As a former home health aide in NYC, I can say that we were thoroughly instructed as to our limitations with patient care. In this case, we would have only been able to INFORM the visiting nurse about the condition of the patient's feet, because they were the only ones authorized to clip nails, etc.. We had to follow the care plan given for each patient. So legally, the aide can only do so much. It also depends on the laws of each state. Now the home health aide CAN accompany patients to medical appointments, such as the podiatrist. Home health aides can also assist with bathing and applying topical treatments such as lotions and ointments. But the bottom line is that the PATIENT and/or their primary caregiver has to stay on top of the situation. They have to take responsibility for their health and be as proactive as possible.

You don't need formal training just common sense especially for a diabetic patient its worse for them they can loose limbs and potentially their life wake up and care for others how you would yourself! Period that's the bottom line. One quick assessment and cream or cleaning the wounds. Im sure it was in his chart that he's both diabetic / with previous ulcers on the feet which or pressure points . That's the most important to pay attention to.

Thank you for this interesting case. Also a big thank you for making people aware about what a nasty disease diabetes is. Lots of diabetici are not aware of the severe complications it can cause in the hands and especially in the feet. Ulcers because of pressure or wounds on the feet can easily lead to amputation of toes, a part of the foot or even the whole foot. This can easily be prevented if patients inspect their feet before going to bed. It cost nothing and it can prevent a whole lot of misery and pain. I hope the patients ulcer will heal. He may benefit from orthopaedic soles in his shoes to take away the pressure points.

Unfortunately, most type 2s anyway are never taught anything about their disease. Nothing. Unless they want to get educated and they're readers, they don't try to learn, either. My experience is that primary care doctors are good at diagnosing diabetes, but they know nothing (ie, zero) about the daily management of either type, T1 or T2. AND add to all of that, if someone has no health insurance, they are really up a creek and will never be able to afford the medical care, testing supplies, or meds they need. It's a problem with many ramifications. And you're right, diabetes can be nasty. OTOH, it's possible for a diabetic in good control to actually be healthier than their non-diabetic friends and family-- they eat better, exercise, watch their weight, and generally speaking just take care of themselves better. Hard to make generalities.

I agree but I'm not sure how old the Gentleman is but if a Aide is being paid to care for this man it is her responsibility to see he is keeping up on his appointment's. Sometimes when we are sick we want to be left alone but your family would make sure you are cared for. Knowing his past history of this issue causes great concern cause it could lead to bigger issues later. Just curious though of a Aide is hired doesn't she or he have to report the concerns to her bosses? Or is this a under the Table kind of Aid. I just know my heart 💔 for anyone who is not getting the proper Care from a caretaker. Doctor Jefferson is a great Podiatrist and shows great concern over his patients. Sure some would say well it is job and a paycheck but you can see in his actions band tone of voice of how much he cares about his Job and the Patients he takes care of. Wish I could find that here in my State.

It’s too bad he doesn’t have a family member who can check to make sure she is doing her job. I took care of my sick father at home until it got to the point that I didn’t have the physical strength needed. I did not want him in a nursing home for the reason you gave. I tried slipping the nursing aides cash in an attempt to get them to pay attention to him, but that didn’t help. I visited him almost every day. I felt sorry for the patients who had no one visit. Those aides didn’t pay much attention to them. And Hospice was no better. I found out they ordered a hospital bed for Dad 2 weeks prior. It still had not come. I got on the phone with them and raised hell. It was 10:30 on a Friday night. They tried telling me nothing could be done until Monday. I raised more hell. Finally around 1:30 they brought one in. Dad died the next week.

He's stubborn and admits to drinking too much. Nurses and even doctors can't make you take care of yourself, much less a home health aide. They're not nurses, and the patients aren't children, adults need to take care of themselves.

My mom is a diabetic. She lives with my sister who checks her feet daily. She's got a dremel tool and files her toenails and moisturizes her feet. The thing that gets me is that her PCP has never once checked her feet nor recommended her to a podiatrist. It's only by watching these videos that I've learned how important foot care in diabetics is. So I thank you for putting these informative videos out.

We were refused Medicaid in NYC, my former home where I lived for 60+ years. And I saw the flaws, exclusions and flat out stupidity in the system. Profits over people, very time. That said....We were so pissed we moved mom, after many independent, vibrant years... in with my sister. To home care her at the end of her life. She died at 95 at peace, with love surrounding. My sister and I took turns to keep her active and engaged in life: bathe her, wash her hair, later on - diaper her, make sure her feet and hands were well attended to, skin cream, fresh clothes...etc. Even though she had memory loss from dementia, and often thought I was 'the nice nurse from the agency', it was well worth it, taking care of the wonderful lady who for so many years looked after all of us!!

Doc you really are a fine physician! I would be so tempted to lose my temper (at least it would show on my face) when people come in and have not been doing what you’ve told them to do (I’m not talking about this patient in particular but rather all patients) and, as a result, continue to have problems. Everyone needs to take a step back on this care aide he has with him. You don't know the situation with what she is hired to do & how the patient responses. My mom has had people in before, and they are hired for very specific tasks, like checking a wound vac, or even just blood pressure/blood sugar. They are not there for full body care, in a number of case. A lot of the time, she never had the same person twice. I lived with her for 2 years and i couldn't get her to go to the doctor for things, because she was too stubborn. Don't comment unless you know the situation at his home.

Wow from healed to an ulcer. So many diabetics don't take good care of their feet until major problems arise. I have seen so many diabetics who have lost toes and even a leg because they either are lax on seeing their GP or just don't keep an eye on what is happening to their toes/feet. BTW. I like the voice over explanations you give. Did anything grow from the culture? Also good blood flow was a bonus.

Whomever is this home health aide is rude. She could of excused herself out the room and talk on the phone. I'm glad that he came in for his appointment. Great job doctor.
