blurry parts without defracring the posterior capsul?? Many questions???!!!

Is the halo effects of a LAL more or less than that of a basic one focus (distance) lens?

Theoretically it should be a little less due to the material used for the LAL lens.

My vision has naturally been 1 close/1 far & I didn't need glasses til both got worse with age. My dr won't do monovision surgery with basic lenses but suggests LAL. But LALs are SO expensive. Is it possible to do a basic lens in one eye & LAL to match it in the other. I canceled surgery until I figure this out.

Curious of your comments; I just had the ALA lens installed in my right eye. Not sure I'm happy with this lens. I live in NC and recently been playing golf in the cooler weather. On those cold days (40-50degrees) I've notice that the lens is out of focus all day. Is this common and will it get better over time. Appreciate your response - thank you

Doctor are you able to achieve much intermediate or near vision with an LAL without going to mono vision? Or is vivity a better option even though the power would be 1/2 diopter near sighted? Appreciate your help

Thank you for your question! We do a lot of RK patients here in the Phoenix/Scottsdale metro area since there were a few surgeons who did high-volume RK in the 80s and 90s. The light adjustable lens can be an excellent choice for RK patients since many times the lens calculations can be challenging. Hope that helps!

I've heard that LOL can also correct for astigmatism, and be modified to offer EDOF. Also, that a significant percent of fixed lenses end up with the wrong focal point due to the lens not ending up in the location that was expected. Any comment on this?

Hello doctor, my cat slightly scratched my eyelid about 10 days ago. It didn't cause any pain/blood or any other problems in my eye . I can see things clearly when looking with two eyes open but when I try to see with my injured eye ( other eye closed) I feel a little blurry from a distance like I can see the hands of a wall clock but the digits look a little blurry . Other than that I don't have any other problems .But I think the bluriness problem improved a little in the week, it was my non-dominant eye(left eye) . Should I be worried?

Secondary cataracts (opacity of the posterior capsule of the lens) sooner or later develop in almost all patients who have undergone lens removal.

Yup. It sucks! The younger you are the sooner you'll get them. Fortunatley, for most folks it can be"zapped" away with a laser in your doctors office.

@@bluejay9890 Okay, it can be DESTROYED by a laser OR another tool - THAT'S NOT THE POINT. 1) This is the tissue of the eye, BUT NOT THE BUTTOCK - THIS is the first thing. Secondly, behind the back wall of the capsule in which a HOLE IS MADE with a laser due to secondary cataracts, the TISSUE in which the OPTICAL NERVE ends (EXPOSED) is anatomically EXPOSED and this leads to COMPLICATIONS ALREADY WITH THE OPTIC NERVE IN GENERAL OR access to the nerve “chest to chest - face to face” .... etc... 2) ATTENTION! , if 100% develop a secondary cataract, THEN IT WOULD NOT BE SAVERER AND RATIONAL, without removing the PRIMARY CATARACT, to JUST make a “hole” in your own clouded lens and USE GLASSES. ....IF there is no penetration into the capsule and implantation of a PIECE OF PLASTIC INTO IT, then there will be no secondary cataract. .... SECONDARY CATARACT, this is a consequence of mechanical IRRITATION BY PLASTIC OF THE REAR WALL OF THE CAPSULE (layering of fibrous tissue, clouding, etc..) ...BUT !!! JUST A HOLE IN YOUR OWN LENS - IT'S SIMPLE AND YOU CAN'T MAKE A LOT OF MONEY HERE.... P.S. my right eye “turned off” and I would need an operation to implant PLASTIC IN THE EYE and I am not a young man or even an old man of my first youth - I am an old old man with good knowledge in the field of BIOLOGY OF OUR HERD. .... I am ready to wear glasses (I have had them on my nose for the last 20 years), BUT TO CREATE A PROBLEM for myself with my own hands and “spinning” around EYE SURGERY AND IMPLANTING A PIECE OF PLASTIC INTO IT, etc. - THIS IS NOT ENOUGH FOR ME YET... ....let them make a hole in my clouded lens - I would do this without any hesitation.....

Hi Dr Lee. Both of my eyes have cataract. The Left had Retinal vein occlusion as well. Since I was a small child both of my eyes are really highly light and touch sensitive. And the worse part, whatever my eye prescription are I would be so dizzy wearing eyeglasses. Wearing it a day or two, my eyes refuses to adjust. Prescription had to be lower than the right one which is now a bit blurry, in order for me me not to feel dizzy. I also kind of like a different iris color. Is there a Version of this being use for permanent eye lens? I'm willing to travel to your state for the procedure. Thanks.

Thank you for your question. It depends how much astigmatism, whether it is regular vs irregular astigmatism. You would need to consult with your cornea/cataract surgeon about whether this lens can help you achieve your refractive goals. Best of luck!

I do not have macular degen but my mom has severe wet, and my dad has dry. Do you think the LAL UR treatment will make me more susceptible to the disease ? I'm 54 and need readers but just minor correction for distance. thanks

Hi Dr. Lee, do you see any problem with implanting the LAL for a patient who is also getting a Hydrus microstent for glaucoma? I will consult with my surgeon regarding this, but just wanted to get your professional opinion. Thanks!

