I wish I knew this 20 years ago. I have a slim top and big butt and big thighs. Currently taking progesterone and testosterone. I have dropped 69 pounds since following keto and HRT since 2014. It feels great to feel slim after so many years of being obese.

As a retired trained lymphedema therapist I wish the doctors who I trained with both in Canada and Germany were aware of the benefits the diet can bring to lipidema patients. We therapists are very well aware that lymphedema can lead to lipidema and vice versa. I sincery hope you get invited to lecture therapists in training and continuing education in their field. Most of the patients I helped mananaged their lymphedema or lipidema condition could not get their GP doctor recognize their condition. Many of them were misguided going on diuretics.

Please explain how I can slim my lipodemic legs? I’ve been eating low carb for three years and nothing seems to work. I’ve had my estrogen, progesterone, and testosterone levels checked. Estrogen was in the normal range, progesterone was borderline low, and free Testosterone was four points too high. I do have uterine fibroids but no ovarian cysts. So confused. :(

@Papa F Jose they're actually improving a bit. 20g a day is the goal, but zero carb really is the way to go. The doc checked for PCOS and I don't have it.

True. However, men do not completely lose their testosterone. Women who are POST-MENOPAUSAL have “0” estrogen, hence the post- menopause condition. Therefore, we post- menopausal women must replace the estrogen that we no longer have AND, if a woman still has her uterus, she must balance the estrogen with progesterone. There are certain women who cannot use HRT due to genetics and there are always some risks with HRT. Not taking HRT increases our risk to Alzheimer’s, breast cancer, uterine cancer, osteoporosis, and heart disease. So, be informed of your immediate family history and risks, such as, are you a smoker, heavy drinker, over-weight? Also, there is the “10 Year Window” you need to be aware of. If you have been post-Menopausal 10 years or more, you are not a candidate for HRT, because your arteries will not be able to handle the estrogen. I praise God for HRT!!! Due to my doctor’s ignorance and horrible knowledge of HRT, I suffered needlessly for 7 years! I finally cried out to God and He brought across my path the information I needed to confidently and boldly ask my doctor for the hormones I so desperately needed! I feel so vibrant again! I have my life back!!!

Very informative video ~ thank you. I just wish (as a Nursing Practitioner) that those who So need to hear this, would listen & change ways for a dramatic health difference (some do but most are So resistant & stubborn) believing the old fallacies which actually suit the addictions/beliefs they have with carbs. There is No shortcut, No quick fix, No magic pill. It's about going LCHF/Keto and changing thinking & lifestyle, forever 💘

I'm 37 and have PCOS. I'm TTC. Started keto/if in September.... Did SO GOOD and then fell off the keto/IF wagon on December 1st and am doing a 48 hour fast December 27th to start again.

I attended that lipadema seminar and appreciate your further elaboration here. Whatever can be done to bring lipedema out of the shadows, I am all for it. I've spent a lifetime being harassed with "eat less, move more" aka CICO advice and quite ready for a more enlightened medical community. Thanks for your attention to the details.

Thank you Dr. Cywes for having this nuanced discussion around hormonal regulation of fat. Gary Taubes, in many of his earlier presentations, when he was emerging as a leading voice on changing our paradigm on nutrition from a calorie centric model to a hormonal one, illustrated the power of hormones on fat deposition by showing naked photos of lipodystrophy patients. Lipodystrophy, for those reading this who are unacquainted with the concept, is a condition where people had an inability to store fat in areas in which most others can store fat. The result was that the person, afflicted with lipodystrophy, would be shaped odd, where they would have pronounced muscular definition in the upper body due to being incapable of storing subcutaneous fat in that region, but would look obese from their waist down. When showing this, Taubes would then issue a challenge to those who advocate for the CICO model, asking how would they explain this? Based on their paradigm, they would have to say that this person's lower body was gluttonous, while their upper body was anorexic. To this day, Taubes' illustration was one of the most powerful explanations demonstrating how inadequate the CICO model is, and why we need to focus more on the endocrinology behind fat deposition and hunger.

