I love how this Doctor not only does such a fantastic job with his feet, but you can tell he really cares about the patients overall health!

This shows the importance of staying in contact with elderly in your family because for him to be diabetic and not checking his pressure and have these on his feet like this are crazy and hasn’t been being maintained, also he also obviously need bigger shoes and some type of moisturizer

This poor guy is just giving up. He's probably got untreated depression. My heart goes out to him, he just doesn't care anymore. He's forgotten how important and beautiful a human being he is. Bless you sir. :-)

More like he needed more education on diabetic foot care. Most don't get told, hey gangrene and loss of foot and or leg comes from you ignoring your feet.

Depression is powerful and can be very, very devastating. There was a point in my life when I literally felt disappointed that I didn't die in my sleep. I was only 22. If you suspect someone you love is depressed but not seeing treatment, please do whatever you are able to do to push that person toward medical intervention.

Gosh those were thick calluses. You did a good job of trimming them down and debriding the centers of the calluses. I hope we get to see a follow-up on this. Thanks to you and your patient for sharing this video.

I knew as soon as you began trying to scrape the skin away with that small blade it was going to be challenging. That was one of the biggest...toughest ulcers I've ever seen. Thank goodness you brought out the "big guns". It was like watching a granite excavation. Great job!

I just want to thank you for taking the time to film this. First of all, it gave me the chance to pray for people with diabetes and I can see for myself the troubles that it can cause. Secondly, I always enjoy watching a doctor doing his job methodically and cleanly and at the same time I can feel that you care for your patients. As a nurse, I appreciate your teaching methods. You were plain spoken, honest, and none judgmental. Good job and I hope your patient is able to keep from losing parts of his feet due to his diabetes. It is sad that the testing machine is free and the strips cost so much that most people cannot afford them, even with insurance. Keep up the great work.

I love your interaction with your patient through his treatment. I am learning so many things as you are teaching him. I have a son who is diabetic and I am always watching his feet even though he's 22 and can look after himself , I still need to see for myself. You are an excellent teacher.

Yeah but it's cringey 👹 cause he's actually kinda destructive and it looks so painful. Dr. Sandra Lee, the toe bro, Tampa podiatry... They don't hound their patients about 🙅 taking care of the issue before now, they ask those probing questions to actually figure out a solution for the person. He's OK but very cringy with methods I'm out. ☮️ Peace

Every case is different dont be quick to judge. He could have mobility flexibly issues. Suffering silently with mental health. Been lazy is not the answer.

Not to mention the exorbitant cost of medication and testing equiptment in the US. Special shoes, innersoles, test strips, doctors and specialist visits not to mention the need to test, take medication, monitor food intake and practice foot care every day. It all adds to a burden that would be hard to get on top of let alone if a person has mental health issues, is struggling financially and/or doesn't have a good support system in place.

I just love watching your videos, how respectful and caring you are of you’re patients entire health. I was raised to believe a doctor couldn’t actually behave in a respectful and considerate manner, and it deterred me from becoming a physician myself. This gentleman needs to get very serious about his diabetes. I can’t imagine the pain he’s been under with that foot ulcer. It sounds like he doesn’t know how to test his blood sugar levels or it’s too much trouble. I soak then take a foot brush to my feet at the end of the day before I take my shower. I’ve never had foot issues with my type 2 diabetes. I see my primary doctor every 3 months, my Podiatrist every 6 months. I’ve so far lost 143 lbs over the course of 3 years. Knowing all that can happen to you when you have diabetes was enough for me too get serious about my blood sugar levels. It’s too easy too do. 🦶

I think that he has neuropathy cause anyone else wouldn't even be able to stand walking around like that. The pain would be unbareable. It happens pretty quickly when your blood sugar gets high and stays high. Your feet...and your life is way too important to not do something as easy as checking your sugar, and taking care of yourself...even going to the Dr. It's way too dangerous to ignore. I pray that this gentleman takes a more active role in his health. Check your feet people...and check your sugar diabetics, I beg you. If not for you...then for you loved ones. God bless Doc...keep up the good work. I love your videos...and also I love the way u practice medicine.

