I’ve had great results from using Hydroquinone 4% for 90 days. Then I rotate with Cyspera daily for 16 weeks. Then back to Hydroquinone. I never use Hydroquinone during the summer months. I use La Roche Possay 70% Sunscreen daily, and NEVER go outside without a sun hat .

I love hydroquinone, but I did use it incorrectly for several years. I didn't pay attention to the directions, and I used them for more than three months at a time. I also used various skin lighteners in my teenage years. It wasn't until I came across your video a few years ago on the original differin dark spot corrector that i realized I needed to take a break from it. Now I make sure to do so. I consider myself lucky that I never experienced side effects because I definitely put myself at risk all those years. I do use Apostrophe to get prescription hydroquinone, and I've had fantastic results! I'm extremely pleased. I don't think they let you get another three month supply until after a three month break, so at least they have some safeguards, but I can see where we should be concerned.

I've been on hydroquinone for melasma since 2017. I go for regular check ups with my dermatologist and I do 5 months on, 2 months off of hydroquinone. My weaning off process lasts 6 weeks, during which I reduce hydroquinone to 3 x weekly for 2 weeks, then, 2 x weekly for 2 weeks, then once a week for 2 weeks. So for the last 2 weeks of my 2 months "off" I am completely hydroquinone free. I am prescribed other skin brightening products during my 2 months off. Over time, my melasma almost never returns during my 2 months off, and by the time it's time for me to go back on HQ, only the smallest hint of pigmentation has started which is quickly remedied by my "on" phase!

I have purchased two brands of hydroquinone for PIH and too scared to start it due to potential side effects. And besides my PIH is slowly fading with 20% Azelaic Acid and strict sunscreen routine every two hours so Dr Dray is correct sunscreen plays a massive part 😊

Yes sunscreen (especially one with iron oxide) can massively help with hyperpigmentation 🙌

Thank you for this. Can you do a video discussing Eucerin's thiamidol ingredient, and also one about how to permanently get rid of the "root" of a melanocyte umbrella (sorry I don't know the proper terminology) that has already formed? I heard that lasers only slow down pigment production and the only way to get rid of a freckle for example is through deep chemical peels. Is this true?

The video never mentions how long to take a break for. Anyone have an answer for how long someone usually should break after 3 months of 3% dosage, with only the last month doing it morning and night (the first 2 months were only at night)? Thank you in advance.

A lot of comments suggest 3 months of on treatment with hydroquinone (remember to TAPER THE DRUG and not abruptly stop usage) then replace it with 3 months of nonhydroquinone (kojic acid/ lactic acid with retinol) then 3 months on w hydroquinone so on and so forth

Hi Dr. Dray. Do the studies show if exogenous ochronosis occurs overnight or does it develop gradually while on hydroquinone and is it visible by sight? I use an online dermatology service and I've been prescribed Hydroquinone for about 8 months now; although I have no signs of ochronosis I am getting slightly worried about the potentially irreversible side effects.

Great video. I've been using Agency Dark Spot Formula with hydroquinone. That service alternates hydroquinone every two months to a non-hydroquinone formula. i.e., two months on, two months off. However, I used a formula from a popular skincare brand for several years and I did get negative outcomes that lead to my stopping that product (2%). These outcomes included pronounced skin lightening around the targeted dark spot. That did stop when I quite using the product.

My dermatologist prescribed me hydroquinone but never told me it needed to be used only temporarily. I literally used it for FOUR YEARS straight🤦‍♀️. It wasn't until 1 month ago I googled melasma that I came across an article about using it for only a few months then tapering off. Needless to say I am no longer seeing that dermatologist anymore and am in the process of tapering off. So far no rebound hyperpigmentation, but I understand you're at risk for rebound side effects up to 10 yrs after stopping. I hope it doesn't happen. I'm so mad at my dermatologist.

you can use it as long as you need to, there is no need to take breaks. That is a myth. Also rebound pigmentation has never been reported in the literature.

Same dude , my derma prescribed me and i have used it for an year , i have stopped using it when I googled Abt it, so for i have not experienced rebound pigmentation, bt I'm very scared of it . I have stopped using it now . I hope i don't get rebound pigmentation. The most irritating thing is it was my dermat mistake nt mine .

Hydroquinone is a hard one for me. I saw a huge difference for months when I first started using it for Melasma and discoloration in general, but suddenly my pigment rebounded and was just as noticeable as before. So frustrating:(

Make sure to use sunscreen with iron oxide and avoid any heat treatments (sauna, steaming etc). Hot temperature makes melasma much worse.

I used hydroquinone several years ago, didn't really see much difference in the discoloration of my skin. After using for several months, I stopped. I've had better results from my otc retinol cream and sunscreen. 😊 Happy Tuesday!! 🌸

My derm said to only use for a month then give it a rest for a month because apparently it can cause the scar to turn purple.