Thank you Dr! I have Lipedema and am doing the Carnivore diet ... have found it hard to lose the weight... but thanks to your explanation my caloric intake is too high ... will go with one high fat and protein meal a day instead of three!

I watched a few of your videos yesterday. I heard quite a HUGE nugget in one of them. I think you could do an entire video about this. This was a lightbulb moment for me! thank you. "Insulin blocks the conversion of cholesterol downstream to steroid hormones". This is SO big! I am in menopause and ALL hormones are low or not there at all. I am stepping up to do strict healthy keto again and feel better already.

I'm so glad someone in the keto world is so knowledgeable about lipidema! I've never been diagnosed with it, but I strongly suspect it in myself as I have that body shape where I store 98% of my weight in my hips to calves and my upper arms. I'm also one who has 2 mutated MTHFR genes. I know you're not one to put much behind MTHFR, but taking methylated B vitamins regulated my cycles even when I was still 400+ pounds, I believe by helping me to detox some of the excess estrogen. Then I've lost 195 pounds, 135 of those pounds in 27 months of keto and noticed my body evened out substantially even though I'm still shaped like that lipidema shape, just much less severe. I lost 15+ inches in my hips but only 10" in my waist so far. I know I'll never change my propensity to store weight in certain places, but it definitely helps to accept that and understand the cause and the dietary tools that will come the closest to fixing it. I interpret your 'no carbs and less food' for lipidema people as carnivore and more fasting are in order to take me to the next level!

PCOS can start as young as 10 - it attacks your thyroid causing hypothyroid - then your adrenals - causing weight gain - then your ovaries are filled with cysts causing hormonal problems and more weight gain, excess body hair, skin conditions, thinning hair and difficulties getting pregnant. Gestational diabetes often occurs with pregnancy and more weight gain. Often the Doctors often do not recognize this condition and it remains untreated until surgery is required to remove ovaries and your uterus. Then you are left with diabetes 2, hypothyroidism, fibro and being fat. One lives with organ damage until one dies. Not everyone is born equal.

I would like to hear more about Progesterone , I am a 62 year old woman and have been using it transdermaly for 25 ish years for bad pms . When I went through menopause I switched over to every day of the month for anxiety anger and sleep . So many women use estrogen in menopause I could not it was horrible for my emotions . I try to stick to a carnivore diet .

I’ve been living a Fasting/Keto lifestyle for 2 years. I’ve gone down a few dress sizes but the scale # hasn’t moved. I recently removed my IUD on the theory that my hormones weren’t in balance, which in turn kept me in state of Insulin Resistance. Could this be true? It’s hard to find any info about Progesterone levels and it’s effects to Insulin Resistance. Time will tell if Im right I suppose. Edit: It’s been 2 mos since my removal and I’ve lost 8lbs! I think I was right😀

thanks doc... always with exellent info... i was bodybuilder when i was younger... always very low subcutaneous fat, amaizing muscle quality, but a big belly.... i realized too late that was visceral fat... i was thinking was just because i was eating too much food, but no... i had a lot of fat around my organs.... i still havin some but much less, i had to leave the high testosterone doses, and just continue with TRT, it is crazy and hard to understand, but high test promote visceral fat.. i just wish to undertand the reasons, but it is what it is.. thanks again doc!

THANK YOU!!! Finally, someone can explain to me why I have been overweight since puberty, gain weight so quickly and have it all stored from the waist down. I can gain 10-20 lbs in a couple of weeks yet take months to lose it. Even doing Keto is slow going. You’d think my body was saving fat for the Apocalypse!!! I appreciate your direct and honest approach to carb addiction. 👍🏻 Keep it up. If you have time to give us the caloric numbers to keep below to actually lose weight faster for the lymphedemic type I would appreciate it. Take care.

What does it do to the body if one represses their hormone cycle by swallowing birth control pills continuously in order not to have a period? Does that elevate a particular hormone?

