Thank you for the common sense and easy to understand explanation. I was diagnosed as diabetic in Oct 2019 with an A1C of 7.7. I went ‘Ketonian’ and started intermittent fasting and have since dropped - and maintained - my A1C to 4.8. Off all meds, lowered my cholesterol and dropped 70 pounds. But during all that, I never really knew the medical science behind what was going on - until now. Thanks for the knowledge!

With rampant metabolic illness in the USA UK and Canada, "they" can't claim ANY kind of competence in the chronic illness camp of medicine (though they do GREAT with the mechanical side). The awareness of the incompetence of the medical community began when I visited a friend of mine who just had a mastectomy. When they brought in a plate of CRAP for her to eat I was incredibly appalled by that hospital's complete ignorance of nutrition! So, years later I find you. I am back to eating keto again. My blood glucose has been 120 fasting. I’m high risk for Alzheimer’s my mother and all her 6 siblings have/had it. This morning glucose fasting 71! It’s amazing what our bodies can do when we stop filling it with crap foods!

Omg. I remember my Dad's Mom had AD and always had a candy dish out with hard candy. She also had phlegm all the time. Well, I look like my Dad who died at 77 of massive heart attack. He also had auto immune hepatitis. So I got my 23 & me and have APOE4 only one. I know it was from Dad's side since Mom turned 90 last month and is sharp as a tack. All the more reason to stay keto. I had A1C 4.7 and 5.0. Before keto it was 5.7.

My husband is Type 1 diabetic, on an insulin pump,, his non-fasting instant A1C in office was 7.9 in September. He started Keto mid October, and his non-fasting instant A1C went to 7.2 when he went to his Endocrinologist in December. His blood sugars have been steady at an average of 130. This is instead of all the highs and lows he had pre-keto. Headed in right direction!!

Does he have continuous glucose monitor? I strongly recommend one. I have been keto for 7 wks. I had to lower basal rates twice since starting. In 7 wks I’ve decreased my insulin (basal and boluses) to 1/4 of previous use. My hgbA1c in Dec was 7.2. Next one is end of April. My average BG over past 30 days is 102. Am anxious to see new result. But still expect about 6.2 since I will only be 10 wks into keto lifestyle. I rarely need to bolus now and that’s usually when I’m under stress and spike hi BG. I now give 1 unit bolus if BG >120 which I never would have in the past. It’s definitely a fine line. Hopefully his doc will assist with pump changes. I’ve been doing it myself since I have no endocrinologist but am under care of family practice (and I’m retired IVU RN). Key is small changes to pump. Good luck!!

His numbers show it is, and why wouldn't it? My husband has been diabetic since the age of 2, been on the pump for years now, (not sure how many).

Whenever I discuss health with friends or coworkers, they are so fixated on cholesterol, and then when I mention the HbA1c , most have no clue what it is unless they are already diabetic. Even the diabetics, who have some high level understanding of HbA1c , still view cholesterol as the primary villain in vascular disease. It just illustrates the misplaced priorities of our medical industry and how this percolates down to the masses in misinformation. This is why I am so thankful for professionals like you who are focusing on the right target.

This is so profound yet accessible to grasp by a layperson. I think about a word to describe the relationship with my very nice PCP over the last 12 years. Malpractice. In the nicest way possible it's nothing short of malpractice. Dr. Cywes I very much appreciate you putting this kind of knowledge out into the universe. This helps me become empowered to be as I believe Dr. Berry coined it: A citizen scientist. I'm alive at a time in history when someone like me can gain knowledge on HgA1c and tie that to carb management (which I know now is tied to my emotional mgt system). It's videos like this that also help me think about keto cheat days as well. I do have a QUESTION for Dr. Cywes but it can wait until our next consult as well if you don't get to this comment ;). What about poor blokes like me that were in that "you're OK" zone for many years when I was actually pre-diabetic but told year and year I wasn't quite ready for to be officially diagnosed as Type 2? The damage I've no doubt done to my endothelial lining....I assume it can't be reversed but it can at least be arrested yes? Also, with respect to the damage done by the glycation that got to all of my organs over the years can that be reversed?

You are spot on! This video reinforces everything I have been learning. I have been keto (20 grams or less of carbs) for a little over a year. My Hg A1c is 5.4. It will be interesting to see what it is in six months. I was never diagnosed as prediabetic. The one question I have is from a presentation that was done at one of the keto conferences. I do not remember who the speaker was but it was a prominent keto person. What they were saying was that when you are keto your red blood cells actually live longer and thus have more time to be glycated. That is a very interesting wrinkle in A1c testing. I am curious to see what will happen in six months. Thank you for your thought provoking and totally accurate presentation!

