@@arunmn8123 Yep. if you have a habit of chewing only on one side (or chewing more on one side than the other). The jaw will start to shift because of it. Start chewing on the opposite side.

before orthodontics, my midlines were off by 4 mm shifted to the left on the top arch. NOW my midlines are bang on with my dental arches, but in comparison to my nose, my midline is just as off as it always was. Orthodontists did a great job in my case, but that is not going to change the position of my crooked nose. Something to keep in mind as a patient, your Orthodontist can only do so much to your facial symmetry by aligning your midlines. Most of us don't have a perfectly symmetrical skull! lol.

Can elastics worsen the midline? I recall my ortho extracted my teeth and shifted(ruined) my bite awkwardly and negatively affecting my midline with elastic. Hi Dr, I’ve been on braces for almost 2 years trying to correct a minor underbite. I had a missing second premolar (upper) before starting the treatment. Initially, I was advised by another orthodontist to do some extractions to so call balance and help the process. However, my current orthodontist, despite knowing the issue, started my treatment without doing any extraction telling me it’s unnecessary. After over a year of treatment, he noticed my midline is still not aligned even after a few tries using rubber bands. I overheard him commenting that I should have done some extractions earlier on and that got me so annoyed and somehow mad. Even now, he’s still trying to ‘fix’ it using rubber bands again. I feel like he’s more like experimenting on me. 😔

Hi there, your feelings understandable. Unilaterally missing teeth can be difficult to manage orthodontically in terms of midline correction. Perhaps you can request a treatment progress consultation with your orthodontist to gain some insight and feedback and see what the next steps are to your treatment. It can often help to clarify intentions and clear up any misunderstandings.

So my bottom teeth had ended up shifting two teeth over. It was actually hard to tell unless you looked at my gum and saw the midline’s being different. I have been using rubber bands for a little over half a year and I am almost finished. However, I have three teeth in the center so facing my upper two middle teeth and the rubber bands have sort of given me a very slight underbite. I was wondering if this is not supposed to happen.

Your question is very specific and because I don't know you, it is difficult to answer. Midline elastics in conjunction with class II/III elastics can help with midline discrepancies. Your treating orthodontist is the best person to ask. Teeth can move up to 1 mm per month in children and teens and half that amount (1/2 mm) in adults in ideal conditions.

So this isn’t about midline. But I have been wearing my retainers I believed as often as possible but my bottom teeth shifted ever so slightly. You can’t tell when looking at my teeth but from an up angle you can see there no alligned. My retainer fits fine without problem so I don’t understand. I can’t afford braces again and it’s a tiny shift. What will they do about it. Will I need to do invaslign?

Great question. A small amount of shifting post orthodontic treatment is normal and expected. Teeth, bone and gums change with time, even with retainer wear because teeth are not embedded in cement. If the shift bothers you, get a consultation to see what your options are.

Hello, I have a pretty rare lower jaw setup, I have a supernumerary lower incisor, so I have 5 of them, instead of only 4. I am currently on the first invisalign tray. I asked my orthodontist about my situation and also about the midline discrepancy. Funnily enough, I have crowding on my upper jaw, my lower jaw, although it has one more tooth, is looking very well, all of those 5 incisors are straight, no overlapping or anything like that. My question is, my ortho told me that obviously after we are done correcting my bite and crowding, the midline can't be coincident obviously. I understand that it is a rather specific question, but reaching an ideal bite, straight teeth etc. is the midline disrepancy given the extra bottom tooth a problem in your opinion? As long as my teeth are straight and my bite is perfect, I don't really care about my lower jaw midline being off. Thank you very much for your time!

Hard to say without seeing pictures but certainly you can ask to see the clin check (Invisalign) of your finished alignment and bite. More commonly, some patients have three lower incisors, either because of lower crowding or missing tooth congenitally. The upper dental midline will bisect the mid point of the middle incisor. In those cases, the alignment and bite can be good, even with one fewer lower incisor. The only drawback is a slight excess overjet in those cases. In your case, one extra lower incisor may have the opposite problem of insufficient/minimum overjet.

I have one missing and retained tooth (23) what causes a midline shift and a big assymetry on my face, one side is flatter, another more curved etc. I had braces over 10 years ago. But without extracting one tooth on the other side my midline stayed shifted after treatment. Currently I finally want to fix it again with braces, but my orthodonthist wants to extract that one tooth on the side when I have more teeth to move the midline to the center. Do you think it's absolutely necessery to do this extraction or maybe there are another methods?

