I recently switched to Xylitol, AN alcohol sugar, as a substitute for white or unbleached sugar for making holiday pies and cobblers. Within a few weeks I found my feet and toes were experiencing acute pain. I was sure it couldn't be gout because I don't eat a lot of meat (except turkey over the holidays), drink no alcohol, drink water freely and rarely eat processed foods. I grow a lot of our food in my garden. My toes turned purple and felt like they had needles being pounded through them. A friend brought me Allopurinol and instructions to take a large dose for 2-3 days. I felt better and began researching gout. I heard Dr. Berg mention that Xylitol can contribute to gout. Research shows that Xylitol alcohol sugar increases the risk of gout 800%. Not only that, it crosses the blood brain barrier and was likely the reason I kept stumbling around like a drunk. I've always been intolerant of even small amounts of alcohol. NO MORE XYLITOL FOR ME!!!

@@toni4729 For years I bought into the 7th Day Adventist - Blue Zone propaganda against meat and was vegetarian (with eggs and cheese) for 12 years from 1968-1980. for 'spiritual' reasons. I've NEVER believed that meat contributed to my health issues. For some time now I eat beef every day, have dropped weight, though I was never fat, and FEEL BETTER as well as not always being hungry.

Spot on information. I just got my first UA test at age 65. It was 6.8 after three years of low carb (basically carnivore). Finally an explanation of why I've had HBP, kidney dysfunction, and undiagnosed gout. I can only imagine how high my UA had been.

Please keep us posted. My uric acid is high but since it's the first time I get tested I can't tell if it caused by keto which I started three months ago or it was higher and got better with keto. I considered stopping keto but I really didn't want to and if I stay on it my 7.5 may get worse and progress to gout. Any thoughts,?

@@zayd7282 i have similar problem. I range from 5 to 7.5. I dont want to stop keto carnivore. I have never had a flare but sometimes I can feel a tiny bit of pain in my big toe. I have high blood press and genetic marker for HBP and gout. I am going to keep going this way. Dont know another way

Great talk and insights, Dr. Boz. I had global gout pain, got on a gout diet, no meat or seafood, and more grains and fruit and everything got much worse. Tried Keto, which provided a lot of relief then after 6 months went mostly carnivore with a touch of keto which made things even better.

If you get a gout flare during a keto diet, try 1/3 to 1/2 a glass of concentrated lemon juice with sparkling mineral water added. It has worked well for me, even though I don’t know how it works. A cup of coffee can also bring some relief.

I had gout, and believe me you don’t want to experience that pain! Magnesium cured it for me. When I felt my last attack coming on I overdosed on magnesium and right away the pain began retreating and within days it was gone. Never did develop into the full-blown pain.

@@lawbee2448 Yes, it must have been at least four years ago. Perhaps five. And I also take vitamin d and a host of others but not regularly as I should. But I really did think it was the mag that did it. Of course I can’t guarantee it for others but it couldn’t hurt to try. I would take 800 mg in the evening. You have to be careful because it can give you loose stools so you may need to adjust the amount according to your needs. And I didn’t really adjust my diet either. I don’t drink any alcohol but I do eat meat although not a lot.

@@lawbee2448 That’s hard to say. I guess I’d say I just eat simply. In case you’re wondering, I don’t eat any of the new specialty type diets. Mostly I cook my own meals and don’t eat out much. I like to make soups and not too spicy. (My daughter says I don’t like “flavor“) but I just don’t care for a lot of pepper. Nor garlic. I don’t skimp on salt either. Perhaps I should just say an old-fashioned American diet. But, I don’t eat a lot of meat. I like fried chicken and roasts, etc. but it’s just a small part of a whole meal. Perhaps one or two pieces of chicken or a hamburger but not a plate size steak like some people do! I don’t have dairy very much anymore either. I hope that addresses any questions you have.

@Taiwo Omotosho I can’t tell you right now what my vitamin D level is. I haven’t had it checked for a while but I was too low about a dozen years ago and started taking about 20,000 units a day. Then I was told that that was too much and my level was too high. Right now I’m taking 10,000 units a day. As for my diet, I do eat sugar although I try to limit it but I’ve never succeeded in eliminating it altogether. And I don’t take any particular magnesium but just what is cheap. Right now I have Puritan’s Pride.

I went through this. I have been fighting this for 3 years. Go get a message and then drink a lot of water for sever cases. Then if you fast for longer times it will clean it up. Last watch you water intake. You might not be drinking enough water. I went from only being able to sleep on my back to sleeping normally on my side again.

Liver, not many people eat liver every day, fructose, people eat a lot of that. Shellfish, I don't think we eat a huge amount of that. Yes, there's too much sugar and alcohol in the average diet.

