Very helpful guidance, I’m now 14 days post TKR I was paranoid on week 1 about ensuring I slept on my back, keeping my leg completely straight noting all the guidance on not using a pillow 😊. Last night totally exhausted I took my pain meds plus a strong antihistamine, iced my knee, opened the window to hear the ocean, got a long pillow placed it between my operated leg and top leg, moved onto my side with difficulty, sent hubby to the guest room and slept for 4hrs in a row, my knee has not gone into a permanent bend or fallen off 😁, the swelling has greatly reduced, I feel alive - yip our bodies need sleep and peace to heal. 🥰

The lack of sleep was never mentioned to me, pre op, so it came as a major surprise to me, post op.

I’ve had two successful TKRs from two excellent surgeries and have no qualms with them . THAT SAID, I really think in the TKR “ industry” it’s a given that they intentionally don’t tell you everything you’re going to be facing because 1) everyone is different , indeed every knee even on the same person is different but I think the main reason is 2) that if they told everybody everything they will face after surgery it will increase the anxiety and second -guessing about whether to have the surgery or not . 3) in their defense , good surgeons will tell you a lot pre-surgery but there comes a point, the human attention span being what it is , where you can just tell someone too much .

I get a real laugh when Dr. Rosen says, repeatedly, "talk to your doctor". He must provide a service I haven't found which is the ability to get an appointment and spend more than 11 minutes with a doctor. Must be nice.

I found Dr. Rosen’s you-tube video on how to sleep after a TKR about 3 am on week 2 post op . It was such a GIFT! Thank you for making these lessons and tools so simple and sensible. I went to PT today and sat across a man who is scheduled for TKR in 2 weeks; my PT asked me if I had any words of wisdom? My response … ice and Dr. Rosen’s videos. 3 weeks pre-op, and that’s how I sleep now. I call it the 3 hour rule. Every 3 hours my knees wake me up by screaming at me.

You have helped me so much! Thank you, 3 weeks post op. I love listening to Niagara Falls for white noise or as a Christian I love listening to Soakstreams where scriptures can be recited over me as I sleep (on you tube)

Thank you dr. Rosen! I had TKR for about 5 weeks and am still struggling to sleep, I wake up with a lot of pain on my thigh. Icing did help.

4 weeks post surgery. Finally slept until 0300 am. Six decent hours of sleep is good for me. Woke up stiff but worked it out using my daily stretches. Awesome video and info! Thanks

Four weeks now and it's the opposite for me. I lay awake for hours trying to get comfortable. I go to bed at around 11.00pm and it's often around 3.00am before I nod off but once I do, I sleep till around 10.00.

Dr. Rosen, I am a 60 year old male TKR patient 2 weeks post op. I am at -2 degrees extension & 118 degrees flexion. I am walking unassisted, I do use my cane going up & down steps for safety. I love your Content especially your brutal honesty with what we will experience & what is expected of us after TKR. It is hard work & we have to push ourselves VERY hard through pain with these exercises to get our ROM back. I have had 2 broken collar bones, a compressed vertebra & bruised internal organs after a UTV accident, A heart attack last year & many kidney stone attacks one so big They had to go in & remove it and I have never had the pain of what I push myself through with these exercises. It is brutal, but I know I have to push through this pain to get my knee back with full ROM & as soon as possible. I see so many patients that were or are not willing to do & push Through these exercise sessions, 3 times a day, everyday. My best pain medication is my exercises & icing as much as possible. I do take 2 Tylenol 1 hour before my exercises. As you have stated in one of your video's, these exercises are our job until we have fully recovered. Between my exercise sessions, walking after each session about 400 ft. & a 2 hour recovery period, this takes up about 8 hours of my day. My physical therapist told me in here 30 years as a licensed therapist, she has only seen 1 other patient be where I am 2 weeks post-op & my Surgeon I seen yesterday was blown away. I am this far along because I do the work, It hurts like Hell yes, but you have to push through it. I increased my reps from 10 to 20 after the first week & push myself as far as I can mentally tolerate the pain. I just wish more people would do this. Thank you for all the great information & your experience you share with us here on your channel. It is very helpful.

I’m two weeks post op myself and am eagerly doing my exercises but dang, there was so much pain in my knee (level 10+ at times, esp at night) from just the operation that my doc recommended walking only and skip the exercises for a few days last week. How did you get your exercises done with the flat out pain in your knee? It was bad enough to get me crying at times.

It's not just for knees! I had a direct anterior approach hip replacement 2 weeks ago and have had great difficutly sleeping. I appreciate this so much. Ironic that I spent the hours before trying to sleep, staring at screens looking for answers to positioning, precautions, and sleep problems after surgery. Tonight I'll listen to audible. I had both together 4 weeks ago……I couldn’t have chosen between them. It has been so painful though, I know I wouldn’t have ever gone back for the other!

