The Epley Maneuver for Vertigo can be very effective at relieving vertigo symptoms, but it’s a procedure that should be performed by a physical therapist or other health care professional. This video is for demonstration purposes only. See Doctor Jo’s blog post about the Epley Maneuver at: www.askdoctorjo.com/content/ve...
Again, the Epley Maneuver should be performed by a health professional. I do NOT recommend doing it on your own or having someone else who has never performed it before perform to perform it on you.
With that said, many of you have asked about it, so here is how it is performed. Always explain what you are going to do with the person. This will increase dizziness and it can be scary. It might also, in rare cases, cause someone to get sick, so I recommend having a bucket near by.
On a bed or treatment table, have the person get into a long sit position with their legs out in front of them. Get them close enough to the edge so their head will hang off when you bring them down. Turn their head to the involved side at a 45 degree angle (this is the side they feel dizzy when they turn in that direction). You want them to be completely relaxed, and you are doing all the movements for them. Then with their head turned, very quickly bring them down with their head off the edge in about 20-30 degrees extension. Hold them in this position for 30 seconds to a minute. You are looking for their eyes to be bouncing side to side. This is called a nystagmus. During this time, the person will feel dizzy, and might want to sit up. Try to keep them relaxed and let them know the dizziness should go away soon.
Once the eyes stop moving or slow down, then turn their head to the other side at a 45 degree angle and keep the 20-30 degrees extension. Wait another 30 seconds to a minute and look for the nystagmus. Hopefully it will be less already and they won’t feel as dizzy.
Now, keeping their head at a 45 degree angle, have them turn to their side, the way their head is already pointing. As they do this, slowly start to tuck their chin in. Wait another 30 seconds to a minute, and then slowly help them sit up by bringing their knees up and dropping their legs off the side while pushing up with their hand.
Slowly bring their head back into a neutral position looking straight forward, and hopefully all the spinning and dizziness will be gone. Now you should instruct them to keep that upright position for 48 hours, so they might have to sleep in a recliner or chair. Also, if you have a soft neck collar, that will help them keep their head in a neutral position to allow the crystals to settle back into their normal position.

Have done this with y wife a number of times over the years. Good to get the refresher. It definitely works. Sleeping in the recliner if the vertigo was strong is key to not having to do it again soon. Also, not working so you have to bend down with head low for a couple of days does help.

I continued to work and working down the hallways was un-nerving. I finally got an appt.set up with PT. I saw the therapist, explained my situation and immediately began the BPPV procedure, while she watch my eyes rolling trying to find the horizon.after about 10 minutes the steps were complete and the eyes had it. I've read a meta-study that said doing so isn't helpful. I also saw another video by another pair of physical therapists who say they just have the patient stay upright "the rest of the day" and sleep somewhat elevated if they can, for a single night. Most likely, it's not going to make a significant difference either way, and you can repeat the maneuver for as many days as needed, anyway.

I had vertigo many years in my 30s, then it reappeared in my late 40s but seems to be happening more often now that I’m 50. In the past, I could perform the Epley maneuver and it worked whenever I got vertigo or feeling super dizzy. Now, it’s been a little over a week and though it’s better, I still get dizzy if I bend over certain ways throughout the day. I’ll go home and try to lay back to reset the crystals but won’t get dizzy. It’s only when I bend over.

Read what I said here about my wife's experience. "My wife had vertigo for over 20 years. She went to a physio who said that my wife had the worst case she had seen. She explained that there are a number of procedures, including Epley, in order to address vertigo. My wife underwent treatment for one session, using an Epley type procedure. She went back, and the physio said the BPPV (vertigo) was completely gone. It seems incredible, but can be easily fixed." Advice: Get a person who really knows what they are doing. Ask around locally, and find someone very competent. The vertgo problem is about misplaced crystals in your ear canal. The procedure is horrible, with everything spinning according to my wife, but well worth it. Your vertigo will be gone!

AskDoctorJo Yes I have, and that’s great advice. Ultimately my physical therapist had me doing all necessary exercises for vestibular rehab which helped tremendously. I still have occasional vertigo from stroke fatigue but it’s better. After the stroke, I also needed PT for a loss of spacial acclimation. After being in the hospital my brain couldn’t adapt to larger spaces. Gradually and slowly building my brain’s ability to process open spaces took years of effort. Many of the concepts I used to improve, I fashioned from balance disorder PT. I’m doing much better now. PT is all about the details.