Thanks Dr. Cywes. I’m 27 and was diagnosed with PCOS over 10 years ago. I had a big belly and large butt/legs. A couple years ago, with fasting and a low carb diet, I lost over 100lbs and got pregnant with my son! Now I have a different problem. Since pregnancy, I’ve developed painful lipedema in my legs. I’m trying to up the fasting to see if it will help. I’ve still got 30-40 lbs I’d like to lose. I’m hoping it will resolve the pain.

My bariatric surgeon NEVER told me anything about carbohydrate reduction and that my body type was best recommended for bariatric surgery. I was told to stick to 1200 calories and it didn’t matter what those calories were. I wish I had known this (what you’re telling me here) before the surgery. I’ve been struggling ever since and am fatter now than ever. P. S. I have PCOS

Great explanation of body hormonal variations !! Enjoyed listening to this, and for me it confirms my whys and how’s to what I’m doing. I had PCOS confirmed in my early 20s, when I look back at photos I can tell through how I carried my weight etc. I have had 3 children but had gestational diabetes, which was at its worst by the 3rd child. This all makes sense how it’s is all related to insulin, and how careful I have to be as I get older especially with my pre exposure to diabetes. Thank you Dr Cywes 😊

One thing I'd love to know is what I should weigh. I'm 56, just now starting menopause female. Been keto 4 years now. I lost 50 lbs and feel good but I'm slowly losing now. I weigh 185 but wear the same size pants I wore when I was 20 and 120 lbs. Should we go by the BMI chart? Does it account for age? I had a goal of 150 to 155? My short goal is 180. My husband and I both do keto. Hes lost 115 lbs. We both have some skin left from weight loss too. Hes 62 still works full time as commercial carpenter. I work as a house cleaner. Very active people. We both reversed so many issues. Hes off 16 meds the VA had him on too. People don't recognize us, as we look so much different. But what should we weigh????

Great explanation Dr Cywes. What about women, like me who have one very large Ovarian Cyst. I was fertile with regular periods in my younger years. Fairly early menopause. Had to have major surgery to remove the large ovarian cyst and sadly also had my right ovary and fallopian tube removed as well. I did get pregnant after that with my daughter not long before my menopause commenced.

Great video Dr. Cywes! Answered some questions I’ve always had about myself, being stage 2 lipodemic since puberty. In my 30’s and still working toward having thin legs. I always lose fat around my waist and rear end first and quickly, leg fat is so stubborn even after six months of keto way of eating. I’m two different clothing sizes, small from waist up and medium-large on bottom. So frustrating.😞. I hope to consult with you in the near future.

I’m the exact same but not diagnosed with lipedema!!! Small upper body and med to large butt and legs!! Been carnivore for 15 months and gained 4 inches in my butt and 3 in each leg. Yup does not matter what food I eat it goes right to the butt and legs!!

@@corrie491 I've been trying a keto carnivorish diet for three months and haven't lost pounds but inches around my waist. I did lose a lot of weight on keto before I married my Italian husband. But after the wedding I adopted his way of eating and gained all the weight back, went back to keto but more carnivore, lost ten pounds and then got pregnant! After the birth of my baby, I lost all the pregnancy weight but been teetering with Italian pizza/pasta weight.🙈😭🤦‍♀️ I'm definitely not a moderator, I have to abstain from sugar and completely obliterated vegetable/seed oils from my diet. Eating animal fat, especially beef tallow, has completely killed my sugar/refined carb cravings! 😀

@@alba7165 yes I’m an abstainer too and why i had to go strict carnivore. It took about 3 hard months of eating lots of meat and fat for me to break cravings but now I’m craving free 🙌.

Listened twice.. this was a tremendous benefit and as I am following your Carnivore food plan, and at gym to increase muscle and heart/lung function with Stairmaster and w/High level fast pace..... I am an anticipating a great 1st appt.. And answers to my problem with your expertise in blood testing. Maybe.. I can get closer to the pic above.. at least in weight .. Appt. Thank you. I had premature adrenarche at age 3, followed by lipedema at age 5. Central precocious puberty age 8 then PCOS since the age of 10. Now 47 and starting to make sense of it all and reverse the damage caused by a lifetime of eating the wrong foods.