I can see your dilemma and can understand your question. Without a clinical exam, I’m afraid I can’t answer your question. In order to shift a midline, you need space. IPR and elastics may be options for mild midline discrepancies. You can always seek another orthodontic opinion before making your decision on how to proceed. Good luck.

My orthodontist said that mid-line discrepancy of 2mm between the top front 2 teeth and the bottom front 2 teeth does not require changing, he said most orthodontists agree to this. However my top mid-line is far to the left from my Cupid's Bow and nose, which are both actually aligned with my bottom mid-line. My orthodontist said he needs to shave off twice the amount of tooth of the length of the discrepancy to fix the top mid-line but does not recommend it. Considering what I've said is this true? Is there no other way to fix it? My mid-line discrepancy is due to my lower jaw being small and thus the bite after braces forcing the right side of my teeth to touch the tips of each other (class II malocclusion I believe) the left side touching the gaps as with a normal bite. I am 19. He also does not recommend jaw surgery, despite me wanting it, but I am not sure if that is just because he does not want to put braces back on me, or because it is, as he says, risky. In this video you said wearing rubber bands can fix a discrepancy due to malocclusion, how is this possible? My orthodontist did not make me wear any rubber bands during braces, except those that go horizontally on top of the wires. After braces my right said does touches tips while my left touches gaps.

Can it be caused by wisdom tooth? I have 20 and they started going out when i was 19, and i never remarked i had midline teeth until this years. Throughout my life some dentist even asked me if i already had braces when i did not cause my teeth were really straight. I also have constant pain in my teeth since last year, i attributed that to my wisdom tooth but may it be by the lack of space and so by the movement occurring in my jaw causing the midline to move? It’s quite unlikely that wisdom teeth can cause any significant midline shift due to their distance away from the midline. People with straight teeth can see crowding with time. It‘s a part of the aging process (even though you’re quite young). Ask your dentist about your dental pain because it doesn’t seem normal to have constant pain.

Hard to say...repetitive oral habits can affect dental change, although they tend to be tipping forces and not mesial/distal forces.

Thanks for your video My daughter is 12 years old she had an overbite of 12mm she her orthodontic said that she needs two fase of treatment she had twin blocks for 9 months to corect the jaw and widen it now she has been wearing braces for 5 months she tightened them twice. It sounds like you’re concerned. I would recommend that you request to talk to your daughter’s orthodontist for more information, as only he/she knows the specifics of your daughter’s treatment. Midline correction is not usually within the first steps, so it may be addressed later in treatment.

I can't find my answer anywhere so I'm really hoping you can help me a little... My top midline is correct and is aligned with my nose etc.. But my bottom midline is off and you can see it on the outside too because my bottom jaw looks asymmetrical and uncentered. So my question is this : Will moving your bottom teeth to fix the bite correct the asymmetry at all? The jaw has to move in some way right? The teeth can't move without the jaw following right? The teeth can be moved within the alveolar bone and dental midlines can be improved with ortho tx. The jaw asymmetry does not usually improve unless it’s due to habitual jaw deviation due to unilateral posterior cross bite and the cross bite is corrected. I would recommend that you schedule an orthodontic consultation for detailed feedback regarding your specific case.

Orthodontics and elastics cannot move the jaws but orthognathic surgery can. Some people have functional shifts of their lower jaw due to unilateral posterior cross bite and correction of the cross bite can “fix” the deviation (it’s not a true asymmetry). I had a bottom molar extracted years ago. I've been going through invisalign treatments and only now realize how bad my midline was. Problem is, the invisalign have straightened my teeth, but a combination of the bottom teeth being shifted to one side and my upper teeth being canted the opposite direction, the midline of my upper and lower row lines up but is not only not centered with my face but my entire bite is also canted! And when I try to bring this up to my ortho, he chalks it up to "muscular differences" (as if that has anything to do with my upper row) or ignored me when I point out that my jaw has to slide to the right in order for me to bite down fully. I don't know what to say to get through to him or if he is hell-bent on ignoring me to avoid the problem.

@@hutchinsonscott100 No, and now I don't believe invisalign--maybe not even braces--can fix my issue. My upper jaw likely has more growth downward on the side where I haven't been chewing all these years. I've been meaning to get a consultation with an oral surgeon to find out. Anyway, if you want to go through with it, just make sure you are crystal clear about your concern with the ortho. If they can't fix it and you don't think it's worth straightening your teeth without fixing it, maybe save your money.

