It happened again - an outbreak of dermatitis this time around my right eye, something called perioccular dermatitis. Usually it's perioral, ie around my nose and mouth. Grrrr. I stupidly applied an overnight lactic acid cream and somehow it transferred to under my left lower eyelid and I awoke the next morning to a red, sore and throbbing area of skin.
Over the coming days red, raised welts appeared and luckily, with the help of my brilliant friend ,a dermatologist Dr Anjali Mahto, I was diagnosed and told how to look after it.
And so here I share her advise and my journey. It is now two weeks later and I am almost healed, almost.

The two prescription options for my dermatitis are

Then the advise was simple - no actives, only unscented unfragranced gentle skincare - easy when even water stung.

As a life long atopic dermatitis sufferer , I when I have eye flares specifically, I have to be careful with hay fever and other such allergies and dry eyes (conversely causing excess watering eyes) as you’d be surprised at eye watering at night brings salty tears, and slather on Boots cheapest cheap lip balm on it - very waxy, fragrance free etc- and works so well for me as it doesn’t budge as easily. I don’t find/was advised that Tacrolimus works during flares, but as maintenance afterwards. But there’s no side effects, no skin thinning etc. can temporarily feel sensitive and makes the skin photosensitive.

I had a very mild case of perioccular dermatitis on my eye lid. Almost treated it like when you have conjunctivitis, I changed my pillowcase, used a fresh flannel to pat my face dry as well as a fresh face cloth & anti bac all my make up brushes. I didn't put any makeup on my eyes for 4 days at least. What helped was using a small amount of La Roche Posay Cicaplast b5 baume. Don't know what brought it on, but still. Brilliant stuff & has since cleared up acne scaring that I've had for ages! Thanks for the great videos & recommendations

I have suffered with perioral dermatitis for a couple of years now and it was horrific! Burning, stinging etc. I cannot use anything with fragrance, a lot of alcohol, essential oils or chemical sunscreens. I love all the cerave products and something called emuaid that helps immensely also doxycycline meds. Thank you for this!!

I had awful PD in November, stopped all actives and got Rozex cream. Only using filtered water on my face really helped and SLS free toothpaste. When do I make the move to start actives again? I’m still using the Rozex even tho all the red is gone but re introducing SPF has broken me out with small bumps and white heads. Help!!

No expert but experienced the same.A mineral SPF is better tolerated on sensitive skin.Physical not chemical.I use the kiehl's it's got a tint which I like but its not for every skin tone.Yes the filtered water is a must too.

I use pure aloe vera gel around my eyes and it really soothes and smooths them. It also helps reduce eye bags. I also use the aloe vera gel on dermatitis on other parts of my body which l get from time to time. It’s brilliant for calming, soothing and healing.

I have an allergic periorbital and perioral dermatitis, which started this year. Cerave and cica baume 5 both burnt the eyelids off me. So what helps me is dermal cream to wash and moisturise and this is the controversial one, vagisil ! It is made for a mucus membrane and it has 2%lidocaine in it which numbs the pain and itch. This was the game changer for me as it stopped me rubbing my eyes. It reduced my symptoms from 3 weeks to 5 days! Not suggesting anyone else does this but I was desperate and it worked for me.

Wow that’s a brilliant suggestion! I suffer to with allergy around my eyes and will definitely give this a go. I use nipple cream as a lipbalm...it’s intended for cracked, bleeding delicate area! Works a charm!

I tried both the cerave and the cica baume too and they stang my eyelids so much! I’m currently using Eucerin Ato Control acute care cream. Not sure if I really should be using it, but it’s the only thing that doesn’t sting the hell out of the skin around my eyes!

I've just started with contact dermatitis caused by wearing disposable face masks. It's on my neck, the corners of my mouth and my eyelids. My mouth and eyelids bleed and it's so painful. All I've been using is HC.1%. Its helping but it keeps coming back even though I wear a snood now instead of the masks. I can't use hardly any of my skincare and definately no actives. Just getting a GP. appointment is impossible at the moment.