Ahh that means your dark circles were caused by the thinning of the skin, retinol encourages blood circulation to the area and thickens the skin which helps to fades the appearance of the darkness under they eyes, where as the hydroquinone is for patients that have stronger pigmentation of the skin under the eyes I had a combination of both at different times and they were both worked wonderfully, now I just use sunscreen

Hydroquinone got rid of really dark spots, but my freckles stayed in place, like whaat? I thought freckles would go away quickly than these dark spots. I don't understand. Edit: Also I noticed that it won't fade a hyperpigmentation caused by acne(

My dermatologist prescribed me a customized lightening cream from Skin Medicinals and I was skeptical first but soon blown away by the amazing result. No side-effects. The cream is composed of: Hydroquinone 8% Tretinoin 0.025% Kojic Acid 1% Niacinamide 4% Fluocinolone 0.025%

That fluocinolone will burn anything off the face. My dad's derm had him use it to burn off possible precancerous spots. My son's derm had him use it for warts - it took 3 days and it burned his flesh off. 😱

@@convenienceandpracticality9032 😱 Sorry to hear that. That's terrifying. I had no side-effects and my skin is super sensitive. I suffer from hyperpigmentation. After about 4 days of using the lightening cream, it worked so well to fade my hyperpigmentation I could almost see through my skin surface. Right now I'm taking a break from it and using Tretinoin, Alpha Arbutin and Azelaic Acid to keep hyperpigmentation under control.

@@mikosato3838 That's awesome it worked for you so fast! I have melasma on my forehead and I have treated it with hydroquinone and tretinoin for many years. It's faded but still there. I've had to wean off hydroquinone and now I'm using tretinoin and azelaic acid. I have been planning on adding arbutin too. Always a constant battle.

Dr. Dray, could you make a video about wound healing, say after some trauma like surgery? Why is occlusion the best way for an incision or wound to heal? Would love to learn about skin care tips for wound healing! Also any insight you have for properly cleaning your skin when you can't get an incision wet (C-section, back surgery, etc)

I have been using Hydroquinone for two years to clear up the hyperpigmentation following cystic acne. I do use for 3 months and then rotate off for 3 months as well as retinol and lactic acid and I have had great results. The cream is hydroquinone 12%, kojic acid 6%.

Alpha arbutin is hydroquinone derivative, that isn’t as effective, but you can use it daily and doesn’t cause any side effects. It’s also sold over the counter with 2% potency.

The only thing that worked for my melasma was total sun avoidance which took several years and life changes including using a UVA protective parasol when outside. If you see Gwen Stefani , Anne Hathaway or Famke Jannsen out in public they always use a parasol.

Me too. UV umbrella, UPF hat and wrap also. Totally nerdy looking, but I have a dark spot on my cheek, so I follow Dr Drays advice and put a round bandage over it.

Hi maybe someone here can help me. I’m a dark skinned guy that used black and white bleach cream around my eyes as I have had really dark eye circles since I was a teen I misused I used it for 12 years daily without issue and it fixed my eye circles. The product changed the hydroquinone use and stopped working. I was prescribed hydroquinone 4% directly and within the last two months developed wrinkles and purple and black rings around my eyes. It’s to the point I’m a dude thinking of wearing make up. Help! 😭

How do you you taper off after 3 months of use? My derm didn’t tell me how to. I just do it myself and taper off in one month then use alpha arbutin (obagi) with tretinoin for two months and then go back to using hydroquinon again. Hope this is oké 😊

That cream has been around for over 100 years now it is suddenly bad to use. i think that we are being "hoodwinked" again as we were about eating eggs and butter. I have used it on my skin for many years and my skin was absolutely beautiful and blemish free.

I’ve battled hyperpigmentation on my face for years. I’ve achieved a nice balance of fun in the sun without compromising my skin but I notice that when I ski in really cold weather ( completely covered despite having mineral sunscreen powder over cream) my discoloration comes right back. Do cold temps impact this condition? Thanks Doc!

Snow bounces the suns rays from the ground so overall you can get a super high dose of exposure even wearing some SPF. That may be a factor for you

Would you be willing to do a video explaining nanoparticle mineral sunscreens? I'd love to know more about it, if it's safe, what nanoparticle mineral sunscreens you would recommend, and why there are so many "non-nano" options on the market. I'm also seeing a lot of labels with "clear zinc", so would love to know what this is and if it is different than nanoparticle sunscreen. I prefer to use mineral sunscreens and am just trying to understand the difference between some of these terms and if it generally helps reduce white cast. Would love to learn more! :) I love your channel and content!

Doc, we know how Hydroquinone shouldn't be used long term but what about Arbutin (at a 2% concentration)? Can Arbutin be used long term like more other skincare ingredients?

I use hydroquinone 5 years with little break and my skin is glowing (I am a skin bleacher) I do always wear sunscreen and go out with uv umbrella. You need to know how to use it laddies

Can you cover isobutylamido thiazolyl resorcinol (thiamidol) in a future video? I live in Europe and bought an anti-pigment product that contains this (a Eucerin product) but I have not found much coverage on this, so I wonder if it is only specific to this company.

