PERIORAL DERMATITIS - HOW I TREAT MY SKIN FLARE UPS

I have suffered from outbreaks of perioral dermatitis (dermatitis around my nose, mouth and chin) for over twenty years and yes it was a hazard of my job, as I first developed it testing weird and not so wonderful facials when I was a beauty editor. Funnily enough skin care addicts often suffer as they overuse strong active ingredients and, as a result, they damage their skin barrier function and set up this annoying, red unsightly itchy, pimply, mildy infected skin.
It took me two decades to get properly diagnosed and then just five days to clear it up. THANK YOU SCIENCE AND DERMATOLOGY and DOCTORS.
Here is everything I know and all of my top tips on how to cope if you too think you have POD.

I literally just left the dermatologist and got diagnosed today 😭 he prescribed me “Aczone - (Dapsone) Gel, 7.5%” I’m hoping this helps me. This video she said to use a cream but I’m hoping this gel works. If not I’ll ask for that cream she mentions in this video. This has explained it so well, I now know this is what I have. For years my doctor called it rosacea and gave me hydrocortisone but I now have a new doctor who gave me Rosez and this has been the best thing in the cream form. I didn’t know it was called Perioral Dermatitis but now I know how to treat it I feel much more relaxed about it when it flares up instead of going into horror mode as it used to flare up all around my mouth, angry red and dry. Thank you for explaining this so well. A provider wasn't sure and diagnosed facial shingles- it didn't go away with 2 courses of meds. Then a doctor wasn't sure but gave me two different meds- one was clobetasol, a steroid cream- which made it worse! A wonderful PAC who works at a dermatologist's office part-time ( I saw her at a different clinic) nailed the diagnosis and gave me the metronidazole cream, and Doxycycline just to clear up the massive flare-up that had spread. I've noticed a difference overnight. This has been ongoing for 3 months. I'll be glad to see my normal skin again.

This video helped so much, I have been battling POD since January of this year. The first Derm. Put me on steroid cream and just as you mentioned it helped and then became much worse. I eventually had it not only around the mouth and chin but around my eyes two round spots on my forehead and on my left cheek. Be careful of strong SPF sunscreens and recently discovered that the Zinc in my facial sunscreen makes it worse.

Found this so very interesting. I’ve not suffered from this but sounds very frustrating and painful. I’ve had always had very reactive sensitive skin, every facial I ever had burned, the worst by far was an organic one! The Ordinary has saved me, I think it’s the simple formulas with no fragrance, I think a lot about Brandon’s foresight. I’ve now gradually built up to stronger actives with no issue except for alpha lipoic it itched like crazy. I loved the results but that itch!.

Thank you Nadine, crying right now. Bad case of PO. I know now what to ask for. A big thank you for this video. Got to see a GP today and got prescribed a topical antibiotic cream. Already looking better after 2 applications. I think a got it from using around 7 new products recommended to me through a regimen recommended by The ordinary. Maybe it was way too much active products introduced at the same time... add to that the fact that I,m going through menopause... the perfect recipe for skin disaster. They tried to give me cortisone cream and after seeing your video I said no way ! I had perioral dermatitis for the first time last year. This was following on from a case of folliculitis which was over my forehead and cheeks, so clearly i had a compromised barrier. I didn't go to my GP about it because i had been through such a trial with her trying to sort out the folliculitis (reacting to antibiotics, myriads of things not working etc.). I mistakenly went for a facial and the facialist said a quick round of microdermabrasion would clear it up *eye bulge*. Obviously, it made it so much worse. I ended up stripping back to basics, gentle cleanse once a day and washed off with cool water (warm water seemed to aggravate). I also soaked oats in water and would use the water as a toner for my skin- really helped reduce the inflammation. I did not use any acids or retinols for about 6 months and i can say my skin has finally returned to its normal state. Now, I am very cautious of using too many products and have a VERY basic routine. Goodluck to anyone who is still battling the PD beast!

Thanks so much for this video. My dermatitis was caused originally by a shampoo/conditioner that contained a coconut derivative that is apparently irritating to a lot of people. Took months to work it out and repair the damage. I'm currently having a flare up which I think has been caused by a combination of my Differin prescription and an overly fragranced Vichy spf. I'm protecting and repairing and hoping it will settle down on it's own, but if it doesn't I will get this prescribed. Such a font of knowledge Nadine, you are invaluable!

I have actually got rid of it completely with the help of azelaic acid (the TO one works, and is also good for sensitive skin, as it is not so concentrated; also, you don't need a prescription for it). And since I have a case of extremely sensitive, intolerant skin, I always use the most gentle of products, no alcohol, perfume, essentials oils etc. Cortisone products are the worst, they can actually trigger PD. For hydration and skin repairing, as someone has already suggested, the Toleriane Ultra and Cicaplast ranges from La Roche Posay are great.