I found putting ice on my knee behind, to the side causes me more pain. I don't like the cold feeling. I am 2 weeks post op on TKR and I find the pain sets me off sobbing during the night. It has not helped zi got my first dose of Covid when I got home. Am doing my exercises with the help of a physiotherapist each day. Having incontinence does not help either, having to get up 2 or 3 times at night. They say it gets better....I look forward to that time, but I have never experienced such horrendous pain. I would gladly give birth to triplets naturally than go through this operation ever again. No, I am not feeling sorry for myself I just cannot believe the pain this operation shocked me into.

So glad I found you!! I’m lucky to get 3 hours in a row, 4 weeks post op. My surgeon is great, but not sure he’s aware of the magnitude of this issue. Exhausted and frustrated.

Thank you for the great videos and information, I just wanted to mention the safety reminder for those taking acetaminophen to monitor their total dose per day to prevent renal damage. I am a 65 year old hypermobile who just had total knee replacements on my knees and have found your videos helpful, mainly in reminding me to keep it simple stupid. Having had an extreme range of motion before surgery I was expecting to have to learn to do things over again with a more mechanically limited joint, but I liked how you put it in one of your other videos... a knee simply moves back and forth. So I am not trying to over think and am using your range of motion exercises and will push it hour by hour to find my new natural limits. Keep up the great work and keep your fingers crossed for me!

Not slept properly for years due to pain. 3 weeks post op, having much better sleep this past week 👍😁💤 Stiff by morning but it loosens once I’m up & about.

What I recently learned from another Doctors discussion video is that the pain killing drugs that are prescribed initially, actually contain Caffeine to counteract the drowsiness they induce! I wasn't in pain at night, I was just awake!

I feel like I’m lost, disturbed by what my doctor told me. I have “severe osteophytosis” I thought this only happened to older folks. I am 49 and Tuesday it will be 4 weeks I have gone through it. My issues are in my thighs and small endurance to pain. If you can start working on your legs and thigh muscles and also back muscles. It’s an easy surgery but the post op needs dedication

I got scheduled for Jan. 2022 only because my doctor is super booked and if anyone cancels I would like to have it sooner. So I’m in a cancellation list. Awaiting in L.A. CA. I went to a neurologist and diagnosed me as a neuropathic pain due to aggressive tourniquet application that affected the femoral nerve, and he prescribed Laryca and nerve conduction study

Thanks, I'm getting a Knee Replacement this Upcoming Friday. I'm 62, I had an operation on the Same Knee in 1976 for a chipped bone at the bottom of the Femur. I'm hoping that this old Chip doesn't interfere with the procedure and results. I have Tiger Balm and Icy Hot. I'll put some in my Back Pack I got something else that works too, called Oxy Rub. It helps the pain better that the Aforementioned one's. Although they all work well. I'm pretty sure that everything will go well with the procedure on Friday. Thanks for the good advice Doctor Rosen. I'm editing this, because I take Suboxone (since 2015). I've been tapering down. I'm not going to take any the Day and night before if I can do that? Because I want the Pain Medication to work!

You need to talk to your prescriber or a pain management professional about that. Not someone on you tube. Nothing to mess around with as you will have pain post op.

7 days post op, sleep is tough. The first 5 days I stayed in the recliner with feet elevated. Finally made it to thee bed the last 2 nites, but once I wake up in the am, I have to get up. Usually I'm a toss and turner, belly sleeper, but this lying on back is not fun. Do have close to 90 degrees flex, and major pain is gone but the drone of pain is still there. Thanks for the videos.

I had left TKR in 2017 I need to have my right knee done and I am putting it off because I am terrified. I went to a hospital in Boston where a lot of sports figures went I asked a nurse friend who used to work there to recommend which doctor she would have if she were me and I made an appointment with that doctor my experience was horrible in fact I went back for my stitches to be removed and because of this doctors attitude I never went back to him I found a different doctor to handle the rest of my care. I had no pain management I remember waking up in the night and looking up knee replacement from hell on my phone because I wanted to know other people were /had the same experience as I was going through. No one ever mentioned the spasms that you can get from the surgery in the knee that feels like your leg is being twisted I seriously still have PTSD from the whole experience.

Hang in there Ellen. I just had a total knee replacement this week. I am sleeping in my chair. As I am to tall for my couch. Can’t make it up 13 steps to my bedroom. But dealing with it. Going to try tonight. When I was in the hospital a few aides had attitudes and one doctor did. So I let them know nicely. I think they were so short of help like every where. They were really working there butts off. We all have bad days I understand. We are all human and have our braking points. Anyway glad you found a nicer attitude doctor. Mine is great. This dr. didn’t do my first replacement. I like this one better. My first one that did my first knee. He saw good also. This dr. I have now explained everything so much clearer than my first doctor did. The Dr. makes a difference. The problem I am having from my meds. I am sure. Is I cry for nothing. I never cry. Then my tongue I feel strings on it every where. I think from the meds. I had my husband look at it. He said looks like a togue, nothing on it. To funny. Meds. can do strange things to you. So honey just do your best everything will get better. Hope my little talk helped you.