I am so appreciative having found you through my sister in law. I have just been diagnosed as "insulin resistant" or "prediabetic". I have an A1c of 5.7. And yes, I was told that I needed to be careful. I have been struggling with bouts of carbohydrate bingeing and abstaining all my life! I have been told it is not possible to be addicted to sugar, to carbohydrates, I just need to eat in moderation. There is no moderation when it comes to sugar for me! Your information around this has been incredibly valuable and life saving in fact! I have accepted that I am a "carbohydrate addict". I have no control around sugar - it's horrible. It's a relief actually to know and hopefully to be able to reverse any damage I may have done up to now. Many many thanks for telling it like it is!

Last two 6 month checkups my A1C was 4.5 so I'm one happy dude. Have to add that after 4 stents and a heart attack in 2017 I realized that the SAD was killing me and went keto Jan 1,2018. On my first 6 month checkup my A1C was 4.3 which my Dr said he had never seen anyone with a 4.3 so you can see keto works.

I just never realized and feel so foolish. Started Keto mid May after having Metformin dosage increased twice with last A1c in April of 7.4. August 9, my A1c is at 6.4 and as a side note I have lost 27 pounds. I am on a plateau but don’t care … will continue and hoping to have an A1c in the 5’s in November. I wish every doctor would have the eye opening that Dr. Cywes has had. After my quadruple bypass Dec. 11, 2019 several times a day for a couple days they were in there poking my finger and squeezing out a drop of blood out to determine whether to give me some insulin. The strange thing is that their menu from the cafeteria was clearly grossly top heavy with carbs, starches, and sugars. An extremely high percentage of medical professional and also nutritionists in charge of hospital menus are clueless about cause and effect.

A guy who was overweight, depressed, and prediabetic, ate ONLY potatoes and sweet potatoes ad libitum for one full year. He lost his weight, depression, and prediabetes, became physically active and was basically transformed for the better. His A1C was between 4.9 and 5.1, his HSCRP was low, and all other blood markers were excellent. His channel here on youtube is called "Spudfit". When I eat keto (high fat) my blood pressure goes way up after meals and my heart POUNDS and results in insomnia. My fasting blood sugars go up (over 100) and I generally don't feel well . I've got a very high CACS so I'm in a quandary and don't know what to believe. Appreciate any comments from anyone.

I relapsed on carbs and shot up to 5.8 from 5.5. I was quite dismayed but my doctor said don’t worry. He mentioned the 6.5 cut off and I thought “this guy doesn’t know a thing.” On the bright side, somehow my blood sugar is at 72, triglycerides are at 36, and HDL is 66. Going to laser focus on A1C now.

I'm 44, I have a blood clotting disorder and have had many tests and scans through the years looking for clots. In 2015 I had a DVT and a pulmonary embolism. As part of the workup in recovery, they did an ultrasound of the carotid artery and other arteries around my heart and neck. They said I had 70% plaque buildup in my carotid. I weighed close to 400 lb. Today I am down 127 lb... Have been eating low carb for the majority of the past 4 yrs. (Had a year in there where I tried eating higher carbs and failed miserably because I am a carb addict and regained 70 lb that I have had to relose). Anyway, I had an ultrasound of the carotid artery again this past fall and they had found no remarkable finding. Only mild plaque. I thank low carb eating for improved cardiovascular health! My BP has also gone down. So thankful!

Please don't wait. Start immediately by cutting out all processed foods and sugar. For food only shop in the fruit/vegetable isles and meat/fish isles. Keep your diet simple; wholefoods and home cooked to avoid all additives. And move as much as you possibly can. My Dad had two strokes. The doctors said he'd never walk, talk or use his arm again but they didn't realise how stubborn he was. After just a few months he was back to normal. You can do it too. Determination is key.

Thank you so much for further explaining a root cause of arterial inflammation. As you and I have discussed on previous zoom calls, monitoring critical markers helped me understand the dynamics of what the keto diet benefits are to reducing or eliminating arterial inflammation. Also your explanation helps me reduce my anxiety of eating fat because it is sugar and carbs that cause the inflammation.

