A first-time patient comes for treatment of her severe fungal toenails. She has put off getting care for many years because she was too embarrassed to show anyone her feet, including doctors. Her sister is already a patient of mine and tried for years to convince her to come see me. She finally relented after getting tired of the condition of her feet.

I think we have all been embarrassed to go to the doctor for treatment. The information that you give your patients is valuable, we all have something going on that we’re not comfortable with but the more understanding and empathetic doctors are the more patients will come in and provide the information to get the correct treatment. Nice video.

Dementia care when properly administered by caregivers (medical or family), should always be about 'checking the feet. Not merely for diabetics, but for anyone who has limited mobility, or any condition that impacts full movement, where the person cannot trim his/her nails, or see to their calluses, etc. When we home cared my mother, then in her 90's with mixed dementia, I always partnered it up with getting her 'fresh, groovy socks.." I made sure they were brightly colored, and the whole socks changing led to looking after her feet. With her dementia, she was still able to have fun with it, and it almost became a kind of game. Meantime, she wound up, with foot care from me...

I always think when you ask them if anything hurts and they don’t really say yes or no, they are just so used to everything being uncomfortable that they can’t differentiate between what’s normal and not. If you or I suddenly woke up tomorrow with toes like that we’d say they hurt. These individuals have endured for so long that they don’t even remember a time they weren’t uncomfortable
