As someone with hereditary joint problems I really appreciate this video because my left ankle has been effected by them almost my entire life and sometimes I worry when it’s acting up and I need to use my brace that I’ve unknowingly injured it so this is very helpful!

I just had an incident yesterday. No need for sports or adventure. I had been sitting on my foot while working, got up without realizing my foot was asleep, and then put my full weight down on it. Since I couldn't feel it, I didn't realize it wasn't actually underneath me, but rather getting caught on a cardboard box on the floor. It twisted and I heard a giant crack as it rolled from the outer edge over to the top. By that time I was catching myself on whatever was around, but the damage had been done. So, perfect timing with the video, doc. Thanks.

Would/could you do the weight bear, test first & if the patient can't weight bear , stop the test & dial 999, if they can't do the 4 step rule , (then stop & dial 999) OR would you do a complete check first (including A,B,C & D, before you do the weight bear & 4 steps. As a side note some people might not know that you CAN dial 911 in the UK for Emergency services. They did this because so many youngsters in the UK watch USA films, documentaries & programmes, they knew the number 911, but didn't know the UK one was 999. By the way I do know you can also dial 112!!

I have broken both ankles and a wrist (one more to go!). The first one (ankle), I walked home 7 blocks from our senior year event afterward, and decided to get a ride to to the medical center the following morning. And that is the number one reason not to drink excessively before going to a carnival featuring an inflatable jousting arena :P So I am glad to see there are specific pain tests even if you can walk!

@@DrJamesGill Oh that was a while ago now, everything healed up fine, although my wrist I slacked off on the physical therapy and it took a couple years to get back to normal strength. Just to be clear, these were separate incidents! Whew, all three at once would've been bad indeed :)

Incredibly helpful to someone who loves hiking! Question though, what about swelling? Immediately after injuring my ankle, point C swelled to a large painful bump. At the time I treated it as a sprain but now I’m wondering if it was actually fractured.

Swelling can be both soft tissue and bony issues, Often tenderness is related to ligament injuries, hence why the positive predictive value is only 1 in 3 will have a fracture

I'm very blessed on that side. I've rolled my ankles a lot, at very nasty angles, and I mean reeeaaally nasty. got in actual pain very few times, and actually never got injured in my entire life. The worst I could think of, I was in pain for like a whole afternoon. A couple days ago, i slept in a bump hole on wet grass. Slided at a 50-60 degrees angle inwards, crossed my legs, almost fell. And I'm overweight, almost obese. But all I felt at this moment was ridiculous (and lucky). I got my balance back, went ahead and walked a good kilometer forth and back (avoided the damn hole back) before climbing the stairs towards my 3rd floor. I have freaking titanium ankles. Or rather, rubber ankles.

Dr.Gill, can you do a video about hiatal hernias, their symptoms, what possibly causes them, and possible treatments? I myself have had one for 3 years now. I'm thankful that my Gastro doctor listens to me about my symptoms and he wants to push for me to get surgery, but its a slow process. However, I am in a facebook group for people who have hernias. We give advice, support, and vent about the frustrations of having this horrible thing. I bring this up because I've heard alot of doctors dismiss their patients when it comes to the symptoms of the hernia. telling them that their hernia isint the cause of their symptoms. I had sooo many symptoms when I first got my hernia and it was just 2 cm. its now 3 cm and I developed Gastritis. Before it became 3 cm I did not have gastritis as my Gastro doctor did an endoscopy on me. This go around I did a second endoscopy and it seems its gotten worse. In the facebook group alot of people are dismissed by their doctors and get gaslighted them. I know doctors are the professionals dont get me wrong I respect it, but they arent the patient, so why dismiss what the patient is telling them? I truly feel like more studies need to be done about hiatal hernias and why they are becoming waaay more common as the years pass. Do you have any experience with treating patients with a Hiatal Hernia? I think this would be a good video to discuss all of this.

I actually rolled my ankle about two years ago at work. Had to limp my way to my car and into bed, but I kept it elevated and on ice over night, then took the next day or two off, and I was fine. Now my only foot-related problems are my flat arches. On the subject of sprained body parts, could you do a video on how to identify/diagnose sprained wrists? I’ve sprained both of mine over the past year, and I think my job might have had something to do with it (I load/unload packages from semi trucks.)

I wish I had this years ago. I rolled my ankle a couple years ago and i think it may have been sprained and didnt heal fully cause it still gives me trouble sometimes at certain angles and sudden pressure or too much weight on one spot. Also when im running or standing for too long

I jumped down a staircase Right foot first and landed on something that twisted my ankle with all my weight behind it. It hurt really bad and my foot went blue and really swollen and sore. X-ray showed no fracture to the bones. Doctors said that the soft tissue will heal themselves without any surgery. Its over 10 years ago but it still hurts just a little when I do certain exercise.

Unfortunately I think we are poor at correlating healed with perfectly returned to normal. Frankly I think all injuries to ankles should have physio therapy offered

I’ll admit, I can’t get wrapped up in the terminology. The crucial bit is Sensitivity: the ability of a test to correctly identify patients with a disease. Specificity: the ability of a test to correctly identify people without the disease.

Thought that was painful, until I rolled my ankle 5 years ago at age 35 and the pain that followed in the weeks after made broken ankle pain feel like nothing. It was so swollen I couldn’t get a shoe on for days. Painful to even try for a sock. Why was the pain so much worse than it was for a break?

@@DrJamesGill Yeah and everywhere the ground is very uneven in a lot of cities Scotland and England and too many people keep falling due to unkept roads and not really enough path ways that haven’t been taken by the Land since the 1950s so a lot of people have to walk on uneven roads to get places

Back in 2018 i had a medial malleolus fracture from drunkenly jumping off a wall and I was genuinely confused on how such an insignificant looking fracture caused 8 weeks of recovery and a month of stomach injections😂