Dr. Boz, I've lost about 25 lbs the past year. I haven't had a gout attack in many years. My uric acid typically fluctuates from 3-5. I've had two tests though where I've had crazy high uric acid. On the first, I just finished a 72-hour fast several days before; one the second, I was in ketosis for 5 straight days and lost 1-2 lbs. In my last one, I suddenly had a uric acid of 11! On my first one, I forget the number, but I believe it was above 7. After two weeks on my retest, it dropped to 3. I suspect on my upcoming retest, my 11 will drop dramatically. BTW, my Dr Boz ratio during this said period of ketosis ranged from 20-40. Having said all that, does prolonged fasting or ketosis acutely impact uric acid that way? And has anyone else experienced this? Does fat metabolism inherently release uric acid into the blood? Thanks! I'd love to hear insights from everyone! I"m pretty confused.

The uric acid is there to save you from oxalates . Oxalates are the problem not uric acid . When you go law oxalates zero carbs your uric acid will go high in the begining as your body started Releasing those oxalates . Sorry for my english

Gout is a symptom of metabolic syndrome. Clear that up and you'll be fine. I also take a lot of vitamin C and the gout hasn't come back, even when I had a few rough months when I was drinking too much.

@@mrright1068 Im so sorry it happened to you. It's always possible some people are just more prone to that. Or that maybe I was just living a lifestyle that was so much more detrimental to me. At least to me living a low carb lifestyle has eliminated gout for me. BUT, in the off events where I possibly get a hint of the initial pain of gout, I will always take 500 mg of Naproxen. I have found that if you cut this off at the pass, it will help stop it. As soon as you feel it coming on...and you'll know it...take like a minimum of 500 mg (there's different versions but youll be able to get it if you've had gout before). It can literally stave off an attack.

I agree with him as there are people with super high uric acid who never get gout. The key factor to address is inflammation and keto does a good job with this. Maybe this is why the patient mentioned here had high Uric acid, but improved outcomes.

@@CreatingintheWaiting Ouch! Sorry to hear that. I had it in the ankle, but the knee would be worse. Maybe extra meat isn’t for you. Maybe kidney function isn’t up to it. I would get into raw above ground vegetables and bitter food like Arugula and bitter melon supplemented with lemon juice drink with added apple cider vinegar and a small teaspoon of bicarbonate or baking soda. I hope you elevate your leg on soft pillows above the heart. This works for me after 2 days, especially if I get more sleep. Stress makes it worse. Take care.

If Dementia is likened to type 3 diabetes, I would liken gout to type 4 diabetes. They are both caused by excess carbs and sugar leading to high insulin responses then inflammation. Consequently, they both improve with keto or carnivore diets. So if you have gout, pretend you’re a diabetic and it should improve significantly.

I find that half of that works for me. I just factor it into my daily carbs. Frankly, when I am not n the middle of an attack I don't care how many carbs. I just want the pain to stop.

If you are concerned about the sugar content in the sour cherry juice, I would recommend taking sour / tart cherry tablets/ pills. Sour cherry tablets taken with chanca piedra and celery seed extract helped to relieve my mother’s gout symptoms enormously. She no longer has pain as she used to complain of.

I’m not sure what is the medicine, treatment or simple drinking more water that my flare ups disappear all of the sudden. I had flare ups solid for 5 months 24/7 . I’m taking allupurinol 300mg since last November 2022, gout still was bother me a lot, in December I got stem cells, months went by and pain still on my joints even higher, I was taking pain killers every single day etc. I went 4 times to urgent care within 4 months, pain was horrible but I still getting the same pain killers, now I’m getting better and haven’t taking any pain killer for close to a week, I’m not sure what is helping me. When I started to drink lots of water, I notice that the pain started to reduce. The clue is drink at least one gallon of spring water Avery day.

I am 43 yo male with %12 percent body fat .as ı was regularly checked due to my body building history.ı know that till my first flare up my uric acid levels was averagely 8 or 9 during the last 20 years.after my first flare up.doc put me on meds of 300 mg allo.3 months later even on meds without any diet it was still 8.8.then ı stopped the med and went for keto but strict one here what happened. 1.month 11.7 2.month 9.8 3 month 7.5 4 month 6.5 5 month 6 during that improvement my ketones are 3 and insulin was 3.2 ıt was going down systematically.then ı quit keto and did low carb then to normal carb protein junk food sometime to time. after 23 months gout hit me so bad and on going flare up almost 3 months nothing helped even streoid shot. now ı have been doing keto for 2 months its not going down. this making meupsat and discouring indeed.