Thank you so much for posting this!! I had this once , very slightly, when I was 22, and it came back again recently in a more irritating and noticeable way at age 30. The only thing I've found to be helping so far is rinsing my face ( no soap ), and applying Black Seed Oil ( Nigella Sativa ). It is anti-inflammatory, anti bacterial, anti viral, and anti-fungal. Its not completely cleared up, but seems to have made a marked improvement. 🤔

It was getting so bad that she wore a face mask at school to hide it and even started drawing it on her self portraits. The last dermatologist we saw gave her a 5 month prescription for antibiotics. Thank you Nadine!! I suffered with this condition for only 2 months but had one of the worst cases I saw from researching online. It was truly the worst skincare experience ever. I’d like to add that once you’ve experienced this, please please please always keep your skincare routine to gentle and minimum products. I use the minimum since I’ve been cured because I notice when I start adding too much, I feel the tingling that you mentioned earlier around the nose. Basically even though you’re recovered, I’d say your skin is never the same. But you should be successful keeping it at bay being a skincare minimalist and using sensitive skin Ingredients (i.e. fragrance free and denatured alcohol free)

I live in an area where only 1 dermatologist services the entire province so that wasn't an option. A great little skincare line that has really helped me keep both rosacea and perioral dermatitis at bay is a skincare line called “Kalme” that can as far as I know only be found online. They have several products, but among them a cream called “Undercoat” that just soothes, protects and stops active ingredients from other sources to irritate or disturb the skin barrier. It is just phenomenal!! I use it specifically on areas around my nose and chin to prevent actives I use on my skin to get to those areas and whenever my skin is even slightly irritated or I can anticipate irritation I use it all over. It’s a lovely cream!! Understated and strangely underhyped, but absolutely phenomenal in its simplicity! (Their night cream is fab too btw, and their sunscreen if you feel everything else itches on you..)

Thanks for posting this video, Nadine, and for helping so many people :) I agree that steroid creams shouldn't be used long term. They are only intended for short term use as they can thin the skin (I know from personal experience) and can cause breakouts. I was bad and I mean bad with PD and was slathering on make up to cover it up. I must have tried over twenty different creams and lotions and just kept on irritating in. The only thing that cleared up my PD was to give my skin a complete break from using anything for one month and then using proskin eczema after reading some great reviews about it. I also switched to a cleansing milk by NYR.

hi, i’m a 13 yr old girl who purchased a The Ordinary product last year during the summer, after i used it I began to break out underneath one of my nostrils, and then not knowing, thinking it was acne, kept using the product and even more so, foundation and concealer on top of that. from there it got worse. i’ve never known what it was until about three months ago, i only have it around my nose and it’s been quite the struggle, ofc i’ve never been to the doctor or dermatologist to see what it is and seek treatment, my parents never took me to one, definitely will try to see one, probably not sometime soon because of the coronavirus, however, IM SO GLAD I FOUND THIS VIDEO!!! bc now i know what i need to look for when being prescribed a treatment!! tysm!! 💞💞

I have been suffering from the exact skin condition that you describe as POD but I was diagnosed with rosacea (even though I have no redness on my cheeks, only in the areas you describe) and was prescribed Metro Gel which helps a little. Thank You for the video. I suffered this two years ago -it was allergic reaction to lip balm Reve de Miel by Nuxe. After I stopped using it this became to calm down. I made a second try to confirm my suspicion and that was it. My dermatosis was only around the lips. And I think it was a reaction to fragrance in the lip balm -linalool. Now I use only unscented products, I don't use any scented lip balms only a vit. A balm and it is ok by now. But after dermatosis I have some capillars broken (over my upper lip) and it tends to be more visible when it is cold-they get reddish. Dermatosis looks nasty and it is so hard to cover....

I'm in Australia and am certain that I have this and I've been suffering from it for weeks. I've had every single symptom. I've been crying and depressed because everything I do makes it worse, the burning and itching drives me up the wall....not to mention the tightness and flaky crusty skin! I wonder if it's caused by oral steroids for my arthritis, though I've been talking those for years. I suffer terrible rosacea as well for 30 years and now I have broken capillaries from that. UGLY!!!! I want to thank you from the bottom of my heart for this video and am heading to my gp tomorrow for a referral to a dermatologist. God bless you my dear and THANKYOU THANKYOU THANKYOU❤

You’ve saved me!!!! I was dreading the announcement that we no longer had to wear masks when going out, as I have had this rash for YEARS, and was able to hide behind the mask, haa! Just as you say, hydrocortisone makes it worse. The GEL base Rosex didn’t work, but the CREAM base has cured me! Thank you so much!!!