I’ve developed 3 red spots close to my nose - clear liquid comes out, but I didn’t realise this was dermatitis! I’ve just started with a vitamin c serum... so will stop this and I actually have elidel for my FFA so will try that on skin! Thank you! X

I can get contact dermatitis from retinoids on my neck. The symptoms are a red, itchy neck that is very slow to heal. I found The Ordinary Niacinamide with Zinc soothes it and The Inkey List squalene helps as, well. Otherwise moisturizer with no actives is best.

Thank you Nadine for this timely video on Dermatitis. Currently battling it with HydroVal...you're so right; it goes away for awhile then re-appears! Taking all your advice and hoping to achieve your success with it!

Yes, wash your makeup brushes but also consider replacing your eyeliner and mascara as they could also be harbouring bacteria and viruses.

Thanks for this video... I do not put any acids or strong serums anywhere near the skin around my eyes. I use Laneige Cream skin and Aquaphor healing ointment. (Thankfully no milia forms.) Insane how my skin drinks it up! Will keep this video tagged. Cheers!

I’ve had a really bad bout of perioral and my GP started me on Hydrocortisone which as Nadine said doesn’t work. It got so bad they gave me a 7 week course of oral antibiotics but no topical cream. It was so dry, sore and itchy. I however was able to get Rozex video from Pharmacy2u for £20 and it’s been a massive help.

Poor you, painful photos. You drew an 8 on your face, I think it should be renamed the 8 zone. We're self administering cosmetics with strong potions, acids, A's, C's, it's a chemical cauldron. My skin is that sensitive and you just set me straight. Skin on outside of my eye will have an eruption if my facecloth is dirty. I want a retinol, where is the best place to test for sensitivity?

What worked for me, I’ve had it for over 30 years) cerave blemish control face wash, rozex cream, Paula’s choice moisturiser, skin purifier spray and slugging with cerave healing ointment.

I found azelaic acid works very well for perioral dermatitis. For my very sensitive eyes I just use a very mild micellar water and Toleriane Ultra (in fact I use these on my whole face since I have extremely sensitive, intolerant skin + rosacea). I also found watery, rich in humectants products tend to really sting and irritate my face...

That is good to know re azelaic acid. I have redness around my mouth and a bit on my cheeks with some under skin bumps. My GP suggested azelaic acid 20% but haven’t used it yet because I wasn’t sure. I also second the feeling with watery products (I just done a sheet mask and now that is dry it bothers me a little). I use Cicaplast as moisturiser.

God so frustrated, my dr is convinced my rash, just like yours, is some kind of acne although my skin is super dry , because I said my nose ran the whole time I was wearing a mask so she wouldn’t give me the rosex cream she’s given me something else that can take weeks to work and said if it doesn’t work, to get hydrocortisone cream. I gave in in the end because she was so convinced. Feeling annoyed with myself

Ok, I’ve rung the drs and had a little rant! Another dr is ringing me later. But for anyone else? I’ve bought it for £25 on pharmacyonline. Have to fill in a form, but I decided to go ahead and get it myself, perhaps it was the price she wasn’t comfortable with?

Hi Nadine I get similar issues with rosacea (which occasionally flares round my eyes), rather than just my normal face rosacea. During a flare round my eyes i use epaderm or plain aqueous cream to cleanse, depending on what my skin will tolerate. I don't even use my normal fragrance free gentle cleansers - just the two things mentioned. The simpler the better is often best when dealing with so severely sensitised skin. Both can be reapplied to moisturise, after a very gentle rinse when used to cleanse. Epaderm (or any other mix of soft paraffin, liquid paraffin and emulsifying wax) is THE most gentle thing I have found to use on extremely irritated/damaged skin. it can be used to cleanse and then seal in the water and provide the barrier your skin is lacking (similar to vaseline etc). As it emulsifies it can be used to cleanse in a way that vaseline type ointments can't. I use it as a balm cleanser to remove makeup anyway, out with a bad flare (gentle, affordable and effective at removing even mascara). My skin is always sensitive, but during a flare it is compromised in a similar way to any other issue like dermatitis or eczema (I also have eczema). I would always advise consulting a medical professional, but if you have to do something meantime (until you see one) this is my best advice. I'm glad the cleansers mentioned worked for you, but they may still be too active for some people. The dermallergo shown DOES NOT work during a flare for me, but my skin does tolerate it in it's normal rosacea state. I hope this helps others who are struggling. Best wishes.

