Immunohistochemistry in Normal Skin: p63, EMA, desmin, SMA, CD34, Factor XIIIa

Before you can use immunostains to help recognize different tumors and diseases in dermpath, you must first understand how they stain the normal structures in the skin. In this video we will discuss these various markers and how they are expressed in the normal skin structures:
p63 (and p40): 0:00
Epithelial membrane antigen (EMA): 3:30
Desmin: 8:22
Mast cells staining non-specifically with various IHC stains: 10:40
Smooth muscle actin (SMA): 12:00
CD34: 14:47
Factor XIIIa: 19:00

I thought both layers of the eccrine duct stained with CK5/6 in your last video, but it seems that inner layer is negative for p63. Maybe this is just a knowledge deficit on my end, but I thought CK5/6 and p63 were similar... or maybe I'm only thinking that because both can be used as myoepithelial markers. Would you mind explaining this?

Dermatofibrosarcoma Protuberans (DFSP)is a soft tissue malignancy testing positive for CD34 while a Kaposi Sarcoma necessitates further Immunohistochemstry staining as these are viral processes and characterstic of immunosupression...?

Thank you very much, I don't suppose you could be persuaded to make a video on adnexal tumours - I know they are quite extensive, but trying to consolidate and memorise them prior to the FRCPath part 2 exam is proving a challenge!

Thank you for the wonderful video, Dr. Gardner! Can EMA stain be used to look for perineural invasion in other organs?

I find s100 or ‪SOX-10‬ to work best for that. I suppose EMA might help in some situations but often times the tumor doing the perineural invasion is a carcinoma, and carcinomas are often EMA positive. So that might complicate interpretation a lot.

If my IHC biopsy report says p63 and TTF1 negative for Non small cell adenocarcinoma. What does it mean?

It depends on the situation. TTF1 is positive in most adenocarcinomas arising from the lung (occasionally there are exceptions), but is negative in adenocarcinomas from most other organs (thyroid tumors are also TTF1 positive). p63 is positive in squamous cell carcinomas and in most skin carcinomas, but it is negative in most adenocarcinomas arising from the internal organs. Please discuss all of this with your doctor, as these stains are not perfect and interpretation can be complicated. Clinical and radiographic information are usually needed in addition to the immunostain findings to help us sort out the exact origin of the cancer. @@JMGardnerMD Thank you for the reply. One more question. Bronchial mucosa was taken from my lungs. Initial MRI and CT showed that it metastasized to my brain from my lungs. So does the current report mean that my primary cancer is still unknown?
