Very poor hygiene when you see that amount of thick, scaly feet. This is easily remedied w/ soaking in a dollar basin & very warm soapy water, thorough rinse & a vigorous towel rub. Comes right off w/ a few repetitions.Thick cream (not lotion), rubbed in. As a retired, 35 year PTA, I saw plenty of this in hospitals & nursing homes & helped them. The nurses did not have time. Folks seem to think the abundance of dry skin between toes and around the toenails is fungus. He has circulatory issues that causes excess dry skin. As the patient said, he felt shame around his feet and men aren’t socialized to take care of their feet like women are. He just needed some good ol exfoliation and lotion. There’s a difference between “washing” your skin and “exfoliating” your skin. I’m so happy with this transformation 💚 you leave the feet looking taken care of as opposed to other less thorough pediatrists only treating the nails and leaving the skin looking like a complete mess.

I just don't buy that. If a person, male, female, or in between, doesn't want to take care of BASIC self care, then they can take responsibility for it. It's not momma's or daddy's fault, or wife's fault, or society's fault. It's each person's fault. Period. I am the last person who is going to give a man a pass for not take a bath, using a washcloth, and a nailbrush. The shame comes from not taking care of himself.

@@idontextback you cannot judge someone because you just don't know their circumstances. He is a 74 year old male. He looks after his elderly father who would probably be high needs. Money is tight for him. Being that age, men were never taught to look after their feet like women were. Having a rams horn such as that can be the result of a nail injury and unfortunately once the nail bed is damaged, it can never be repaired to look normal again. The Doc mentioned that his dry skin is due to his poor circulation. Again being old school, moisturising his feet is a girl thing to do. He may also have flexibility issues and doesn't know that there is equipment out there with handles so people can reach. So please don't judge what you don't know.

Dear Dr. Jefferson, Thank you for posting this video. At different times in my life, I've worked as a journeyman in the Trades, and (quite a while later) as an engineer, with a bit of teaching thrown in with the latter. So I have great respect for people who do skilled work with their hands, and doubly so when I see the work being performed with the care and attention that you showed not only in taking down that unusually large "ram's horn" but in your approach to your patient both in cleaning up his feet with a great amount of attention to details, and in how you explained to the gentleman what you were doing, and why, and gradually learning some of the "back story" of his previous health issues that might bear upon his foot issues. It is a genuine pleasure to watch a real pro with "good hands" at work. That was very evident with some very tough and awkward-to-reach areas on built up nails. If this was an orchestra, you'd definitely be sitting in the "1st violin" or "concertmaster's" seat, given your skill, your patient and the slow, gentle way you worked. You never gave the patient any sense that there was a need to rush - you steadily, but with no hurry kept cleaning up and making a plan for treatment as you went. I could see that you were using compound-action snips, but even with that extra leverage, doing the work you showed here required strong hands and a measured, careful and delicate touch. The ability you have is uncommonly special. I have wished more than once that I did not live several thousand miles from your office! That said, I have learned a tremendous amount, not only about self-care and self-examination of my feet, but of when it is necessity to get help from a professional if I find something that's "not usual." Happily, I have had no emergencies, and I've learned a lot about how the fit of shoes can affect foot health. Up here, we also spend a portion of most days in winter boots, which bring other challenges. Since first seeing you use Shea Butter on your patients' feet, I made a point of finding a source willing to work through the mail, and now Shea Butter is part of my daily routine. I think my feet appreciate it - I certainly do. Thank you, and best regards from the northwest of Alberta, Canada. I always enjoy watching you work with your patients, using your skills, compassion and (if necessary) firm and gentle persuasion to help them, whatever the issue.

Any Doctor who ends up with a patient addressing him as "Man" should be teaching classes to doctors on bedside manner and how to treat patients with respect. Dr. Jefferson, you addressed your patient as "Brother" and "Man" in a number of your questions and statements to your patient and that did more to put him and me at ease than anything you did. I am seriously jealous. I have been a diabetic since 1976 and only two doctors in my life have put me at ease like you did with this patient. If you ever move your practice to the Milwaukee area, I will be your first patient. You are one of the best doctors I have ever seen. Again, I am seriously jealous of your patients.

Being allowed to ask questions and getting decent answers makes a great doctor! Even if watching him whack threw that toe nail makes my toes hurt, I would go to him, if I had any problems. He's a good doctor. He talks TO his patient, not at them. I like that in a doctor!

Congratulations. Thank you for the video and many more videos. Dr. Kevin J. is explaining to his new patient about the growth of toe nails. Some people watch their toe nails growing but be in denial. They allow fungus to grow and do not care. Some of these people are homeless, but too proud to say that they are. This patient is asking questions. Dr. Kevin is very caring and explaining what it is about his feet. The patient will leave with healthy feet. I hope the patient gets the help that he needs. It's very sad. Everyone is greatly appreciated. Love always, Patricia ❤🎉😊Great job!
