A patient in his late 50s comes in with a foot problem of 4 weeks duration. He has a history of uncontrolled diabetes and toe amputations. Due to the COVID-19 crisis, he hesitated on seeking treatment. He thought that he could handle this issue as well as his slightly out of control diabetes. He presented with a hard and thick callus on his foot with drainage towards the toes. Debridement of the callus showed an underlying ulcer. He was further treated with antibiotic ointment and bandaging. A prescription was written for oral antibiotics as well. He will follow up in one week. It is important that during the COVID-19 crisis that people with urgent and emergency problems still seek immediate professional care.

My mom lost all the toes on her right foot because she was in denial about her illness. Now its my responsibility to monitor her feet for sores, calluses and the like. I'm very aware of the importance of this and I really like receiving education about foot care. Thank you forvyour willingness to educate the masses.

Dr. Jefferson, My diabetic friend had been battling an ulcer on her right big toe for months. Her podiatrist just whittled on it every few weeks and we did twice daily dressing changes. It developed gangrene and smelled so bad. When he wouldn’t return our calls we took her to ER where she was admitted and her toe was amputate down to the first big joint by a wonderful podiatrist. She was sent home with IV Antibiotics and I’m still doing daily dressings every day. Slowly healing. Thank you for the wonderful work you do.

I was in a similar situation 13 years ago. I was trying to keep a toe ulcer clean but it got infected and had to be removed. Ulcers are no joke.

I am tearing up watching this .Why do people don't understand they have to take care of themselves.You lost your toes and that should have made you WANT TO SEE DR DC... It’s only since watching this channel and Toe Bro that I’ve learned that callouses can be so dangerous. I had always thought they were just thick ugly skin that sometimes you would get removed during a pedicure or just to use a pumice stone in the shower if you remembered. This is been a real eye-opener! I am 68 years old, not diabetic, but after watching these channels, I’ve become a fanatic about caring for my feet.

These channels are so educational! I love them to! Because I'd this channel, toebro and dr.nailnipper learned how to take care of my 90yo grandmother's feet as well as my own. Whenever i see her, i take care of her toes that she gets ingrowns with and check for calluses. I'm so thankful for these channels.

Dr. Jefferson... I am sincerely glad this patient has Y O U has his podiatrist because you really speak to patients bluntly, but it’s always honest and sincere. I feel like this patient unfortunately, does not care as much as he should about himself 😢 specially after he mentioned, that he hasn’t been to his primary doctor in 6 months. 😢😢 I am a medical assistant! And if this guy was on of my patients I’d be calling him saying you cannot have a refill on your meds till you come in for a check in! I pray he loves himself more. He deserves it more than anything. Bless you doctor for helping him !

I get shudders every time I see neglected wounds on diabetics. I lost an aunt and more recently a much loved cousin who started with toes, then forefoot, foot, bus then aka till he died with overwhelming sepsis. What a waste in his 50s. I used to be so angry with them and also frustrated as I warned them till I was blue in the face what would happen. I even used to show them the goriest photos in my textbooks of diabetic complications to no avail. This man was very lucky!

at 73 i look at mine every night. i never go barefoot.a1c is now where i want it yet. occasional back pain keeps me from walk ing as much as i want. pain seems to make sugar higher too.

He is extremely fortunate that it wasn’t as bad as it was. If he follows the doctor’s recommendations, he can get it under control. He also needs to see his Primary Care Physician ASAP to get his blood sugar numbers under control. Six months without a doctor’s visit for a diabetic is way too long.

I agree. 3 months would be better, with a repeat A1c test done at that time. And even more often than that if someone's diabetes is way out of whack and the doctor is still trying to find meds that will help.

My primary just said I didn’t have to come back for six months, because my A1C has been pretty stable. 5.5 and 5.6 the last two visits. 🥰

Those are good A1c levels. Things can change quickly, though, so make sure you do home testing of your glucose levels at least a couple of times a day (at various times), keeping a good written record of the results, and don't wait if you see the numbers start to change-- make an appt much sooner than 6 months if it seems like things are going south. Normally an A1c every 3 months is a good idea for anyone actually diagnosed with diabetes.

@@ancientsgate you absolutely do not need to check your blood sugar a couple times a day every day with a A1C that good. Check it once a week before breakfast. Once that number goes up, do a 4 times a day check for a day and call your doctor with the results. Be mindful about it, but not obsessed.

Thanks to you, I’ve learned that callouses and corns can be dangerous, although I do not have high blood pressure, diabetes, or obesity. Still, it’s just important to take care of the feet! Thank you!

I would guess the patient doesn't feel his feet. He has neuropathy most likely that is why he is missing his great toe/hallux and that lack of feeling is what allows him to keep walking on that callous.

Many people with diabetes have neuropathy - nerve damage that makes their feet numb. I can’t feel my feet at all.

Since starting to watch your channel I make doubly sure about the care of my feet and visit my podiatrist twice a year. Thank you doctor for highlighting the problems which can occur with diabetes. Your patients are so lucky to have a kind and caring practitioner like yourself. You never judge which is the reason why your patients come to you no matter how bad their feet are.

You are such a blessing from God to your patients. You provide 'total' care. You gather medical history and you encourage them to be responsible for their health. You are God sent! I have concluded that we as black people suffer with medical conditions more than other races because of the way we are treated by health care professionals. when money is not the issue we are reluctant seekers of care because our care is not valued. If he is suffering of polyneuropathia, then the nerves don't transfer pain impulses anymore.

He obviously has severe peripheral neuropathy. His pain-receptor nerves just don't work anymore, which is why it's so dangerous not to do self-examination of the feet at least daily.

I see you take care of your patients and truly care about their health. I just wish the ones with diabetes knows how not taking care of their feet can be an upcoming amputation 😔

I am pre-diabetic and watch my feet like a hawk being that my father had both of his feet amputated 2 years before his death in 1991 on Christmas morning. He was also a smoker.

Dr. Jefferson, you are just such a great practitioner and an amazing educator. I look forward to each of your videos and rush to watch them the minute they pop up! You have such interesting patients that truly need your help. And the way you teach your staff and therefore your viewers brings me back to this channel time and time again. Mostly I love the care, kindness and compassion you give to the suffering patients that come to see you, frightened to the core , even if they try to hide that fear. You direct them on the care they need, not to wait to come in, but you don't beat them up up over the times they haven't held to the schedule, you just remind them how important it is to see you and their primary. Anyway , just me again, letting you know that I adore, as many clearly do, your work, your bedside manner, your spirit, your staff and your patients, who are so kind to be filmed and allow us to take part in their experience's. Many people don't feel very good about themselves to begin with and not taking care of their physical problems is a symptom. All a caring Dr can do is emphasize they care and encourage the patient to become a partner with them in that care. This Dr is the best at this of any Dr I've ever seen. Period.

I wonder how many of the homeless in dc are dealing with this same issue. Breaks my heart for the folks that can’t afford treatment. This was the best/worst one I’ve seen. I love your patients. ❤️