According to the USDA database, wild salmon contains lower total purine (188) relative to most commercial fish species. That's a pity that there is never an answer to the cases where the advice doesn't work. I think that for some people there is simply a genetic clearance problem of uric acid, independent of the insulin theory. Not every gout sufferer is overweight and insulin resistant. And I guess unfortunately in these cases diet alone might not work.

Polycystic kidney disease (PKD)does cause renal insufficiency because of the cysts growing on the kidneys. There is a medication called Jynarque that a nephrologist would need to prescribe. Your primary care doctor cannot prescribe this medication. Jynarque will slow the progression of this kidney disease. I work at a Nephrologist office and I am the liaison for the doctors and the drug company that is why I know this information. The kidney doctors will suggest how many grams of meat to consume. The provider I work with does the Keto diet himself. Not everyone with PKD goes on Jynarque. Best of luck.

Nice video content! Apologies for butting in, I would love your initial thoughts. Have you heard about - Lammywalness Quick Defence Guide (do a search on google)? It is a great one of a kind guide for getting rid of gout without the hard work. Ive heard some incredible things about it and my close friend Aubrey got excellent success with it.

I disagree on one point...Vit C will increase uric acid and it takes 3 days of one glass of fresh orange juice to give me gout. This is confirmed on Pubmed whom I consider to be the primary source of research on most things healthwise. And when I fast my uric acid level goes up.

Vitamin c does lower uric acid not in the form of high sugar orange juice That will surely give you a flare

For someone who is on allopurinol for gout, but also seems to have it well-managed on a ketogenic diet, is there a situation when you would consider/recommend cessation of allopurinol treatment?

The allopurinol dissolves the uric acid crystals, making your blood levels higher. Unfortunately, my kidneys didn't excrete the uric acid faster, so it just caused me more flare ups. Uloric helped me, and when I skip it I am now getting tophi outside my joints, so no more joint pain. Colchicine for 3 days usually takes care of a flare up. Went several years without treatment after allopurinol failure and was on crutches for about 4 years. Wheat Belly (Keto) really helped. Had flare ups after a large dose of summer sausage after a purchase of one of each kind, and a separate large dose of thin crust pizzas when I was feeling poor.

If I stick to any meat and vegetables I’m fine, but anything with sugar and too much bread with a few too many sips of wine can set it off again. Then I take an anti inflammatory until the pain goes then back to keto with no meds.

Eating one small bowl of sour cherries every morning for three weeks is a good remedy for gout. Also drink at least two quarts of fluids daily. Gout is actually a form of arthritis. To lower Utica acid,take activated charcoal by mouth,one half teaspoon to one teaspoon four times daily.

My uric acid is high but since it's the first time I get tested I can't tell if its caused by keto which I started three months ago or it was higher and got better with keto. I considered stopping keto but I really didn't want to and if I stay on it my 7.5 may get worse and progress to gout. Any thoughts,? It's delimma

I do OMAD with an 18/6 restricted eating. I eat an avocado/chocolate/cream cheese concoction (stevia sweetened) for dessert nearly every day. Last bite at 1pm. I test at 10:30 pm. Ketones 3-6.7 are normal. Glucose 98-108. Does the Dr Boz ratio count even if sugar is that high? Am I getting the benefits? Or do I have to give up my stevia and get the glucose down to get them?

@@hairway2heavensalon541 I never test in the morning bc I heard “dawn effect” means we should wait at least one hour after waking before testing. I REALLY want my coffee with cream first thing... waiting over an hour to get it doesn’t work for me. But I haven’t eaten for 9-10 hours by the time I do the blood test at night.

Yeah, i dont know why they call this a restrictive diet when you can eat desert every day. If you eat every day keto sweets i think you should do some long fastings to reset your body.

Friend showed me a quick solution that works, Right when you slightly feel it coming on. drink a gallon of water as fast as you can. works every time. Cheers;

@@directajith Uric acid levels increase for a variety of reasons - even for healthy activities like exercise or eating wholesome red meat. Ideas on how to get the body to best process uric acid is important. Restricting processed sugars and unhealthy seed oils in the diet are only the beginning.

I have a ua at 10-13 mg/dl and doing carnivore straight since 4 weeks. Fatty meat butter and eggs + salt and water. Interesting thing my gfr after cystacine c went from 60 to 99 ml/min which sounds great. Should I use adenuric/allopurinol to counter that while on keto adaption? Can't find sufficient information on this topic. Thank you!

Keto is not worth it imho. It is risky and hard to adhere to. I recommend the *Agoge diet* because it's personalized and diverse and most importantly worked great for me.