I got prescription online for Hydroquinone 12%, niacinamide 2%, hydrocortisone 1% for 4 months then take a break for 2 months. My dermatologist explained the possible side effect of HQ for using it more than 4 months. I have dark spots on my face that I want to get rid off. I will use my prescription together with 0.025 % tretinoin. Thank you Dr. Dray! I just subscribed.

@@GK-ys4ik Hi there! Yes, my dark spots still there but it's fading. I've been on it for 60 days. I purchased mine at musely with Doctor's prescription. The tretinoin I'm using is not from them. I combined it with the spot cream I got from musely. So far so good, my face is smoother and lighter.

I've been using 4 percent hydroquinone for the last 3 weeks on super stubborn and dark hyperpigmentation on my cheeks. It's helped so much. I use it once a day at night before going to bed. I am just starting Tretinoin .025 again. I have my calendar for 2.5 months and slowing down the usage over 1.5 months. When I come off for my 3 hydroquinone hydroquinone break I will not only be on tretinoin but I will also start on Cyspera (Cystamine). When I notice extra redness in the area I am using hydroquinone... I oil cleanse with anti-inflammatory oils tamunu oil mixed with apricot kernal oil... or I ice my skin. I've noticed that doing these things take the redness out of my skin and that makes me feel like I am at a lower risk of developing extra pigmentation if I manage my inflamation. I have been using SPF 50 religiously (anytime I go outside). I am getting really good results so far... can't wait to see what I will end up with.

I just received a compound cream from my dermatologist. It’s Tretinonin, hydroquinone, kojic acid, and ascorbic powder. Is it safe to put all over my face or should I only be putting it on my spots? I should have asked during my appointment!

Dr. Dray, it would be really helpful to know how to use HQ. My dermatologist was pretty vague on how to use it. I see so much contradicting info about spot treating vs putting it all over the face as well as how to use it with vitamin c, Lactic acid, tretinoin, AHA's, azelaic acid, etc. I haven't been able to get straight answers anywhere. It was prescribed for my sun spots and hyperpigmentation.

I recommend watching a video by Doctorly, they've explained very well how different ingredients work to prevent or treat hyperpigmentation, so it will make more sense to you regarding the actives you've mentioned :)

I used Murad Rapid Spot Remover with Hydroquinone 2% for a year or two on and off and it absolutely made a stubborn brown spot on my temple vanish. Sadly, that product has been discontinued and even though years have passed the spot never returned. I just started trying hydroquinone 4% cream, which burns. The prescribing doc insists hydroquinone does not work but I know it does, at least for me. I'm somewhat apprehensive about this new product because it does burn and doesn't have whatever goodies Murad was adding to their product.

I used it once under medical supervision for a face wax, NEVER AGAIN on the face wax stuff...lol, that discolored my upper lip. Worked great but would never randomly use.

I‘m using hydrochinone since 6 weeks now because of melasma and hyperpigmentation, but it made my skin worse, my melasma is darker than ever. And I have also used tretonin on the rest of my face, my skin tone changed. I am darker than ever. Horrible

I have been using 4 percent hydroquinone from obagi after a chemical peel for the past week. Can I stop or do I taper like I’ve read? I plan to become pregnant soon within the next month or two to try to conceive

Thank you for this video! I have been using a compound Hydroquinone I got from my derm back in October. I think I was very consistent the first 3 months. I didn’t see much improvement in my melasma so I continued to use it on an off almost to none by now. I think I will stop for good now 😂

I have two stubborn large sunspots. Living in Arizona, I use 4% HQ for 12 weeks in the winter, then take a break for the summer using other pigment inhibitors during that time: FADED by Topical, Vit C, etc. Works for me, but I respect the stuff and use cautiously.❤

I am the patient that have been using hydroquinon for couple of years and was diagnosed with exogenous ochronisis. Used it under doctor supervision, 2 Monty off and 1 off, not in high concentration. So, it's not super rare

i have type 5 skin and hydroquinone 4% gave me a black patch on forehead only with 4 uses. retinol, microdermabrasion, laser toning have been helpful for my hyperpigmentation tho

​@@dynamitedynamite2737 it's safer to avoid laser for people with types 4 and higher type of skin tones, because laser can cause burns and once the burns heal the patients will be left with hypopigmentation patches which takes months to go away. Although Microneedling With PRP will be helpful for hyperpigmentation, I think the sessions could be monthly once or every two weeks. But if u have hyperpigmentation the chances are it eill go away in a few weeks to months, with proper skincare

I'm a new follower and I've been so desperate to find out how to get rid of my breakouts. I'm leaning towards fungal acne but mine isn't typically itchy. I do have ABPA which is allergic bronchopulmonary aspergillosis. I wonder if this condition can make me more suseptable to fungal acne. I've also been on zythromax long term. I just switched to trying the nizoral to see if it helps. Please give me some tips on what follow up skin care I can do that won't aggravate fungal acne!
