Every time I've taken it, I've lost that run. Usually through my own lack of skill. In your hands, yeah, it's effective, but someone without that level of experience? It's a run killer

Pretty solid item early on. As you go onto the later floors it loses value because of the situations it can put you in during room fights

Prette sure that eye item you got in the start of the run gives half of your tears a 1.35 damage multiplier

Cool little mini-synergy for Jacob’s Ladder - if you use the item FLUSH! to flood a room, the electricity gets amplified. It’s not limited by floor, just by terrain. Can be very useful in large boss rooms and actually does a good job against Delirium as the lightning instantly tracks his teleports.

I love decap Attack. Its a straight up reference to a classic Sega game called decap Attack In which the main character can chuck his head at enemies. A very fun item to use as well.

Normally don't comment, but for your cough have you tried an anti-tussive? I recently got sick, and the cough made my lungs worse, which made me cough worse (horrible cycle). Taking Delsym both day and night helped dramatically in reducing my coughing so my lungs could heal and not be inflamed. Just my 2Cents!