Good advice, from one who knows. Kudos to you and your doctors for learning how to take care of your diabetes and then having the oomph to get it done-- you will never regret taking care of yourself. I wouldn't exactly call diabetic daily self-care "easy", but it's certainly not impossible either. It can be expensive (test strips, medications, doc visits and lab visits) for those with no insurance, but the day to day management is just something that becomes routine, like anything else we do on an everyday basis. Not hard, just takes some discipline and some insurance benefits or cash.

My mom has type 2 diabetes also, and has for almost 25 years, and I’m glad to say that her feet are okay except for neuropathy. She has managed her condition, and I hope this man has learned something from this experience, and will take better care of himself.

Omg the last two videos were amazing. The toe with gangrene I thought there is no saving that toe. You did a great job. You are very determined to keep going as long as there’s hope. Many drs would just amputate. This gentleman could find himself in a grave situation if he didn’t come to you. I know it must be very hard to work with people who don’t take care of themselves. In educating the patient, you are also educating us viewers. I’m an ICU nurse and little pieces of information like this are always helpful to learn, especially since we seem to only focus on the more acute issues during patients’ hospitalizations. But maybe I’ll look a little harder at patients’ feet now and possibly prevent a future hospitalization for them :)

That was a great job, he needs more help at home with a nurse going through his diabetes mess and blood testing as he doesn't seem too sure of what he is supposed to do. Will you post again on his next visit?

Could be he's never been taught that taking care of his diabetes, testing, meds, doc visits, etc, is something he's responsible for and something that's not impossible to do. OTOH, some people are just kind of "simple" and they can't put two and two together and get four, until it's too late. Others simple do not have the $$$ to purchase testing supplies and see the doctors as often as necessary. I hope that whatever his reasons for not taking care of himself are, he can get some teaching and financial help and get with the program, because it's never too late.

So much has been learned about diabetes treatment. Some folks are holding on to old info. The nutritionists are still keeping carb counts too high for a diabetic person

ancientsgate Unfortunately there are a ton of non compliant, out of control diabetics. Idk about the guy in the video but there are a tremendous amount of people who basically exhibit this same behavior despite having access to the medical materials and being properly taught how to care for themselves. So it does not always come down to never being properly informed. This dude would seriously benefit from having a home health aid to help him out. Like he is an A++ example of the type of patient who needs someone else to help him

My mum always made me and my siblings take good care of our feet, wash and moisturise every day. This video has given me way more incentive to keep it up 😭

Diabetes caused this. Not lack of moisturizing. Although that may have helped some. Mostly people just need to manage their diabetes.

Many people whom are diabetic have neuropathy. This may cause tingling, pins & needles, pain or just numbness in the feet!

It's wonderful to see this skilled, compassionate doctor provide such needed care and gentle education. I had no idea until I read the comments just how many diabetics are non-compliant. My dad was diagnosed with Type 2 a year and a half ago and has been diligent about checking his sugar and caring for his feet. He keeps a little notebook and pen on the dining room table with his meter and test strips to record the numbers and track highs and lows. It took me several years after my mom passed to convince him to see a doctor, but I'm really thankful he did and is taking care of himself. This video reminds me to tell him how proud I am of him and how much I love him. He's just now recovering from prostate cancer surgery and is, thank God, now cancer-free. We're now working on dietary changes and weight loss.

I never knew that skin could get that thick and hard to needing nippers to cut into the callus to get rid of it & even doing it that way was tough going. He’s lucky he came to see you when he did as that foot ulcer would of turned really nasty. You’ve done an amazing job on his feet considering how they looked at the beginning.

A lot of time they can't feel anything happening because numbness sets in. I have seen so many people loose feet from diabetes related issues.

