A new patient in his late 70s was referred to me by another patient. He is a diabetic with very long and thick fungal toenails that cause pain when wearing shoes. The toenails are “terrible”, according to the patient, and often get caught in his socks. His toenails have not been trimmed in over a year. But not to worry, this tale has a happy and dramatic ending. The toenails are trimmed and sent to the lab for fungal identification to guide treatment.

Been going to local Podiatrist. I have toenail fungus and I am diabetic. Not once did my PCP or podiatrist mention a circulation test. Thank you so much for all your information for us! Also, never once did the podiatrist try cutting off the bad nails. He just had me pick up some expensive Clarus antifungal solution at checkout. Greatly appreciate all that you do!

As a wound care nurse it’s been my experience that toenails that neglected on a diabetic patient usually means some of those toes won’t be there eventually. Hope they continue to receive the maintenance they definitely need. Bravo doctor!

What a makeover. This must help this patient both physically and mentally. His feet were restored to a “normal” appearance. Dr. Jefferson is the best podiatrist I’ve ever watched.

My grandma is 102 years old and this is how her feet looks. I have been cutting her toenails for over 30 years. She needs a Podiatrist like you. His feet look really nice

This Doctor is sooooo low key. He knows these people have neglected their care but he’s .....I dunno - just lovely. I know this is so they return and don’t feel ashamed. He is terrific.

I think it's amazing that his nails are so long and are so curved and he is diabetic and yet the skin on all his toes is so healthy looking. This man is very lucky because it appears that he has come to the doctor just in the nick of time.

My husband's NY VA Primary Care Doc was like DC Foot Doc..very attuned to the entire person, not just toenails - making sure all the bases got covered: diet, exercise, circulation, biking and lifestyle activities, current meds, proper shoes, the right type compression socks, no smoking, on and on...Hubby is also Hispanic, a higher risk group for type 2. Doc even gave ME the foot tutorial so he can stay on the right track, and I see to it hubby's well looked after! Thank you, Sir.

As a nurse, I realize the importance of educating the patient. This Dr. is a phenomenal educator and I love it. Following up at home in between visits is also paramount. I need to see a podiatrist myself. Gotta get an appointment. Way to go doc‼️

He educates well because he doesn't bombard him with information. He gives his advise in small bits so it sinks in with the patient. Excellent technique.

@ Kim Stanymyr I couldn't have said it better. I'm a new subscriber to his channel & he's such a wonderful Dr. & human being. Most people who have bad problems with their feet are so embarrassed to go to see a podiatrist so unfortunately their problem gets worse & worse. & they never get treated because they're too ashamed & embarrassed but they shouldn't be because that's what these Dr's. specialize in & I'm sure they've seen all sorts of gross stuff, I mean look at some of these videos. He's performed miracles on these patient's feet. If only there were more Dr's like him! As u said @ Kim Stanymyr he's gentle, compassionate & most importantly NON JUDGEMENTAL! That makes all the difference in the world! He does his work, actually helps the patient, keeps making sure to ask them if they're ok or in any pain while working on them & follows up with them until he heals or fixes the issue they came in there to begin with. He makes no judgments & put them at ease. God bless u Doc💕 thats true professionalism & one of the few Dr's left in my opinion sadly is actually there to help his patience & cares. He's not in it for the $ like a lot sadly to say are these days. Great job! Also, wanted to apologize when typing fast my spell check messed up some words so I hope it can still be understood. I am impressed with the dignity with which you treated your patient! You cleaned up his feet so very carefully and I wish you were here. My feet are not full of as much fungus, but now I see how bad it can get. I have taken a few notes and will resume rotating my shoes again. Thank you for sharing.

I am a diabetic that started developing thick nail fungus on both my hands and my feet. I started self medicating with chlorine dioxide and humic acid capsules. My fungus has all but disappeared and my nails are no longer brittle and have returned to their natural colour again. I have also taken on a plant based diet and daily intermittent fasting. My insulin intake has been reduced from 50 units daily to just 10 and I reversed kidney disease (stage3). Diabetes is a terrible disease, but it can be overcome with total commitment to a better lifestyle. I intend to get off insulin.