Dr. Cywes, thank you for making all this expertise freely and globally available. We live in Turkey, and all the physicians here are the way you describe: they do lab tests and throw medication at conditions with no wholistic or lifestyle approach. Besides, we can't afford anyone but state physicians, who don't like to talk to patients and don't like being asked anything or even talked to. I know I'm asking too much of you, but I have no other choice but to ask this to you directly: Does birth control medicine do any good or harm in PCOS (gynaecologists here prescribe it)? Can fasting work?The woman in question here is 25, 1.58cm - 47kg, has IBS, has lost 7-8 kgs, can't totally get off bread, doesn't like high protein - high fat foods (some chicken, very little fish, eggs, ground beef and cheese), and is the apple of my eye! I'd totally understand if you didn't answer it. I'm learning a lot from you. Thank you SO VERY MUCH!

The doctors in Italy are the same way! They don’t have any bedside manners, don’t introduce themselves as Doctor so and so, sometimes they don’t even wear a coat or scrubs- they wear street clothes so I can’t tell if they are doctors or not, they don’t like to hear new ideas about one’s own health. They just write prescriptions and follow protocols to the book. It’s so sad that is their healthcare...more like disease management.

Please tell us what Insulin level is best to strive for health benefits. . My level is 5.4 but I have hear Dr Paul Sabino Carnivore Dr.. said his is very low neat 1.5 if I remember correctly. Thanks for your fast reply... I see you mid - January 2021... Jacksonville for.. resistant fat loss on Carnivore.. Till then , I will continue on 16/8 or 18/6, grass-fed beef and suet or butter, eggs and a little bacon. Humic/Fulvic Minerals and Kangen Water. Best to you and your family.

Not all PCO patients have high testosterone levels. Also consider the hormone cortisone which also aids abdominal fat storage

59 year old male, 5'10". 2 years ago was 240lb, now 162. Still have some fat and loose skin. Off all meds, (BP, Asthma, daily pains) . But diagnosed as low T. So been doing T creams. Last measure was 743 with cream. A1C was 5.4 shortly after some hernia surgery, last Cpeptide was 2.7. Recent CAC score was 32. HDL 57, LDL 99, Trig 69. First time I have heard that Testerone will improve. Going to get tested again on Wed. Do I need to talk to Dr about decreasing use of gel.

Yep! It’s so wise to do your own research and use critical thinking. Women need estrogen to thrive! To think, or “reason” that once your menses has stopped, a woman suddenly doesn’t need her life-giving hormone, estrogen!?!? We give diabetics insulin(A Hormone) so that they can thrive and live longer.

So, I know carbs are an addiction. I have been saying this for along time. I know that I have an addictive personality because of things that have happened in my childhood and as an adult. Before my issues manifested in this addiction manifestation, they manifested in other ways. Now I'm 59, short and over weight, very metabolically impaired, I truly believe this, and have for some time. But I have been unable to MAINTAIN abstinence for an extended period of time. But for the time I do, I am successful in losing some weight. If you are in Jax, I am.not THAT far from you. I work in a hospital with psychiatry and know there are drugs to help addiction and have even thought about studying about them to maybe talk to the doctor about. That's how desperate I am for help.

Stop carbs as they make you starving. Dont have anything but good real food that you have to cook. Prep your meals. Distract yourself with pleasant activities.

Your story is common. Most people can "be good" for a while, but then the flood gates reopen. It really took an epiphany for me in order to look at food differently - more as energy than entertainment. (M 6'0" 352 > 192, age 56-58.) Extinguishing the carb cravings takes perhaps 6-8 weeks on strict keto, which is very difficult for addicts or people in difficult home/work environs. A self help group is beneficial for many, which is how weight watchers achieves most of its beneficial results (IMO.) You might try www.reddit/xxketo and seek out user/anbeav. Yours is not a one and done KZbin issue. Good luck.
