The ketogenic diet is very special and is the only thing that has ever helped me to look and feel my best. When I'm in ketosis (using the keto mojo), all my mental health issues go into full remission. I maintain my weight, my brain works properly, my appetite is regulated and I function optimally. If I eat sugary fruits or even too much protein and knock myself out, I soon get hungry, lose focus, get tired and irritable and within a few days, the anxiety, ADHD symptoms and depression creep back in. Bread has the worst effect on me. I'm my own N of 1 experiment and have prob been in ketosis more than 80% of the time in the last 25 years. I'm 60, have no diagnosis and am on no medications. There is no doubt about the power of ketones.

Hey Layne, not sure you'll see this but I can actually speak to this subject as a psychiatric nurse practitioner. It's been theorized that ketogenic diet might have some benefit in bipolar for a while now. The theory is actually born out of similar treatments for seizure disorders where the ketogenic diet has shown clear improvements with epilepsy. There's actually a lot of similarities between the types of medications that are used to treat bipolar disorder and seizure disorder and some speculation that the neurological conditions that lead to both have some overlap. Also, speaking to your note of correlation between obesity and bipolar/schizophrenia. A variable you didn't mention is the medical treatment options for both of those conditions very commonly cause significant weight gain. Thanks!

I know it’s anecdotal, but my bipolar sister was really helped by the keto diet. She wasn’t overweight when her bipolar episodes started, and she didn’t loose weight when she went on the diet. Since being keto for 5+ years she has been able to almost entirely come off the antipsychotic medications without negative side effects.

This data is not clear enough. Maybe for patients that are unable to function normally in society. I could see this being beneficial. But for individuals for have depression, anxiety, ptsd and being overweight. Its all variant on the person. Which makes this claim shakey. I've had a lot of my friends and family, inwhich who has those symptoms did keto and for that entire 6months-1 year. they were miserable and there psychological symptoms got worse. So yeah..... Honestly, this varies from person to person. I'd much rather a study stating what Dr Layne had mentioned. Eating what you deem is fit for your body and being active. That has more proof in the pudding than something like this.

As others have alluded to, the ketogenic diet was actually invented by doctors in the 1920s as an effective treatment for epilepsy (not as a weight loss diet), and fell out of favor once anticonvulsants started to come out that were easier to adhere to and roughly as effective. Multiple medications, such as lamotrigine and valproate, are very commonly prescribed for both epilepsy and as mood stabilizers in conditions like bipolar. Your conclusion seems very reasonable, but I definitely think it’s worth mentioning that mechanistically the keto diet is actually very compelling as a potential treatment option for these conditions. And anecdotally (for what it’s worth), I do know people that were very metabolically healthy and saw significant improvements on the keto diet for their conditions.

As for my experience: Bipolar 1. I use to not be able to manage episodes at all while struggling with binge eating (I was having A LOT of sugar all together) I was feeling worse then ever, now (not binging anymore but eating carbs -AND sugar in a “normal” way) . I have such a better power over my mind and I didn’t have to quit all carbs, just stop binging on it. Ps: I am a very lean person and I always was. I was just binging and restricting for many years unfortunately. For my where the binges that gave me so much lethargy, brain for and feeling of heaviness that when paired with a depressive episode would make it spiraling downward so fast it’s crazy… when manic I wouldn’t eat much and I was always in the run so I did not get that food intoxication feeling. Now that I have a balanced nutritious diet my episodes are gone down close to zero and they last nothing compared to before

Another consideration (this is only a pilot study, needs a follow up obviously) is antipsychotics tend to be obesogenic. There is a study on those with eating disorders who are weight stable and they kept calories the same, eating disorder improved. It has to do with the brain metabolism of ketones not losing weight or lowering calories. Here is an interview with the author: kzbin.info/www/bejne/Z33Xg4eNfN2bqZo I believe the study finished and the results were positive. Lots of ongoing research for keto and mental health conditions in stable caloric conditions as well as weight loss