Regarding fragranced products; I always make it a habit to try and not buy fragranced. I bought an Elemis cleanser (in fact 2 by mistake!) and the fragrance is just overwhelming. I haven’t had any reaction to it but I use it so quickly it probably barely has time to work let alone do any damage. I’ve used it only as a second cleanser to my Clinique balm; but some less sensitive friend is about to be gifted some expensive Elemis cleanser. Lesson learned for buying online & not checking reviews or ingredients! The only things I’d heard about the cleanser was how faaaabulous it was ... anyway. Thx again for the reminder! I suffer from eczema and hayfever and have also had periorbital dermatitis, Elidel dealt with that. My hands and feet are the areas where the eczema is worse but I think I have sorted out a regime to deal with that. Skin tests have identified about 8 allergies which is a nightmare/minefield, i.e. fragrance, preservatives etc. etc. My main concern is shampoos, I am having difficulty in finding suitable products - are you able to offer any ideas? I have tried quite a few without any luck. My hair is fine but normal and I do not colour or bleach it.

I sure my daughter has recurrent peri-oral dermatitis, but the GP has diagnosed impetigo. She uses Fucidin, but has a persistent area just u set her bottom lip.

I forgot to mention. On a cruise ship, I ended up looking like I had 2 black eyes. The Russian doctor wanted to give me a steroid injection, me being a massive wuss asked for a topic steroid cream instead. He was impressed with the results. I don't recall having bounce backs from steroids though.

Cerave healing ointment, CERAMEDX Extra Gentle Body Cleanser, PROTOPIC Generic Name: tacrolimus Eye dermatitis from the eye drops that I have to instill twice a day. Since Cerave, my skin is almost perfect. I apply at bedtime. Protopic is good during periods of inflammation.

I am so disappointed with my POD Since 3y i am not able to find a solution I think in my case the food has a big effect on my POD I hope one day i wake up without pod cz i am really tired of it literally i cant use any sunscreens or creams or anyth

Been there, done that 😳 My neck and chest !, I cleansed with DHC oil cleanser (my Cerave hydrating cleanser stung) and used La Roche-Posay Cicaplast Baume B5 - it took about two weeks but now it is almost gone.

Dr. Dray saved my eyes,3 weeks ago I used Alpha skin care serum 14%, was so dumb to put it even around my eyes. I got terrible irritated thicker skin, become wrinkled. Nothing helped. Watched Dr. Dray . Don't use any of eye creams for sometime just apply Vaseline- simple original.and let your skin to heal. Still have a lot of wrinkles under my eyes but its getting better, my eye lids cleared completely.

Hello Nadine I had peri-oral dermatitis bad a few years back and after doing everything wrong because I just didn’t know what to do right a lovely lady in boots the chemist advised I use an un fragranced cleanser and Avene skin recovery cream moisturiser, it took two months to get my skin back to normal. I sometimes get a minor flare up and always take my skin care back to basics with these two products.

I have something very similar around my eyes, about 3 or 4 times now..prescribed antihistamines and Diprobase. Have a flair up right now. Does the eczema cream repair the barrier function/make the recovery go slower than the two items you mentioned? Thanks Nadine, videos are so helpful!

Make up seems to be my issue, I have had it since the summer but I can’t find which product it is. So frustrating. Previously the Doctor has prescribed Dermol 500 for cleansing and moisturising.

