For anyone else that might be wondering, in order for this to work, you need your axes to be X/Z (red along the horizontal and blue in the vertical). Otherwise you will just get bones piled on top of each other.

Ideally, your model should always be facing -Y in Blender, as X axis considered left/right (and Z up/down). Also remember to apply transform to armature and check its origin point.

Maybe you got a newer version of blender so the indexing changed. I name my bones "NameOfLimb L". Make sure to have a space inbetween the name of your limb and L. Also when you shoose left and right, make sure you name the bones as if you were standing behind the character.

Looked up this video because only some of my bones were symmetrizing. The bone naming gave me a clue that I followed up on with some experiments. Turns out, Blender will not symmetrize bones if they do not have an L or R at the end of the name of the bone. And because I added noIK to the end of the bone's name, Blender didn't detect the L I included in the name to label it as a bone for the left side. I moved noIK to the beginning of the name instead. A game engine I use, looks for noIK in the names of bones when you import a model, to tell it what bones to ignore when setting up the inverse kinematics system. Luckily, it's flexible about where in the name it can be.

Thanks a billion. for some reason i was using the mirror modifier and when I would move the left arm the right arm would go too. This was driving me crazy for ab a week as i couldnt figure it out.

It’s not working. It’s duplicating and renaming the bones, but it’s not moving them to the other side. Now I just have two of each bone on one side with no bones on the other side. Can someone please tell me how to fix this?

Even after renaming to the _L naming scheme, I got it working but it didn't copy over the weight paint. So moving the bones on the left moves the mesh on the left and right. Moving the right bones do nothing to the right mesh.

Rotate your armature facing properly (for a character, feet pointing in negative global Y, on the viewport navigation gizmo it's the green orb without a Y inside) Apply rotation (ctrl+a menu in object mode.) Symmetrize. Be mindfull of your pivot point and your origin ( for a character it should be at the bottom ) 7 month late but who knows might help someone in the futur.

Why the Blender devs insist on making thing complicated and down right moronic? I just don't get this program. If I extrude a bone and the symmetry is on I expect it to just extrude another one on the other side (which ever axis is selected) why on earth do I need to name them for this to work? why do i have to make all one side and than do this moronic naming and copy to other side? why can't blender do things in an industry standard way? and the thing I hate the most is that blender devs NEVER EVER listen to criticism (even when not calling Blender moronic) and actually providing solutions/ideas and constructive criticism they insist on doing things differently in way that just makes UNESSECERY EXTRA STEPS THAT ARE UNINTUATIVE AF. regardless of my rant thank you for this video!