My experience in mental health comes from Depression. In my experience one problem doesn't come alone. It brings some friends along. For me depression began along with paranoia, a new fear of the dark at 22, panic attacks, anorexia (started out overweight, was on the verge of underweight at the end) and learnt helplessness. Exercise for me was the thing that helped getting the symptoms in check. Because of exercise and wanting to improve I at least ate something. The weightloss also was very much wanted allthough the goal was a little too low. Resistance training later was what helped the most. Seeing myself being able to do things I was not able to do the week before was huge. Depression itself an learnt helplessness is what stuck around the longest. The latter still hasen't completely let go of mysel even more than 10 years later. I don't see a correlation between my weight and my mental health. I do see the regular feeling better when diet is good (fresh homecooked meals, not too much sugar, fruits and veggies) and exerciseing regularly.

The hair loss could be from diet fatigue too however, I have heard low carb diets can cause hair loss. When I was going through hair loss it was handfuls not just the daily 100 hairs a day lost. I've worked with individuals with mental illness and disabilities for over 20 years. Antipsychotic and other psychiatric medications almost across the board have the side effect of weight gain. Additionally nicotine reduces schizophrenia induced psychosis and is tied to weight loss, but we don't suggest people take up smoking to alleviate their symptoms.

Keto is used therapeutically to treat Epilepsy and severe seizures. High carb diets cannot do this regardless of calories. Dr. Chris Palmer of Harvard has used keto to treat people with very severe hallucinations. The research is very nascent. It is very fair to critique the limitations, but keto does have calorie independent effects on the brain and neurons as evidenced by the use in epilepsy. The hypothesis is laid out in Palmer's book called Brain Energy, which posits that mental health issues are a side effect of metabolic disfunction in the brain. One way to address this is through the ketogenic diet which has very specific effects on mitochondria and ATP generation through alternate, non-glycolytic pathways. Ketones are also another source of fuel for the brain, which bypass the problem with glycolytic pathways altogether. You should look into it more and do a deeper dive. I think it is a very interesting topic and holds a lot of promise for very diseased individuals. Have a great day Layne and thanks for the content.

At what price though I‘m concerned. I believe we are putting our body in a permanent stress fight/flight metabolism and overwriting the safety switch to turn it off. Ketones, as far as I understood rise in stress as glucose is used up rapidly. The safety switch we see in chronic stress. Low mood, fatigue, immune system suppression . The body is calling for replenishment of resources, rest, stress relief and after that to also directs the bodies resources to all tissues again, not only focused on the most important one. With keto, the safety switch is overwritten. During ketosis, we force the circulation, heart, brain and muscle to run more efficiently in expense of other tissues. We are doping. I think it‘s kind of like a performance enhancing drug letting the body run at 120 % capacity. This is great in life or death situations. One claim of the keto diet is that it reduces pain. During acute stress endorphins are released to suppress pain. Guess what can cause endorphin release? Ketones. Endorphines cause the release of several neurotransmitters. They make you happy and alert. Ketones widen blood vessels in the periphery increasing perfusion to heart, brain, muscles. Ketones stimulate EPO and thus stress erythropoesis transporting more oxygen fast. Ketones suppress hunger drastically. During ketosis, electrolytes and other recurves are transported to the liver. The blood stream becomes depleted. Nutrient absorption in the GI reduces as well. In other words, if eating a keto diet - I think is like being permanently high and constantly ignoring pain, hunger and tiredness. And I think this will take the same toll on the body long term as using performance enhancing drugs 24/7. There is evidence in rodents for this. Rapid aging of the cells in internal organs due to ketosis. Drugs like cocaine do that. I think chronic stress is not the way to a better health, even if there is a way to suppress the immediate negative effects. Ketosis would have to be used precisely as a treatment. Short term, or repeatedly with small breaks, giving time to replenish and recover. That‘s my idea. So I would say intermittent fasting would be far better. It‘s even better for the neurotransmitters they are boosting. Psychopharmaka can have a loss of effect with time due to dependence. Breaks might help to avoid that with ketones.

