Yes! Epithelioid cells have less phagocytic capability than macrophages. But they have microbial killing ability than macrophages; mainly by secreting various enzymes

Can TB in latent state be diagnosed in x-ray? And what is the difference here between activated and normal macrophages as you said initial macrophage only phagocytose the bacteria and unable to kill but activated macrophages by t cells can kill???

My kidney biospy reports of lesion (Dec 2018) found Granulomatous interstitial nephritis and I started dialysis. On April 2019 no lesion was found but a cyst found in chest lymphnode and biospy showed Positive for MTB. Can you explain what has happened with me? What is the cause for my damage kidneys? Is there any connection with Tuberculosis? Please help me out.

Granulomatous intestinal nephritis has various causes. Mycobacterium tuberculosis is one of them. There may be a connection with the kidney lesion and this pulmonary tuberculosis. Seems like even if the kidney lesion has been cured, the organism has stayed dormant and caused pulmonary disease when it got the chance. Do you still have this condition?

@@medtoday After 1 yr of MTB treatment( which includes Rcin, Pyzina, Combutol and moxiflox) my culture report of lungs water tapping showed negative for MTB... So dr adviced to stop TB medicines and start Becosules. Please explain what is happening with me. Is kidney transplant is the only option? Something went wrong with me and doctors in 2018s. Though I have family history for TB. My father had intestine TB in 2007.

Tuberculosis is not related to family history. However, a persons immune response against MTB may acquire from the parents. Becosule is a drug which contains vitamin B and C. As these vitamins are water soluble, once they are taken into the blood, they tend to attract more water into the blood plasma. As a result, your blood volume increases. This may increase your urine output. Do you still doing dialysis? How old are you?

@@medtoday Yes I am on Dialysis since 2nd oct 2018 . I am 18yrs old . Yes I am taking Becosules once after breakfast daily. By the way it is said that Dialysis can't be reversed.

@@preetam_laskar sorry you have this too my husband has been told it won’t go without surgery he’s been checked by ENT but we have looked into speech therapy and been told that possibly can help. His was def jab related he was told by his ENT because of the timescale. Is yours after the vac?

Granuloma is not caused by bacteria. It is caused by the hypersensitivity reaction evoked by our own immune system. I will take Tuberculosis as an example ; In tuberculosis, bacteria get into the lungs and they are engulfed by the alveolar macrophages. However, they cannot kill the bacteria because the macrophages are still not activated. Also, Mycobacterium tuberculosis has the ability to escape intracellular killing mechanism of the macrophage. So, bacteria will then proliferate unchecked within the macrophages. Several days after the initial infection, bacteria reach the regional lymph nodes. Then the T cells get activated and they secrete cytokines to activate the macrophages. Activated macrophages will then secrete proteases, reactive oxygen and nitrogen species to kill bacteria.
