Yeah, when I heard those doses I was like, "These people are not messing around!!" Gives a whole new meaning to the word "overkill".
@J_McPhearsom9 ай бұрын
Those are for initial IV dosing though, not oral, which would be 10mg for Diazepam and closer to 30mg for Morphine. (Still an incredibly huge dose in compared to those) For reference on the higher end of therapeutic index - I was high functioning on an oral dose of nearly 200mg of morphine equivalent a day for multiple years. This was following an intense brain surgery, a lot of suffering, and finally being accepted into palliative care a year after (all due to an incurable brain tumor on brainstem and spinal cord in my neck). This was under the care of a pain clinic & spinal cord recovery team, but still, when combined with 0.5mg of Clonazepam, that alone could OD someone who had no tolerance. Regarding Diazepam, the _oral_ therapeutic dose for that is actually in 10mg increments. It’s Clonazepam and other benzos that are between 0.25mg and 2mg. I was on 40mg of Diazepam, then switched to 4mg of Clonazepam for a year before they accepted that it was more a pain control issue. (So based on my experience, these doses, taken orally, are a factor of 100-1000 times higher than even the highest therapeutic doses. I’m guessing partially because some terminal pain patients are taking pretty high doses, which still don’t help much, and part of the reason they’re asking for “game over”.) Doctors don’t like to prescribe that much unless you’re much older than me or death much closer, so though it helped make me go from debilitating nonstop pain to going back to school, working again as a researcher, and getting a Master’s in Engineering, (high functioning again), I was (am still being) slowly weened to a much lower dose. Plus, my brain & spine tumor is regrowing back, though slowly, and now, in anticipation of another surgery, I need a low tolerance to the pain medications for it to be able to blunt the pain of having my skull and spine cracked open again, and tumor cut off of my brainstem, spinal cord, and the vertebral arteries it’s wrapping around.

I know you don’t do it as much as in the early days. I know it’s a lot of work for you. I want you to know how much my wife and I enjoy it! All your hard work is very much appreciated on our end. We also would like you to know how much we’ve learned from some of your more serious works. It’s obvious how much research you must put in to preparing for your videos. Just wanted to say Thank You for your hard work. It’s very much enjoyed by us two obscure people in the States.

Wasn’t there a Little Britain sketch where the hapless vet euthanised the wrong dog? This is giving similar vibes. 😬 Crikey, I just tried googling that, and instead of finding the sketch, I got an alarming number of results where actual vets *_have_* sadly euthanised the wrong dog! 😱 It seems to occur far more frequently in the veterinary world than I would have possibly imagined, alas. Edit: Many thanks to unclenogbad1509 for pointing out that it was the League of Gentlemen, not Little Britain! The sketch is available on KZbin (I don’t think I’m allowed to post the link as YT tends to remove the comment) but the vet character’s name was Mr. Chinery.

as evidenced by the Great Pangolino Scare of 2020™ most docs are, to paraphrase Osho, retraded and vets are no exception. 🙄

My problem with this is that it’s almost always a slippery slope. Canada and Denmark have already seen it expanded from the old and terminally ill to people with disabilities and mental health issues.

Back in the day I was an assistant to an assistant professor, and some of my menial chores involved grading papers from a remedial college English class. And that's no joke--this was a special class for probationary freshman who had to demonstrate basic levels of academic competence to be fully accepted into the uni next semester. So one day I was grading essays, and I think the writing prompt was something like "argue for/against a current social issue." And as I started reading this essay, it made no damn sense at all. The writer was arguing in favor of Asian kids, but it wasn't clear what exactly he was in favor of. Then it hit me: he wasn't in favor of Asian kids, he was in favor of "youth in Asia." What was really crazy was that he seemed to understand what euthanasia was, but he'd written it phonetically like that. These were the days before an internet search engine had seen enough dumb asses to understand that a search for "youth in Asian" = euthanasia. Seriously, try it in Google right now and it will totally know you want info about medically assisted suicide. And that's just...unfortunate.