re pain and anxiety - i dont have any type of chronic pain illness BUT i have pretty strong bouts of anxiety/OCD. actually before really working on my mental health i would experience very significant, debilitating menstrual cramps (whereas now theyre bearable to the extent that i can continue strength training without too much of a hitch during my cycle). for me, recognizing my thoughts as just thoughts and slowly improving/increasing my tolerance to the physical sensation of anxiety (sympathetic activation) improved my symptoms quite a bit. and - somehow increasing the tolerance for this also reduced how MUCH i experience that fight/flight in response to every little thought i may have. to bring it back to my original point, letting thoughts be thoughts DID really help my menstrual cramps and i think that goes quite a bit with what Layne is saying! not saying this is true for everyone but it was shocking how significant it was (for me)!

I know it’s anecdotal, but my sister was diagnosed with schizophrenia and bipolar by different doctors. She had severe Hallucinations in the worst of her episodes and the paranoid delusions were constant. My parents were able to get her on an antipsychotic, but the keto diet has done wonders for her as well. She wasn’t ever significantly overweight, and when she initially went on the diet she actually put on weight. She has been following the keto diet for more than 5 years and it has slowly allowed her to come off her antipsychotic medication and recognize her delusions when she has them. I know it’s all anecdotal, but keto really helped her regain control of her mind.

After a bout of obesity at the age of 10, I started becoming slightly more "health conscious" and did the slimfast diet for a few months around the age of 17. although I did lose body fat, I looked malnourished. After 23 more years of guesstimating a caloric deficit on a majority frozen food diet- fat mass increased. I am 5' 7" and my body weight was at 157 in the beginning of 2018- I had fat stored in adipose tissue all over the place, but the worst was around the midsection and chest. I could not stand the sight of myself with my shirt off in the mirror. In February of 2018, I decided to do the ketogenic diet, and I was focused... during the first three months I lost a marked amount of weight, getting down as far as 137. The trunk fat had all but disappeared and I could see my abs. I estimate that I went from 24% body fat to 14% in that short time. In tandem, I also started strength training on a disciplined, regular basis and IF, where I skipped breakfast six days and OMAD for the last day. After a year, I increased fat calories because I heard some KZbinrs saying "calories don't matter." What is obvious to Dr. Norton but wasn't to me at the time, that the increase caused me to start gaining weight in body fat. I was easily able to reverse the trend however, and now I seem to be the same body composition of about 14% body fat, but gained 6 pounds of muscle over the course of these last few years. I know it isn't a lot, but I am an old guy who started late and am too scared of getting fat again. Calories are very important. Although my body was metabolically healthy and was conditioned it to prioritize fat for fuel, I learned from Dr. Norton that fat stores quite easily and many of the foods are more calorically dense than their carb counterparts. Today, I prioritize protein and fiber, eat carbs post-workout, but maintain a low carb protocol the days I am not strength training. I still OMAD once a week, but I eat breakfast again.

At least they did a study. Interestingly the keto diet is used in migraine patients - lots out of Italy. But of course as everything! Not for everyone and not for every brain. There are benefits and risks for all. Meds used for these dx have their own side effects that aren’t desirable . Some people may find that the dramatic change in their diet more beneficial than being prescribed medications that dull their cognition or lead to insulin resistance (lithium, antipsychotics). Very difficult to become metabolically healthier on antipsychotics. Agree control arm is important- I think that is the goal of this first line of research. A medical ketogenic diet must be monitored and prescribed by a doctor and RD to be safe, especially for neuro dignoses.

That being said I personally found (anecdotally) that keto seemed to help tremendously with my adhd symptoms. It helped me stay focused longer with less susceptibility to distraction. I used it periodically during exam prep times for example, or other professional times where sitting down and staying focused while sedentary was critical. It may be psychosomatic but I definitely noticed a difference. My baseline is probably lower carb than average as I don’t really like sweet im more of an instinctive meat dominant diet with conscious inclusion of potatoes and rice for training performance

Question about clinical studies: do RCTs get funding if there are no case reports or other type of studies showing at least a safety or possible benefit?
@fran6b

The big pro with keto, and for most of specific no-no diets, is that it provide a foolproof method to control your calories intake, which is the sinews of war. Not ideal, because you often miss the benefit of a balance diet of macros (or micros), but way better that what you'd normally eat , i.e. too much junk. I totally get that whit no clear hard rules, it can be extremely difficult to eat a non restrictive balance diet, thus the need to believe in some particular narratives.