Hi doctor Lee, thanks for your helpful and interesting info. My opthalmologist doctor said i have 20/25 vision she doesn't want to do the cataract surgery but why I have blurry vision. I cant understand. Other opthalmologist doctor wanted me to do the cataract surgery already because the doc saw my eyes covered with white spot. Im confused. It could be a great option for anyone. You will have to discuss with your eye surgeon what the potential risks are for your eye, as well as expect to be blurry before you finish your adjustments. Hope that helps!

Thanks for your question! Pricing really does vary depending on where you live and your provider. You can ask your eye doctor if they are offering this lens and how much out-of-pocket cost it would be. Sorry I can't answer that question directly.

Hi Dr Lee I have Catarac surgery 7 months ago they put 3 focal lens but I have astigmatism and I have frame glare’s around everything I see reflection all letters and objects I can see but not Quality pictures even colours not warmest as before I feel like living in different world I can’t concentrate please this Lal lens is suitable for me or not.

I am so sorry to hear about your issues with the trifocal lens. You need to discuss this with your eye doctor and see what your options are. Sometimes there are issues that can be fixed, such as scar tissue (PCO) behind the lens or refractive error, and sometimes the lens itself may not be well-suited for your eye. I hope you find some relief!

Hello, I have to make my lens decision in 3 weeks (late Oct 2023), so I’m watching videos and learning. Two questions- first, can a person take those listed medications after the lens is locked and the UV treatments are concluded? Or is that a lifelong issue re: taking those meds? I’m relatively young- 54 - with genetic premature cataracts, so I assume there’s a possibility that one day I may need one of those meds you listed as a concern for contraindications with this lens. Second, is the only option for multi focal vision with this lens the one where one eye is set for distance and the other eye is for near? Or are these lenses multi-focal? I’m confused on this point. I’m very nearsighted but I hate glasses of any sort and would prefer to stay away from reading glasses post-surgery. Does this desire eliminate the option of LAL?

It's OK if you need any of those medications AFTER your lenses have been "locked" in. The danger is only if you need those meds during the adjustments to the LALs. And...yes, the only way to get multifocal vision with the LAL is to have one eye set for distance and one eye set for near. If you don't want to wear glasses the LAL might be a good option for you. I already had Lasik earlier, will i suit for this LAL ?..my requirements & needs are to be completely glasses free..is the LaL a better choice than Vivity eDof?

If you are post-LASIK, the light-adjustable lens is a fantastic option to get you to your refractive goals. I love offering this lens to post-LASIK patients. Vivity is an EDOF lens, meaning it has some range in vision (i.e. distance and computer, or computer and reading) that an LAL may or may not achieve as well. If having a range in your vision is more important to you than having a "perfect" refractive outcome, you may want to go with a Vivity.

@@Gstar999 You are correct. Sometimes we can achieve some range in vision depending on your light treatment but it is not something we can promise.

@@MicheleLeeMD my requirements & needs are to be glasses free as much as possible but I'm not going with multifocal since my doctor said me I'm not the candidate for panoptix as I had Lasik he suggested me vivity edof and that too he said 50-50 chances of success in vision without glares or halos

Can the LAL be used for not cataract people who wants to get rid of there reading glasses , and are seeing good in the distance . Would it be possible to only do one eye ? with a LAL .

Don't do it! Cataract surgery is not risk free and complications are not uncommon. I would never, ever have this surgery just to avoid reading glasses. There is no artificial body part that is as good or better than what nature has given you. Be glad you don't have cataracts and just wear the reading glasses.

My LAL has been a disaster. Although I think LAL is a great option, not sure where my situation went wrong. Be great to have someone to discuss my situation with.

@@pytube777 I have learned a lot since this post. Overall, I would have been better off not having the cataract surgery. The vitreous behind my lens somehow was impacted and still causes issues with my vision. I have an option for surgery to replace the vitreous, but have decided to wait a few months to determine if the problem subsides. We are not just talking about a few floaters; the problem impacts my vision during the day, which sometimes makes everything blurry. I do not know enough if this can happen due to the surgery. I am told it happens due to aging, but I do not accept this kind of change on its own overnight. Either I did something wrong post surgery and I followed all instructions, or this is just something that happens to some individuals after surgery, part of the list of risks.

Just for people understand insurance will cover cataract surgery only if it is at a point that it's affecting vision.

@@MicheleLeeMD Welcome! I had LAL surgery in October with previous RK and lasik. I'm very satisfied with my results I see 20/20 60 year old vision and can use computer and phone without glasses but do need reading for small print and darker rooms or darker background on the phone etc.

@@timmccarthy5782 Yes I tried mono vision but wasn't for me. Supposedly if I didn't have previous RK and lasik I could've maybe done a different lenses but I am happy

@thomas I had RK in 1996 and Cataract in Nov 2023 with LAL lenses. I am not happy with vision and have been told since I had RK, I can’t expect better vision outcome. Wish I was told this before spending the extra money for the premium lenses.