I'm 23 and when i was 12 my left upper canine was removed by a dentist. Then at age of 15 i noticed my lower teeth set is a mess. Thus i got braces. My teeth become aligned. i had my braces on and dentist at starting told us about total money...but after the removal of braces he said i have to pay for retainers......So I went to another dentist to gain some knowledge about it. My teeth improved but the problem is they are still not aligned. And the new dentist told me about how my teeth are forcefully moved which damaged my roots of the teeth.......Ryt now i am 17 but before old age i will loose all my teeths..and something even can happen related to physicl or mental problems...He spoiled my life 😭😭😭😭 i am not able to do anything how can i Claim him??? Thanks for watching. Wisdom teeth don’t usually cause changes in your jaws but impacted teeth can create other dental issues like resorption, difficulty in cleaning, etc. Check with an oral surgeon to get your options.

There are many ways to manage missing teeth, depending on where they are. Sometimes spaces are closed and other times, dental implants or bridges are planned after orthodontist treatment. Your orthodontist will work with your general dentist to formulate a plan that best suits your needs.

Hi Dr. Bailey! I'm currently on invisalign and have a cross bite + overbite and overjet. My bottom teeth's midline is perfect with my nose but my top teeth's midline is shifted heavily. My bite on the left side is good with 1 elastic for the overbite/jet but on the right side I have three elastics to try and align my bite and fix the overjet. Midlines are usually corrected with a combination of elastics, IPR and/or extraction, depending on the severity. Good elastic wear is very helpful.

Retainers are passive and they help retain teeth. Unless the midline discrepancy is tiny and favorable settling of the bite occurs, retainers do not correct midlines.

Midline discripancy can cause delay in speech,speech problems?? My face is always looks sad,depressed....midline shift correction can be done over 35 yrs also??if not possible with braces with the help of surgery we can correct midline shift???

Your question is quite specific. Proper position of front teeth and jaw position can help to support the lips and soft tissue. I would recommend an orthodontic consultation to get an assessment. The orthodontist can also make referrals to other medical and dental specialists if needed.

As long as the periodontal health is good, there isn’t an age limit. A few of my more mature patients are in their early 70’s.

Hi, refinements are used to try to obtain a coincident midline. Sometimes elastics are needed. Talk to your provider and let them know your thoughts. Good luck.

Hello Dr. Bailey! I have a question, in the video you've mentioned that if the back tooth is extracted then the teeth will move to the side where the tooth was extracted but what if it was extracted on both sides? Where would it go? I got myself curious in this 😅

Great question. The teeth move into the spaces that are available via orthodontic forces based on the doctor's plan.

Healthy teeth can be moved into better alignment with orthodontic treatment.

I had double jaw surgery in June, I still have an open bite on both sides, my orthodontist put me in class 2 elastics and called them midline elastics, I’m 10 weeks post OP and concerned about it, because I’ve been wearing elastics for the midline since 2 weeks post OP and still haven’t noticed much change. Appears to be only be 1-2 cm off but I am concerned I would have to have another surgery and praying that’s not the case because I had a crossbite and underbite at first.

Talk to your orthodontist and see what he/she says. Depending on the results of the surgery, post surgical orthodontics can take some time to finalize the teeth position to the new jaw position. Good luck.

@@TrayMinder the midline elastics seem to be helping, I’m chewing better and noticing less and less shifting when I eat now. When I take the elastics off my midline is getting closer and closer to being centered, thanks for the peace of mind.

If the upper and lower midlines are not coincident, then it's called a midline shift. If the teeth are slanted, a midline shift can be detected.

Great question. Spaces can me made for the small lateral orthodontically and the general dentist can enlarge the small lateral via bonding or veneers to preserve the dental midline.

Glad you found the videos to be helpful. It depends on the number of millimeters of midline discrepancy, whether you have crowding that warrants extraction as well as midline correction, and your bite. Even significant midlines can be corrected, al la Tom Cruise. Seek the advice of an orthodontist for your assessment.

Good question. Straight teeth (alignment) is not in the same category as midline discrepancy or good bite as the latter two pertain to the relationship between the top and bottom teeth. As such, you can have straight teeth and bad bite. Ideally, in orthodontics, we strive for good alignment and bite, but that is not always the goal or even achievable in some cases without adjunctive treatment.