Whether or not long term sustained ketosis is of greater utility surrounding a mental health protocol is certainly much more debatable. All in all I find this video interesting but of little utility

Yes, understand your view point. Bad studies... However, in clinical practice have seen mental health improvements with very thin patients using a keto diet. Truly believe it is helpful but only one nail out of the coffin. Agree there are many factors. Have also found the #1 fastest way to make improvements in mental health disorders is a significant change in the person's environment, living location and lifestyle at the same time the diet is implemented.
@MrSpwn
@MrSpwn 6 ай бұрын
Very thin patients? Likely because they are now actually getting calories, nutrients, and enough fat to support cognitive functions. Very thin = disease and/or malnutrition (or extreme endurance athlete lol). I'd argue that if you had them eat a diet w/ carbs AND higher amounts of fat, you'd see the same improvements. Again, I think it comes down to establishing a healthy body weight and diet providing the right amount of calories and nutrients and not some magic about one diet or another. However, if someone who can't adhere to any other diet than Keto to maintain a healthy weight and get proper nutrition... then that does make Keto special for that person. Your point about environment though, spot on.

@@MrSpwn low body fat doesn't automatically= disease or mal nutrition. And, everyone, everyone has some level of disease or issues, guaranteed. Place any person on an MRI, do a whole body scan, take your time, you will find something wrong. I also agree, there might be other methods, just what has worked for those persons. This is the problem with "studies", almost unlimited permutations, combinations, variations......

The fact they don't have either a control group or even better, a comparison with other weight loss diets presents a lot of difficulties with interpretation. It really needs to be a random study that compares weight loss vs weight loss on a specific diet before you can actually draw any conclusions about how well a keto diet works. Further, as this isn't a blind study, you also have an issue with what are known as 'researcher demand characteristics.' This is where the researchers could influence the outcome by providing more support and positive feedback to individuals who are better adhering to the keto diet than those who aren't. This is something that really should have been addressed.

I have anecdotal experience with keto. I tried it to see what it was like 2 years ago for 3 months and noticed a few things: - I shed a lot of weight (15kg), but I suspect it's because of my second observation - I was not hungry... the first 3 days were very rough, but afterwords I didn't feel hunger pangs and my energy levels were extremely stable throughout the day, this made it easy to not eat as much which I think was the primary reason to losing weight - I was also able to run, cycle and do BJJ rolls for 1-2 hours at a time at a moderate pace without feeling exhausted - the first day I ate some carbs, I felt like I was high. My brain felt like it was buzzing. I ended up stopping because there were some downsides: - any type of sprinting or explosive activity was extremely difficult and would lead to cramping - I love carbs. Bread and potatoes are delicious, I don't want to live a life excluding food groups - the adaptation phase is horrible and it felt unsustainable to go back and forth long term - poo-fart gambles happened all the time My anecdotal conclusion is I suspect there's some truth to the hypothesis that what you eat will affect your hormones and that can have a drastic effect on your perceived hunger. But I guess we know that hormones are important considering that's basically how Ozempic works.

The 'keto flu' and the cramping from exercise are likely 'treatable' by supplementing magnesium and assuring adequate potassium intake. A lot of people get these from carbs and when they go keto, suddenly the changes in electrolyte balance may cause a more severe keto sickness and these cramps. Thanks for sharing

Here's the thing ketogenic diets were used to treat epilepsy - so I don't know if this diet treats these conditions or not - but its definitely an area for more research - because it passed the straw in the wind and possibly a hoop test in that direction - is it smoking gun - definitely not - doubly decisive - 100% no. So yes more investigation is needed - also people shouldn't be using diets like this for treatment based on their own research - OR quite frankly a KZbinr's - no matter how credentialled - its something they should be consulting the appropriate professional about.