I find any make up with ''glow' or 'brightening' claims has microscopic glitter and that irritates. Might be your issue

i am in Germany, had very bad flares for years (cortison didn’t help much), then i ordered dermol 500 from UK (cream, and gel) and used it to wash hands and even as soap substitute for shower.) my skin was so flaky for first 2 weeks into using dermol, then it starts to heals after all those old skins gone. Now i can’t ordered dermol anymore (maybe it is brexit thing 😞) and i haven’t found anything as substitute and my skin start to get worse again 😫. enough ranting now.. 😕

Thank you so much for this I had this on my eyelids and under my eyes. My eyelids looked as if they’d been slashed. I was prescribed 0.5% to 1% Hydrocortisone it didn’t help !! Finally after 3 weeks it went I found I could only use diprobase as both a moisturiser and cleanser even water stung. I’ve returned to using Your Good Skin cleanser definitely a thumbs up on that one !! From now on everything will be non perfumed and nothing will go near my eyes !

I completely agree with everything you said. Plus I also suffer from allergies, so that might be one of the problems. My go to products are rosex (or rozamet in my country), toleriane caring wash or cerave hydrating for cleansing, toleriane sensitive cream or cicaplast if it's really bad. Around the eyes bioderma sensibio or bioderma intensive eye which is a bit thicker cream, plus it's huge, so that's a plus. The actives I use I never put around the nose and lips like you said.

Glad to hear your dermatitis is under control. I took your advice some time ago & used La Roche Posay, it definately assisted with calming tge rocacia on my cheeks. Following that I decided to pick up the Avene Antirougeurs cream cleanser, just beautiful product. Strangely enough when I suffer hayfever here in dry hot Australian climate, the only thing that works + calms my eyes is Vaseline! I appreciate your clinical perspective. Stay well👋

This is so interesting! I was using retinol and then an rose hip oil for number of months every evening and then all of a sudden one day under eyes got red itchy and sore, couldn’t use a thing for a month or two after. I wonder if that’s what has happened, I’ve got very sensitive skin as it is

My dr has just told me to buy hydrocortisone cream for mine! I asked for the rosex but she was so snotty I gave in, so mad with myself 😖

@@clairehawkes1112 hydrocortizone is an incredibly weak steroid thats why you can buy it over the counter. The purpose is to reduce inflammation, if you have inflammed skin, a steroid mixed with the moisturising routine can work great! :)

Have you tried a tar shampoo? It's important to leave the form on your scalp for a couple of minutes before rinsing it off to allow the tar to work. The other shampoo that can be good is called Dermex. Both tar shampoos and Dermex can be pretty drying for your hair, so you might need a richer conditioner, or try using them on alternate washes rather than each time. Itch can be helped with a good moisturiser - pretty much anything that suits your face will be okay for your ears. Strangely enough, Epaderm ointment or Hydromol ointment work really well as scalp moisturiser. Apply to your scalp overnight and shampoo off in the morning. For some reason they wash out really easily. Try not to scratch or pick at your scalp/skin- it will only make things worse. A lot of people find they scratch in their sleep- the only thing that helps that is damage reduction- either cut your nails right down and keep them super short or (and this is a strange one) false nails cause much less damage. (FYI I'm a dermatology nurse in the NHS - these are things that have worked for various patients)

I am sorry you suffer with this dermatitis! I used to have peri oral dermatitis quite badly, and strangely enough, when I went plant based, it disappeared within days abs has (knock in wood) not returned for years. I truly believe inflammation caused by diet (certain foods we are sensitive or allergic to) plays a big part in any dermatitis.

My son had this when he was young, just from sensitive skin. It became infected and he was rushed into hospital because they thought he might get meningitis 😔 scary times. Take care 💕

@@nickkellerman1638 Nick, first off no kale smoothies for me, yuck. Secondly I am just sharing my experience, hoping someone can benefit from it. I was not praising trees, but talking about leaving away allergens, or foods we are sensitive to, to which the body responds with Inflammation. A dermatitis is an inflammatory disease, therefore it makes complete sense, to lower inflammation in the body

Thank you so much for this video and the previous one. I have recently been afflicted with this after overdoing it with retinol and niacinamide and was feeling really low as the rash has been so stubborn, but you've made me feel so much better about it. For hands, wash them with an emollient wash, not a soap. Use a moisturiser without fail each time you wash your hands. Overnight, use a rich moisturiser- one with 10% urea can really help with cracks. Don't pick any loose bits of skin- if there are bits that drive you crazy, snip them off. If you pick, it is easy to pull a bit, and it can make your hands really sore. With your scalp, you need to moisturise it as well. Consider avoiding sulphites, as they can irritate some peoples scalps. A lot of shampoo and conditioners for curly hair are sulphate free and moisturising- may be worth trying those. Good luck.

