Since no one pointed this out, the debuff was not known because it simply does not tell you what it is. It's just an icon. Unless you fully read and understood what the icon and item given were used for it was just there. The debuff is useful in early play due to low endurance and poise since you get broken thru blocks easily. Strength and endurance are to so low you can not properly tank hits.

the reason why fia the death bed companion decreases your health, is becuase she helps people cross over to the realm of the dead. meaning her powers is to quite litteraly to kill the last couple of % of your lifespan/health.

Are you trolling? Or do you not know the reason why, and came up with a lore reason yourself? If the latter, it actually makes sense, so kudos. But that’s still Kayfabe. After hugging her, you get an item called “Baldachin’s Blessing” (I have probably mis-spelled that.) which will temporarily boost your Poise stat after you use it. HOWEVER, having that item JUST sitting in your inventory gives you a debuff of “-5% Max Health”, which is removed when you finally decide to use the item.

However her being a DEATHBED companion, and having the ability to take away the last amount of life within a person, and doing so painlessly and giving them the grace and elegance(meaning of poise) to meet death, without fear. As for the trolling part, none of it was to make you intentionally angry. It was merely a rationalisation of what i had to work with, given my limited amount of knowledge, and to make sense of it without making a stretch too large.

I think Radahn used to be so difficult that they had to nerf him. He's still a little difficult, but I honestly could've loved to face him before the nerf. Literally a demigod that's holding the stars still with gravity magic despite having long since lost his mind to Malenia's Scarlet Rot which had made him little more than a rabid animal by the time you face him. Oh, and I think he's still using gravity magic to ride his horse even after he grew to such an immense size. I think that was the whole original reason for why he learned gravity magick; so he could continue riding his horse.

A common misconception, Rhadan wast actually "nefrfed", he was fixed. His Hitboxes were twice the size they were supposed to be on launch by mistake and they just reduced them to the orignally planned size. They did nerf his damage numbers a tiny bit, reverted that a couple patches later tho. So he is much easier than before, but just because his hitboxes were literally wrong at the start. Now, Promised Consort Rhadan from the DLC was actually nerfed quite a bit from his release version. Animations slowed down, delays added to combos, damage numbers decreased, brigthness of VFX reduced for better visibility, AI Agression reduced, also some hitbox size reductions and placement adjustments etc. And you are right about the horse, thats Leonard, his childhood hores he didnt want to abandon when he grew to big for him, so he literally went to the best gravity mages in the land to learn from the just so Leonard and him didnt have to part ways. Rhadan was a big softie when he still had his wits about him^^

The context about this Radahn being in his first form is that, you're fighting him when he's half dead, has no feet, is insane, body partially destroyed by the rots and the damages accumulated from countless fight against people who tried to put him down, while also constantly has a part of his mind focusing on holding back the celestial body. His final form is just him fighting you without all these handicaps (Well, technically there are still handicaps.... or buffs depending on how you look at it).