The big mismatch here in terms of your analysis versus the keto people is that you view metabolic health as a separate state from how one gets there, whereas keto people believe that a low-carb diet is necessary in order to be metabolically healthy meaning they don’t distinguish between the diet and the results that the diet provides because they believe that a high carb diet inherently causes insulin resistance, which of course is wrong, but that’s what they believe.

Kind of excited to see you tackle a study on diet and mental health. Especially surrounding the keto diet. I agree with you that one study is certainly not enough, but considering the keto diet was originally created to help treat epilepsy and anticonvulsant medication are sometimes used to help treat schizophrenia and bipolar does make me wonder if there's something there. But you're right that this study still leaves a number of questions unanswered, and even then, we shouldn't act like keto is a cure-all and overhype what it MIGHT be able to do. Even if it turns out that it is helpful for those with schizophrenia and bipolar disorder, that doesn't mean it's going to cure ALL other neurological disorders anyways. We all need to be careful about the claims we make and back them up first. As a mental health therapist, I really want to believe that maybe we've found something to help cure these kinds of ailments, but it's far too soon to start saying that just yet. We're talking about peoples' lives here, and that's what should be our focus. Not trying to force our favorite diet or lifestyle as the cure. Besides, this stuff can be very individualized anyways. What works for one may be the exact opposite of what someone else needs mainly because there's a lot of stuff we're still in the dark about. Also, I was gobsmacked when they tried to downplay exercise. I feel like there's enough research to say that exercise is an important foundational element to good physical AND mental health. Anyways, thanks for all that you do and for sharing your thoughts on this paper.

You have to understand the complexities of sugar and other simple carbohydrate with this cohort. Between the disease and the drugs used for treatment, they do have a greater than expected carbohydrate use. Improving the diets have proven beneficial. This study offends me…my dad has schizophrenia and I can tell you that there is no possible way he would ever possibly be able to be adherent to a ketogenic diet. He has much bigger things that he focuses all of his mental energy on (visions of bomb threats being made against our President, sex with invisible witches…I kid you not). And I know several people who are bipolar and managing via meds and would argue a similar argument…Did they even look at what “adherence” really meant compared to what was reported? People lost weight and increased health markers so they were able to adhere to some extent. But I honestly think that mental health impeding with any health habit improvements and absolute adherence to a new diet would be a major factor in any similar study.

That last thing you said bothered me. As far as I'm aware (correct me if I'm wrong), there hasn't yet been indication of whether the anxiety/depression has a causal relationship with pain/autoimmune disorders or whether people with pain/autoimmune disorders develop anxiety/depression from the difficulties they face. Anecdotally, I was an incredibly optimistic, non-nervous person until years of persistent symptoms and having several doctors unable to diagnose the problem causing immense anxiety and frustration by claiming I was imagining things. Being forced, over several years, to learn over and over that to be unprepared for a sudden flare of symptoms would cause major issues in the moment, I developed further anxiety about being prepared for every eventuality before leaving the house. Ultimately, getting diagnosed and getting a handle on my condition with drugs that quieted the symptoms DRASTICALLY reduced my anxiety. But the anxiety/depression most certainly did not come first.

I think it can go both ways depending on the circumstances. In your case your pain/autoimmune disorder caused anxiety/depression, however I'd imagine anxiety/depression also makes you more sensitive to pain in general. I have limited knowledge on this topic, but I could for example argue that anxiety decreases testosterone due to increased stress, and that this usually leads to increased pain sensitiviy, because testosterone usually decreases pain in people.

@@ginoyesano5649 We can't say that either. I have no scientific evidence to definitively say my condition or experience caused anxiety/depression. It could just as easily be that I have, say, some sort of virus that triggered both. It could be they are unrelated: It could be I was genetically predisposed to an autoimmune condition and also, separately, developed a terrible diet that somehow prompted anxiety. I obviously don't have enough data to say anything beyond, "I have X, and I have Y."