A good emollient, and good hand cream, so many on the market, always use rubber (Lakeland sensitive) gloves and white cotton gloves in bed and for all house work, keep those hands moist, I’ve suffered years with dermatitis, contact dermatitis, and lichen plants auto immune skin disease, it’s miserable, but if yours is bad get referred to a dermatologist, I’m under the care of them, they’re the experts...good luck x

I would definitely recommend banning SLS (Suphate) products from your life. They are not only found in soaps and shampoo but also toothpaste and creams and many others! When my skin is bad, I try to avoid water as well as cleansers. "Wash" your hands with gentle moisturiser or oil and gently wipe it off. Aloe Vera (especially straight from the plant) is good for the scalp. You can slather it on with no detriment to your hair. Also wear hand cream and cotton gloves when you are blow drying your hair!

@@AnnMargaretCP totally agree, I can’t wash my hair In The shower any more, has to be over the bath, I do use sulphate free shampoo and no conditioner, I use the double base gel for showering and hand washing, think I said diprobase in my other post, moisturising is the best way forward for me! Yes,I get this around my mouth sometimes and I have always gone to the 0.5 Hydrocortisone, which to be fair combined with Child's Farm moisturiser, which is awesome by the way, has always cleared it up. I also suffer with Hay-fever, it's a bitch. Good video, nice to see more problematic skin being covered.

I would also remind anyone using Elidel that it can make you more sensitive to UV light, so using a high factor/high star rated sun protection is essential. Tacrolimus is more of a problem with UV sensitivity, but both can cause a greater sensitivity to UV light. Nadine- you said you have hay fever too- greater skin sensitivity tends to go hand in hand with asthma/eczema/hayfever (or the unholy trinity). I wonder if you are more prone to getting dermatitis because you may be atopic?

Good reminder, and interesting point on the allergies. One issue is that sunscreen will aggravate the issue too - at least that is my experience with incredibly persistent perioral dermatitis. I tend to try to stay sheltered/wear a hat/etc. If anyone has experience with sunscreens that don't sting or otherwise make the issue worse, I am really interested to hear which!

My eyes are exceptionally sensitive, to the point that removing eye makeup is a nightmare. Can I ask what you use for that. I have tried most of the high and low end removers with no luck.

Avocado oil. I used to use West End style theatre makeup when dancing and we used it for hardcore makeup and no ouchie

Hi Nadine. Thank you for your brilliant videos and everything that you do. I'm a big fan of yours. I too suffer with Perioral Dermatitis and I'm allergic to nickel, including iron oxides found in 99% of make up 😪 I only use 100% PURE at the moment. I watched your other video and started Elidel yesterday. I'm waiting for Rozex on prescription and I already have 20% Azelaic Acid. Please would you be so kind as to advise me what to use when, in which order. My Dermatologist said very little. Can I use a repair moisturiser at the same time as these prescription products for the dryness and soreness? Thank you 😊 Xx

I saw your older Video that led me here. I’m 2 years in and need help! I can’t take this anymore. Derms have made or worse, my research has gotten me so far but it won’t go away and i never had any skin issues in my life. This ruined barrier has made so many issues and this periroal i have i think and I’m seeing little bumps around my eyes now.

Dairy makes it angry. Same with hard water, sls, dirty brushes, essential oils. It has to be left alone. Aveeno products saved me.

Periorbital dermatitis is great fun. Mine is generally better since I stopped using micellar water (a Nadine Baggott suggestion). I use protopic which is an immosuppressant (tacrolimus). I happen to have it now and am struggling to sleep atm. Protopic makes my eyelids warm and then even more difficult for me to sleep which is a huge pain. I also highly recommend Childs Farm baby moisturiser, which my friend with eczema recommended. However, between layers of that and vaseline, I have managed to give myself a spot on my eyelid which is brilliant.