Thank you Dr. Jo for sharing this video. I’m just having the BPPV when I wake this morning, and searching for the treatment in youtube. I will ask someone in my family to learn your video, and treat it to me. My question is, how many time should I do this in a day, how many days and when the right time? Thank you, doctor Jo...🙏🙏

You're welcome. This particular one should only be done once. If it's truly BPPV, that is often enough. You might have to repeat it in a few days, but give it some time to work before trying again.

In 2001 I had BPPV and went through this procedure. It is scary and tough to suffer through...especially when you don't know what is happening to you. It caused many sleepless nights, time away from work, and anxiety attacks. Thankfully our hospital has a vestibular rehab center and they were able to treat me. I never found out what the root cause was, I wish I knew, but I know I never want to go through BPPV again.

I'm having BPPV it started yesterday. I told my parents that it's vertigo but they didn't believe me. They instead said I ate too much food last night, and my stomach didn't digest the food and that caused dizziness. I vomitted countless times it was quite hard for me to vomit. I treated my vertigo yesterday it was gone. But it came back earlier. I can't handle the extreme dizziness. There was this time a thought came to my head. It's better to die than to feel this extreme dizziness forever. I'm having vertigo and at the same time the mucus(phlegm) is thick. I have multiple vertigos. I'm all alone facing this problem.

2 years ago after contracting covid I ended up in hospital for a week with a migraine and vertigo which I couldn't shake. I had the Epley treatment by a physio for awhile once out of hospital. It seemed to help. It did make me quite nauseous during the treatment. Now I seem to get vertigo when I get headaches which is often or when I lay down to sleep.

Hello Dr. Jo. First of all, thank you for the detailed explanation on the Epley maneuver. I suffered from vertigo after getting hit in the back of my head. This maneuver cured the vertigo, but I have been feeling a little dizzy and off-balance afterwards. I heard that this is called residual bppv, and it takes a while for your body to adjust. My question is, how long would it take for the dizziness or light headed feeling to go away after this maneuver is done. Thank you again for your help!

This is the maneuver that the doctor told me i need to do to get rid of it, but she didn't really explain it all that well, and so far i haven't noticed any improvement at all, so i either have a different kind of vertigo, or I've been doing this maneuver wrong all week

I took my son camping in a tent in 2017, it went from hot to cold and damp and I didn't sleep well if at all, and I woke up the next morning and fell over after sitting up in the tent. It was so bad I thought i had a stroke or something, had never had vertigo before. I was so off balance, I couldn't turn my head on the trip back while driving and had to take it very slow. Ended up getting a CT or mri to be safe, which was normal, and a week later an ENT did eply in his office, and I was 100% cured. I couldn't believe it. I've done it from home a couple of times since, and it always helps, though not to the effect the ENT had.

Will one epley fix BPPV? I have been so careful since the real bad vertigo that I am just off balance in the morning. I am going to an ENT Monday to have this maneuver done. I just want to be back to normal. Now I am anxious a lot and scared. I feel like it takes me 3 hours in the morning to get passed the anxiety and general off balance feeling. The shower is difficult and driving I get anxious.i will continue my daily activities because I have kids that need me. to live like this is awful. I am praying the epley works Monday and I can resume normal life. I hear sometimes it fixes people the first time. Praying that it does that for me! Thanks for the video

When I was diagnosed by the doctor, she said it was on the left side. I did this Epley Maneuver at the physio, and then at home a couple of days later. On both occasions I felt nauseous and headachy for hours after - is this normal? I slept sitting up with a soft collar, but getting out of bed and getting ready for the day i still felt disoriented. also when i did this i felt equally dizzy on both sides. this began a week ago. The symptoms began Sunday, Saw Dr on Monday, did the Brandt-Daroff exercise 3 times a day for 2 days, Wednesday, went to the physio to have the Epley Maneuver done, felt awful directly after the procedure and the following day, Friday things were looking up, then went back to the physio for some neck work, then when I was sitting up after treatment all the symptoms returned with a vengence. He then did the maneuver again and I had all the normal spinning symptoms, but again felt disoriented that night and the next day - Saturday. I decided to do the Maneuver myself that night (last night) - i still feel awful, hence my questions. thanks for your video, is very comprehensive, and I do this at home again, I will have someone help me as per your instructions
@AskDoctorJo
@AskDoctorJo 7 жыл бұрын
When you say you feel equally dizzy on both sides, are you doing the Epley on both sides or do you mean you feel dizzy when you turn both ways? Usually the symptoms only last for an hour or two later, but shouldn't last for days later. Sometimes the crystals are out in different canals, and a different technique is required. Unfortunately, I don't really know without getting to evaluate you. I would recommend asking your PT about it, and if he says those long effects are normal or doesn't have an explanation, I would see if you can get another therapist. Good luck.