Thank you so much for this, Nadine. I’ve just been to my GP and specified the cream over the gel and I’m feeling very hopeful. I have eyelid eczema in the corners, however now I’m wondering if it could potentially be the same condition as it’s itchy and similar to the mouth area. Anyway, I’m so grateful to you for making this video. I can already feel my mouth area calming down.

I am so so glad I found this. THANK YOU. I have had this for some time and I have a brilliant GP - she recognized what this was straight away, but she prescribed Metro Gel...and now I need to go back and ask her for the Rosez. I can't thank you enough!

Thank you so much for your video. I had an allergic reaction to a face wash (which I’ve never had before), which led my doctor to prescribe hydrocortisone and he didn’t tell me it could leave me with PD. I’m no longer going to that doctor and, though I’m still battling with PD, I have a better idea what to talk to whoever I see next. Thank you so much!

I found that my skin vastly improved by using a toothpaste that does not have SLS in it. A tip I think I got from Caroline Hirons. The SLS was drying my skin out so much. When mine flares up I resort to washing my face with emollient cream called dermol that is antimicrobial. But I think I need to try this wonder cream! Thanks Nadine

So informative, loved this video. Since you mentioned broken capillaries, what do you do for them? I seem to be getting more as I age, I take very good care of my skin, sunscreen, resin a, etc.. I did have a few layered but is there anything else that might be used on a daily basis??? Would love a video on this issue. Thanks as always!!!

I was given oral Metronidazole, months supply, because my dermatitis got to the point it was no longer just a flare up, there was no in between. Now I only have flare ups- which I will take any day over having it daily for over a year. I have been trying SO HARD to find something to control the flare ups. I was recommended this video through Reddit- I am so happy I was!

Wow you explained this perfectly. Thank you so much for the information. I suffer from POD for a few years now. When I get it I have to stop all make up and skincare. The only cream that helps is the lipikar balm ap or the bioderma intensive baume. I just got another episode and getting over it. Takes patience. :)

Ive had this nightmare,Ive nicknamed it Ironbridge Chin,as I had to have a day out at Ironbridge Gorge Museum with my chin looking terrible. I eventually bought a copy of the museum newspaper and walked about with it held in front of my chin. Best 40p I ever spent!

Thank you, I have always had some kind of dermatitis, so I'm very used to using corticoids. I'm a bit puzzled about your comment about the Ordinary. I haven't overused them. But maybe the dates match me starting to use them, must check as I started various dieting at the time too. Anyway, I'm sure there is a connection with the corticoids, whether diet or cosmetical products, as nothing seems to work as I've been using these creams since young and they worked then..

I've been going to a dermatologist for acne & had a PD flare up during the course of treatment and she kind of glossed over it. I have old, long-expired metronidazole cream in the cabinet that I haven't thought to use yet (might try it since the expiration date isn't a huge deal). what I've been using instead is nizoral shampoo as face wash at night since it has ketoconazole & azelaic acid 10% from the ordinary. it pretty much stopped the spread of the rash in its tracks. so if you can't wait to get a prescription for metronidazole cream or don't want to take antibiotics, I would try an antifungal from the drugstore first

So glad I stumbled across your channel... I also suffered from perioral dermatitis and at first my doctor prescribed me steroids cream but it kept getting worse until I went and see a dermatologist that's when I know I had POD.. Long story short, I've try antibiotics doesn't work then I try natural remedies also doesn't work and been going on for nearly two years... Gonna give Rosex a try and pray it will also work on me and thanks for making this video ..☺ ♥ Australia

Thank you again for a wonderful video. Any chance of a video for barrier repair in general? I have eczema on my eyelids that I use prescribed medication for. But it would be good to know how to strengthen skin during the calm periods so that it would be better prepared for the flareups.

My magic bullet was Zineryt. I suffered from PD for most of 2015. Zineryt finished it. I used it for a few months to be sure it's gone. It never came back since. I'm glad you made a video about it as I've been seeing a lot of customers with PD in spacenk... To help with dryness I was putting a nice Caudalie moisturiser.

I have been prescribed with topical metronidazole gel for perioral dermatitis hyperpigmentation ( i have dark patched at each corner of my mouth + it is not itchy scaly or acne) now i totally understand why it goes away after doing a peel or using hydroquinone but returns after short period of time. I have been dealing with this for 10+ years and it is very embarassing. I cannot thank you enuf.. I'll definitely try this

Yes! 👏🏼 thank you!!! And sometimes it can appear in the outer corner of the eyes. And, once I had a very intense crisis that spread all over the body, in many spots, almost like varicella, but it was the same exact dermatitis.