Great video, I am so impressed with how much compassion you have for your patients. I hope this man stays on top of this because I think it can go bad very quickly. Should this have been something his primary noticed especially him being diabetic?

Yes, but studies have shown that many primary physicians don't check the feet, even those of diabetics.

I'm of the older generation and can remember when as a child The foot washing ceremonies at the little country church we attended. The pastor said to wash anothers feet was the most humbling thing one could do. But you sir take it far beyond that. What you do is remarkable and I do so admire you Dr.

I have this callus remover that is used at home and in salons and it would eat that stuff away. My dad has feet similar to that and he is also diabetic and I use it on his feet at least every two weeks. I also use Gold Bond Diabetic Foot Cream on him as well.

As a type 1 diabetic in the UK, I am so so thankful that all my medication, foot care, and appointments are free and have not been interrupted by the pandemic. I can’t imagine not being provided with everything I need. I feel very grateful.

I notice a few people commenting about the patient being difficult or apathetic. Basically saying there is something the patient can do better. Long story short, diabetes affects the whole body. Energy levels, brain chemistry, muscle agility, and much more. Its not the patient doesn't want to or can't. Diabetic bodies operate differently.

I have empathy for this man. He didn’t ask for diabetes anymore than I did. I was so angry about it. Mood stabilizers called Atypical Antipsychotics are big weight gainers. Seroquel made me crave sugar constantly. The med itself causes weight gain. I tried 3 types before they finally prescribed clonazepam for my anxiety. It’s a benzodiazepine and addictive but no other side effects. I have been taking it for 20 yrs now and have never abused it. I gained 75 lbs and have never been able to lose it. Depression does not help. I take Gliclazide and Insulin. I live in Canada so the price is much better here. Plus our Federal govt provides significant tax credits for Diabetics in order to compensate for the costs of the meds and supplies. I never judge diabetics. I know only too well how hard it can be. Fear can take hold and put me in denial if I let it. I loved how he became calmer as Doc talked to him. He sounded irritable at first. Being able to buy decent food and afford medications etc should not be an issue in North America. But it is. Just an FYI.....Charles Best & Frederick Banting invented Insulin in Toronto Canada during WW1. The University of Toronto was built on Insulin. Best & Banting didn’t make a dime off of Insulin; in fact they sold the patent to U of T for $3.99. Less than four dollars. They did this upon the promise from the University that all Canadians who needed Insulin would have access to it. The University sold the patent to Eli Lilly years later as they could not keep up with production demands. The University of Toronto reneged on their promise....that is why the miracle of Insulin is never publicized or given its due in Canada.

You can tell by the guy’s answers that he either knows absolutely nothing about his health or knows and just doesn’t care. Either will have the same unfortunate result. I manage a medical practice and see it every day. A physician, nurse or significant other can only do so much. There are simply so many patients that are not compliant. It can be very frustrating.

Y'all are always calling it non compliance when you do not realize that a lot of these people truly do not understand what is being said or what is really going on. They also lack full cause and effect issues. It is lack of educational background and fighting cultural misinformation and norms. The best place for a provider to sit in is understanding and knowing you may have to ask the exact same question 3-5 different ways BEFORE you get a factual and accurate answer from a black American patient. After that, understanding their living conditions and support system is essential to providing them better, more comprehensive care.

I will say this again and again, I am absolutely grateful that my podiatrist has taught and educated me on how to clip my nails, how to remove excess cuticle from my nail plate and what seems to be the best thing for me is to be able to use a scalpel blade to remove excess amounts of callused tissue from the balls of my feet and my heels so that I am at a much less of a risk for developing ulcers. Then again, I am alao grateful that I have hyper sensitive nerves because at just the tiniest amount of pain from my feet off goes my shoes and braces as well as my socks and I check my feet over with the equivalent of a fine tooth comb and if I don't like how something looks or feels then off I go to my podiatrist.