Had an A1c of 6.8 (couple years ago) and my doctor said I was PRE-Diabetic! I have a family history of diabetes on both sides of family. I went low carb (less than 20 carbs, no cheating) and A1c went to 5.3 (a year ago). My doctor said I was great but my LDL was high and I should take a statin. I chose not to take meds as I feel great, but no mention of 5.3 still not great. I will make an appointment to get another A1c test to see if the additional year has improved my numbers. I'm consuming under 20g carbs a day and my FBS is still elevated. That tells me that my insulin is still very high and gluconeogenesis is keeping my sugar high enough to compensate. My "target" carbs is 3.4g, and hard to stay under.

Morgen, the more I study all this, the more I learn about the role of omega 6 fatty acids in fomenting insulin resistance. Suggest viewing Cate Shanahan, Chris Knobbe and Paul Mason videos on this subject. They will advise the elimination of "vegetable" oils and all that linoleic acid from the diet. Around my house, we call those fats "Devil Oils".

@@BeautifulGood713 No, i'm not on Kaiser. Doc just said we'd "keep an eye on it." I didn't realize it at the time, but some of my kidney function tests were not good either. They weren't so high that they would be flagged in her computer, but they weren't in the optimal range either. After I went back a couple months ago and got all my test results. I compared to the last two years and I looked up what everything meant. I realized how much Keto is helping me. Not just with my weight and fasting blood sugar but my kidney too. I was probably one doc visit away from diabetes and kidney disease. I started keto last June thinking it would be a crash diet, and it has completely changed my life.

An elevated A1C is end stage insulin resistance, occurring long after basal insulin levels have been increasing. Therefore lowering ones A1C is only the first step in reversing the actual problem which is the insulin resistance, not the elevated blood glucose levels.

I have a mutation to my G6PC2 gene. It makes a protein that is the last step in releasing glucose into the bloodstream, the consequence being higher than normal fasting blood sugar. My FSB, taken by a lab, has been 99-109, never higher or lower, for at least the last 30 years, and probably my entire life. I have had high blood pressure (since I was a teenager), a hyperactive thyroid, and lifelong weight gain, all of which I blame on my blood glucose levels. I am a career airline pilot, and because of this have never been able to stay on a Keto diet. Try doing Keto when half of your meals are eaten on airplanes, in airports, or in restaurants. Thanks to Covid, I was offered early retirement which started at the end of November. In that time span, despite including Thanksgiving, Christmas, and two birthdays (not mine), I have lost 20lbs., my blood pressure is down at least 10 points, and my knees no longer scream when descending stairs. I am in low optimal ketosis. My glucose levels however, have stubbornly refused to move. I expect them to never be normal.

Thanks Dr Cywes! Great video. Recently I had my HBA1C tested. It was 5.9, but my physician said it was normal. After watching yourself, Drs Ken Berry, Eric Berg, Jason Fung, and others, I knew that “normal” was not an accurate response to an obviously not normal test result. Thankfully, my physician has asked me to return after three months to get a progress test done. (I am on the ketogenic lifestyle). Without the invaluable information you and other wonderfully generous people provide, I would not be in a position to defend myself when given false information by my physician. I appreciate you.

I began watching your videos this past week. I have gone from being happy with the Keto "diet" my naturopath has me on for the past five months to realizing the massive implications of the lifestyle. My A1c numbers went from 5.9 to 5.6. I quite honestly had no clue what that meant, until now. I'm doubling down with greater understanding. I cannot tell you how grateful I am. Thank you doctor.

Your carb addiction and sweet tooth will lessen over time. Eat fatty meats to satiety first. Know that you are nourishing your body. As your stomach acid, etc., adapts to a new diet , you’ll be able o eat more meat to satisfy hunger. Get rid of all 5he unhealthful foods at home.

I was 5.3 before Keto so I caught it in time. Still, wish it had never got that high. Look forward to another blood test one day and we shall see it much lower.

Omg. Why?why?why? Thank you for very succinctly explaining this. Now I have the info of WHY I have to change my mindsets on foods for LIFE! 5.2 hmmm...goal So, 6 months dirty keto. 1 month carnivore-keto clean. Obese. Pcos(duh). Meta Syn. So, I should just hang in there cuz it seems like it’s gonna take atleast 3-6 months till I see some “health” happening on the outside. BUT, it is cuz all the health is happening INSIDE! So, I need patience, right? I.could.listen.to.you.all.day.long!!!! I’m gonna share and tell every single person again...about you! What a Dr! Oh...consultation???!!!! Blessings from Tx

I have been on my ideal weight, bmi, health markers for 4 years on keto omni carnivore.But I still fight carb addiction all the time and periodically I give into it and binge...in chocolate and cake.. Just saying people think it's easy but it really super difficult at times.

