Plz make more videos like this! It's so helpful to hear how doctors would "think through" a case. Interviewing the patient, summarizing the symptoms, & deciding which tests should be run, and what the results might mean.

I'm guessing he might have Acid Reflux or a Stomach/Duodenum Ulcer b/c he is worried and his senses are heightened. Why aspirin and not Tylenol? Aspirin can cause bleeding and if he has an Ulcer this could make his symptoms worse. I'm still learning very new.

Thank you for the comment. GERD (acid reflux) and stomach/duodenal ulcers should be in the differential as you mentioned. However, if you watch the whole video this patient was having a heart attack which is why they received aspirin and not Tylenol. An EKG was performed when the patient arrived which showed a STEMI. Prior to the EKG, yes it is reasonable to consider acid reflux as a possible diagnosis. However, what you just stated was the main learning point of this scenario - is to consider myocardial infarctions in patients who arrive with epigastric pain (especially if they have risk factors for an MI) because if you assume it's reflux/ulcers then you may miss an MI which would be bad. You are correct that you should not give aspirin if this patient was having ulcers/GI bleed etc. Hope that helps and thank you for being a subscriber to the channel! 🙏🏻

My roommate is a nurse & says they are seeing a lot of cases like this right now but in younger ages.

People come in complaining of "heartburn" or "food poisoning" or vague shoulder pain. The patients under 40 are more likely to assume its just indigestion or food sensitivities, whereas the older folks are more aware that the intermittent burning sensation might be heart-related. She said the patients do well when the problem is detected early. So, if you are in pain, do not try to "ride it out" at home--- go to the hospital & get your heart checked out! ❤
