Sir, Thanks for sharing . There are different type method/tools of RCA. Pls share a video on them, their limitations, benifits & specific uses. Currently I am using Why-2 analysis & fish bone diagram ( 4M) which is not enough for all types of problems. I had heard about FMEA, Now i got it 😊

Thank you for the video suggestion. I will add it to the line-up of videos I have planned for the remainder of the year. You are correct in stating there are multiple methods of RCA…I’ve personally used 8D, 5P, 7-Step, PSP, Kepner Tregoe and all the various tools that go along with those methods. I’ll put a video together to quickly run through the list + the difference between solving problems at the bowler level versus the tactical level.

Maybe…I’d hate to be the one leading that FMEA…too many possible failure modes…one almost happened on the second trip (I believe it was the 2nd) where the bomb light activated meaning that it was armed. They quickly shut it down though…then there was the weather that forced the Nagasaki hit but the original target was Kokura (I believe)…none the less, I’d hate to be the one leading it due to the devastation more so than the failure modes. Appreciate the comment.