Doctor, You are such a kind and exceptional foot doctor. You ask your patients questions, and you actually listen to them! What a rare quality you have, especially because we don't see it in the many types of medical practices of today, who don't have that personal quality. I find that an encouraging asset to those who are thinking about going into any type of medicine. You explain exactly what you are doing, to the patient, and to those of us who are watching you. We can learn from you, as well. Thank you for your genuine gift of caring for all who trust in you! I find your videos so interesting and educational. Great job telling him not to do his nails at home. A tear in the skin can have consequences. Of course they don't usually mention it. The answer is usually "I was giving it time to heal." It isn't going to heal because your A1c has been 11% for two years! Good job Doc.

He should fire the primary care physician. PCP is supposed to check the whole person and refer him or her to the different specialists when needed.

You know your recommendation for alternating shoes and spraying disinfectant in them every day is just a really great tip for anyone who is on their feet all day every day. We should all do that to protect our feet and keep them healthy. Thanks for the tips. 🙏🏼❤️

Appreciate the takeaways! My husband is diabetic. Has had a foot wound gone to cellulitis 2x because of rule #1. (I tend to follow sometimes) It's a callous that blistered. Dealing with wound care Dr. .. I think that is how I stumbled here. 🙃😀 I enjoy your videos and your compassion for the patient.

I wish my podiatrist would take as much care as you do with your patients! They just cut and roughly file my toenails and that's it! I'm diabetic and have many other illnesses and my son is my full-time carer. He's really good at looking after my feet. I've certainly learnt many things by watching you on KZbin, you have a lovely voice and bedside manner!!!

Thank you to the Dr's doing great jobs on feet. Getting the ingrown toe nails out & killing fungal nails have got to be 2 of the toughest jobs out there. You have to care a lot about people to do such great work. Thank you. I'm a retired State Licensed Manicurist & have recommended a lot of people to go to the Podiatrists for appropriate treatment on some terribly neglected feet.

I really like how you take the time to take all that fungal material from under his and your other patients nails and thin them out so they can be comfortable in shoes. I imagine there’s also a medical reason for debridement. You’re the best. God Bless You

My goodness. You’re the best podiatrist on KZbin by far. A supreme professional that educates the patient and the viewer. Consistent in application of the proper technique and procedure for each patient. Good karma to you and the continuing care you give your patients. Also - you’ve lost weight! These huge toenail nippers are the most amazing thing I've ever seen... Maybe this is casual for podiatrists, but this is very unusual to see how the nails what grew a couple of years could be cutted off just in a couple of minutes...

Wow you’re the first doc I’ve seen that uses a curette to clean the debris out from under the toenails. Very good!☺️👍🏼👏🏼

DC Doc you are the man. If she has diabetes the circulation test is critical. I used to take care of my grandpa and he was diabetic. I deployed to Afghanistan and had been away for 8 months. Came back home on R&R and asked him how he did was. Asked him who’s been cutting your nails? He said”they ain’t been done since you left.” I told him I would come by and do it. Normally When working the smell was typical. This time there was a lot of bad smells. The small toe on his left foot was black and did not look right. As I worked my way from big toe to small it was normal. Within a few seconds of working on that toe it started to come off. I stopped and gently wiggled it. It came off in my hand. I didn’t say anything because he didn’t seem phased at all. Diabetes is a terrible thing.

Thank you so much for the information on athletes foot and how to care for that with your shoes, I have been having issues that way and now I definitely will do better. This is why I watch so much I learn something new about feet in general from your channel and how important it is to give them proper care at home and professionally

Well, my dad flat out refused to let me check his feet for over a year. I even mentioned it to his doctor and was told he refused the doctor's request too. Finally after one nail was causing him pain did he allow it. Can't exactly tie them down to get their socks off.

II’m in Australia and am an ICU RN (feet are usually the least of our problems! 🤣) BUT I have an 80 yo mother who is diabetic and she goes monthly to her podiatrist and then gets and mani/pedi. That constant care is hard to keep up with as age creeps upon us. I would sincerely like to comment on the way to speak to your clients with kindness and respect- even if they haven’t been for proper foot care for years you just say ‘I’m glad you are here’. I don’t know how the medical payment system works in the US, I watched a video where an elderly gent was talking to you about prices for foot care. Seemed astronomical! It's so hard to see the before pics and think about how long they suffered. I'm so thankful you get their feet back into good shape. I wish I lived closer and cook take my dad to see you.
