He was already dealing with addiction issues (alcohol and cocaine) when he was cast on the show, but the show made it much worse. He had a lot of easy access to both on that scene, lots of money, having to go to all these Hollywood events, and also being in the public eye, criticized, insecurity etc. I actually dealt with the same addictions, at around the same time as Laguna Beach and it's so hard to get out of it - you literally have to leave much of your lifestyle and friends (because they're party friends going down the same hole) behind. There would have been added layers of shame and anxiety if it had all been documented on TV for years. I can see why he struggled so hard.