I watched this and your previous vid on PD and you explain it so well!! I'm 7 months into a substantial chronic perioral derm outbreak, and after seeing x2 top-end dermatologists and x3 courses of antibiotics, 3x courses of itraconazole and 12x different creams incl Elidel and rozex, I'm now being told to wait 6 weeks for a complete skin turnover on zero therapy. It's terrible, I have skin coloured bumps like sand all around my 'muzzle' with continuous outbreaks of spots. I look horrendous - but nothing is working! It's just one of those things. my trigger was a nasal spray with steroid in it (Beconase) which after I stopped taking it, sent my skin into overdrive.... ah skin...

Thanks Nadine you have given me food for thought. I thought I had eczema on my eye lids and GP gave me steroids . You are so right about subsequent flare ups being worse. Now I think it is peri orbital dermatitis as I also have rosacea. I will try the Rozex I already have and see how that works.

I think the over availability of actives in skincare is causing skin sensitivity in a lot of people. I’ve got really reactive skin so I tend to stick to soothing non-fragranced products and a basic skincare routine. Something Dr David Lim said something in one of his skincare for sensitive skin videos which has stuck with me: “grease for peace” which I now follow for my face and body care. I even switched to unfragranced shower products like Sanex zero and good old aqueous cream, and moisturise with cetaphil all over to prevent flare-ups. Haircare is a bit more tricky but fragrance is less of an issue with wash-off products (and I need something which treats seborrheic dermatitis).

For more than a year now I have redness around my mouth, and slightly on my cheeks too with tiny under skin bumps (no fluids coming out). My skin is dry, and I guess sensitised too. I don’t know what to do with it and what to use to improve the situation!! What are you thoughts on azelaic acid? Also, if you have good dermatologist that you can recommend in London, please do!!

Really interested to know if you still used a microfibre cloth with the cleansers? No matter how gentle I am with them, they rub and irritate my skin which means everything eventually stings! Did you manage to still use them over this period?

Thank you so much Nadine. I went for a facial for the first time in almost 2 years at an Avene Salon here in Australia, and I now have peri oral dermatitis around my mouth. Same spot every time, but now it’s spread to the corners of my nose and the Finacea is not cutting the mustard. So off to GP to ask for the correct medication to try and fix my poor face 😢

Thank you, Nadine. I have suffered for years with Perioral Dermatitis. My skin is in a really good place right now. I think the fact that I haven't been wearing much makeup during the pandemic and being extra careful around the affected areas. You are correct in that you have to keep anything irritating away from these areas. It really is frustrating and even when you are being so careful a product can migrate and cause irritation. I think I have been suffering with something similar for the last 12 months. I’ve been to see a dermatologist who prescribed me with two different steroid ointments which I have used sparingly over the last 3 months, only as needed. I’ve even had allergy patch testing but nothing that came back as allergic I can find in any products that I put on my face. I’m also an annual hay fever sufferer, so I am not looking forward to spring as this will most likely make my eyes worse. At the moment I am in the middle of a particularly bad flare up (right eye is so incredibly sore, left not so much) and not sure where to turn. Maybe immuno-suppressant cream is the next thing to try! Thank you so so much for sharing. It actually made me quite emotional seeing that maybe there is something else I can try to help sort it

I wonder though if allergy to anything can show up on your eyes still? So I started using a heavily perfumed shower gel recently & after the only 2 times I used it, I came up in an itchy rash on my arms and my cheeks and jawline, even though the shower gel was nowhere near my face.

I’ve just rung my dr about this. I’ve had it around my nose, down my smile lines and on my chine for ages. I thought it was because my nose runs terribly when I’m wearing a mask but whatever the reason, I know what to ask for now, I know he would give me steroids otherwise

Skin is even more delicate, and with steroid it the the other active creams she mentioned I’d, more photosensitive. So I’d say definitely needs sunscreen, and likely a mineral one in case of sensitivity
