Thank you so much for this contribution. I work with dental surgeons and I have often personally observed how ergonomics is a greatly underestimated matter, not only for the operators’ health but as a substantial contribution to the quality of the treatment.

I agree. I was in an oral surgery fellowship program at Baylor Dental College for 2 years following dental school. Correct seated and standing position is so important, especially after one has ben in practice a long time. Thank you for this! Dental assistant for 10 years, and I can attest to this…ergonomics are so important! As a right handed individual, I’ve found it’s much easier to assist a left handed dentist.

I really needed this video 😭 last Monday i had my mid exam, i did class 2 amulgum restoration on upper right first molar it went well but i struggled a lot to maintain proper vision and be able to operate Next time I will try to use a bigger mirror. thank you doctor

Yes. Lean the patient back as far as they can comfortably go. Sometimes it is easier for you and your assistant raise the dental chair and stand up for some procedures. Hi Dr ! how do you deal with water and fogging when using indirect vision ?

When working on maxillary teeth in the posterior are on the palatal, water spray is always an issue. I always start the handpiece and get that initial burst of water out of the handpiece so it does not wet the mirror. Then I try to keep the mirror as far away from the operating field/water spray as possible. The rubber dam method I teach is very helpful. I always rub the mirror on the inside of the patient's cheek initially so it does not fog. Take your practice to the Top Tier. I would say about 50% of patients accept the use of mouth props. Also, you did not use loupes for this video. Loupes can help with maintaining proper posture while working. The retraction mirror ( or whatever it's called) is a nice tip...

I do use loops (2X). I don't ask the patient if they are OK with a mouth prop (I call it a "tooth rest") or rubber dam. I just tell them what it is and the benefits, briefly, as I am placing them. I don't want the patient to have to make decisions if the procedure is in their best interest. It's like a hygienist asking their patient if they want to schedule their next appointment. I want the hygiene patient to schedule their next hygiene appointment when they leave the office from their current appointment so my receptionist doesn't have to call many patients scheduling hygiene appointments. If something is in the patient's best interest and doesn't harm them, just do it and stop asking if they want it. This also reminds me of parents constantly asking their small children what they want, i.e., "do you want to eat some fruit or would you rather have some candy?" You are the parent. The small child does not have enough experience to know what is best for them. Teach them to eat things that are good for them. Of course the child will say they want the candy! I loved being with my grandfather when I was a child. He was an old rancher. When my brother and I, and our cousins, were about 9 and 10, we would go to a diner in the small town near our ranch for lunch after working on the ranch all morning. We were hungry! My grandfather would have the only menu. He would tell the waitress, "We'll have 5 chicken fried steaks with French fries and 5 glasses of tea" or "We'll have 5 hamburgers with mustard, hold the onions and mayonnaise, with French fries and tea." Our young minds were uncluttered with unnecessary decision making and he taught us to like different kinds of food that we might not have known to order. To this day I like chicken fried steak and hamburgers just the way he ordered them 50 years ago. Take your practice to the Top Tier. Years ago the cost was $20/month. When our costs jumped the past few years, the monthly cost went to $39.95. Still inexpensive, but the cost of everything in life has gone up. We keep the cost as low as possible, but we have an editor, videographer, many computers and cameras, an editing studio and a film studio. It is expensive to produce the videos. Which type of loupes do you suggest for new grad(2x-2.5x-3x, through lens or flip ups)? And do you always use the air spray on the mirror while working? Thank you

I use 2 or 2.5 power loupes. 3 and 4 power hurts my eyes and it is too hard to focus. I use through the lens, not flip up. I usually just periodically wipe the mirror with 2x2s and hold it away from the working field. I like working with narrow, occlusal mirrors vs. the small round mouth mirrors. The assistants chair is hilarious. I think her resting on the bar may also be an issue coming forward. The Vaseline will spread over the dentists fingers this insecuring the grip on the bur. I tend to grease only after the treatment.

talking about ergonomics? in this small room the dentist cannot sit in the 11 o'clock position when the patient is in supine position,and there is not enough room for a disabled person in a wheel chair!

Move to a larger office. This work is what you will be doing every day for many years, so put yourself into a comfortable work environment or you will dread going to work every day. I purchased the dental office adjacent to my office 20+ years ago and took out some walls in the new office and turned 2 operators and a hall into 1 big operatory. I do my all morning cases in that operatory. In addition to the operatory chair, there is a couch, a comfortable chair and a table and lamp, similar to my home. Take your practice to the Top Tier.