Suffer from heavy periods, anemia and result in abdominal hysterectomy. Left with small red blood cells and Stage 3 kidney disease, high blood pressure and heart failure! Not one physician checked my A 1 C! Carnivore allowed me to lose 25 lbs in 8 weeks, but since surgery having issues with digestion. Urgh...trying so hard to recover, but a slow go.

3 years on keto-vore , train heavy for 30 years , I am 42, train fasted, eat 1-2 meals per day , but last blood work, is not good, a1c is 5.6, crp 32, triglyceride are 1 , hdl 1.3, ldl 18 , insulin 4.3 after 12 hours fast

To correct the title and the thumbnail photo, the Hb in HbA1c is an abbreviation for HemogloBin. In contrast, Hg is the symbol for the element mercury. This is a meaningless typo, but for clarity people should know we are talking about HbA1c and that there is no such thing as HgA1c. Thanks for a great video.

@Monique Roberts Don’t quit! Keep on goin! I have found I need to focus on just today and each decision is really only 1 decision at a time. If I focus on (the 100 lbs)all the weight I need to loose, it seems insurmountable! We can do it! You can do it. Only 1 day at a time!

I was 540lbs a couple years ago when I recieved my Type 2 Diabetes diagnosis. At the time I had an A1C of 11. I flirted with keto for a while, but only started taking it seriously in the last year. Down 60lbs and my A1C has been down to 5.6. I thought I was doing pretty good, but I guess that I still have work to do.

@@robertcywes2966 I've seen enough of you videos to understand your stance on carb addiction, not drinking calories, and the No rule on fruit. What about lemon juice in your water? A lemon does have calories, and a few carbs, so I'm guessing no.

Excellent talk! I would add that hydration affects MCV (red blood cell volume) and so variations in MCV can affect HbA1c. It is often glossed over.

A year ago my A1C was 3.5 and had more energy & less hypothyroid + IBS problems - all those things worsened when it elevated to 5.....no diabetes in my family....but stroke, big-time!

My medical data March 2020 Diagnosed with type 2 diabetes, - HBA1C -March 2020 11.7 -June 2020 6.3 -April 2022, 4.9 -August 2022, 4.6 --- Weight - December 2019, 120 kg -August 2020, 89.25 -October 2021, 53.35 -January 2022, 65.85 -April 2022, 62.1 -July 2022, 60.6 --- -2020 on diabetes medication. - From January 2021 till today, no diabetes medication.

My dad had a 6 Dr Pepper per day habit & sneaked cigarettes. At 62 he had 4 bypasses & a LIMA, died a dozen years later. I wish I’d had this info to share with him, but damned if I’ll follow in his footsteps.

I just sent this presentation to my primary doctor who keeps telling me my A1c at 5.5 is completely normal. I have a very large family history of diabetes. Maybe I should look for a new doctor. Thank you so much for this information.

This is what I've been trying to explain to my family who are all diabetics (just without all the medical terminology, lol). They are obsessed with the number and don't understand the damage behind the number. They're more focused on what junk food they can get away with and keep the BG down with meds. Unfortunately, trying to eat healthy alone was too hard and I fell victim to an A1c of 6.6 on 12/7/2020. I quit carbs that day because I knew there was damage behind the number. I'll never go back to eating carbs again because I don't want to see if I can survive a heart attack without being a vegetable.

Thank you for this video. Every night I cringe as I watch the tv ads for drugs that claim they may get a persons A1c below 7. As if that was horribly hard to do. I took my A1c from over 12 to under 6 in six months and ditched the drugs simply be fumbling around with a do it yourself low carb diet. There is absolutely no excuse for the ADA or the medical community not encouraging people with type 2 to eat a truly low carb diet which would get their A1c's down to normal ranges.

Let go (of carbohydrates) and let God (strengthen you) has never meant so much to me as it does right now. As most things in life that are ultimately good for us the process is Simple but not always Easy. I've been struggling for 3 years, mentally, of how I'm going to live (enjoy) life without carbohydrates, though I eat better and in smaller amounts, I have not fully committed to this lifestyle.