Do you have any ideas to help with constant burning? I understand this comes from nerves. My surgeon says it can be present up to 1 year after surgery. I get up through the night to turn the cold shower onto my burning knee. I have good mobility but the burning!! Knee is really hot to the touch. Thank you

Hey Doc Great video!.. just had TKR 6 days ago and Im waking up at 4 am..the pain is from beyond...the Oxy 10/325 is doing nothing...my surgeon says it gets better...we shall see.

Oooh I want to try this! I also tend to pop awake at 4:00 am, always have. But it’s different when you can’t fall asleep as soon as your head hits the pillow. I’ve got in the habit of coming out to my living room when I wake up in those early hours. Put headphones on and listen to some storytelling. Sometimes I drift off back asleep and sometimes I’m just awake. What really really needs to happen is me falling asleep when I get in bed. I’ll lie awake for hours sometimes. Still can’t sleep on my side and sleeping on my back is quite uncomfortable. I’m sure this is temporary and everything will get better with time. It’s hard to be patient

Thank you your information is great. I just had a total knee replacement this week. Doing pretty good. But have to sleep in my chair. As I have a lot of steps I can’t go up. My couch is not long big enough for me. So not getting good sleep. In the hospital I could only do four steps. Going to try tonight to do my stamps. Pain is pretty good moving good. Had my first replacement 11 years ago. This time had more problems after surgery with meds. But doing much better, day 5 now. Staples are staring to pull. But still do the exercises. That I was told to do when I left the hospital. No PT at home this time. How things have changed. I think for the better. Thank you again dr. You people out there that are having a really hard one. Hang in there it will get better.

I've had 2 TKR's (latest one 7 wks. ago) and found gabapentin helpful with sleep.

I had TKR 2 weeks ago ,I had side effects from the tourniquet application and had hematoma on the whole leg and developed neuropathic pain. I couldn't sleep for one week, 24 hours my leg is hot as an oven.

I have severe Osteoarthritis and a crooked leg from this. Here is the problem I face at 63. The knee does not hurt when I sit when I sleep and can walk relatively pain free for 5 minutes. I can go up and down stairs with some pain. My surgeon wants to do TKR to straighten the bow leg but wondering if this is worth getting rid of a knee that seems rather functional. Cortisone shots help the knee occasionally swells up but all in all very manageable. So is this worth it?

I have always had issues staying asleep. I was prescribed Trazadone and now I have a really good schedule, even do the things like making my bed, never sitting or napping in bed. I was told that when you treat the bedroom as a sleep area and nothing else, then maybe your brain starts to set you up for falling asleep easier. My question is, will I still be taking Trazadone after surgery?

I’m having trouble sleeping as well but I’m recovering from an ACL surgery (16 days post op), is it really going to take 6 weeks to sleep normal again ? Or is that just for TKR patients

I’m 3 months out from my TKR and I still have a lot of pain and insomnia pain. It’s mentally weighing on me. I’ve been taking Tylenol 2000mg during the day (anything les doesn’t help). At night I take hydroxyzine but this has been giving me bruised feeling in my head in the mornings. I cry every night from the pain and not being able to sleep. I totally forgot about the Tylenol PM. I’ll try that this weekend to see if it helps.

Totally understand as I'm 2 months plus myself. Dr had to do a revision on knee 8 months after replacement so this has been a bummer all along. Jan 2021 was hip replacement April was knee replacement, December was revision all on same leg! Dr said it would take at least a year to recover from all of these 3 major surgeries! Talk about a kick in the gut! Hoping to get better before that length of time. Wishing you better luck as well.

That Tylenol PM works: On time at thee firehouse I had a terrible headache so I took some tylenol from my friends locker - didn't realize it was PM, I was half asleep the rest of the day fighting off a good nap.

All excellent advice. I do wish medical society were researching the use of CBD/THC to help prevent pain. It seems to help me focus elsewhere when the pain becomes hard to deal with. Scheduled for TKR in a couple of weeks, apprehensive and looking forward to it at the same time.

I’ve had both knees replaced and a shoulder replacement 3 weeks ago. I believe in my case using the narcotics at bedtime made it hard to sleep once I no longer needed any pain control meds. I felt like I had to learn to sleep on my own again. I have RA and am used to sleeping with pain so those days on narcotics brought some deep sleep. It’s an adjustment to go back to sleeping without pain control.

How did it go and how is marijuana treating your pain? Pkr here and I was on medical marijuana before the op. Pot helps some but nothing replaces opiates. Pot is great for inflammation, helps with nerve pain and is not that effective with acute pain (helps but it's like you feel the pain but don't mind as much). At this point I'm trying more natural things in my pain kit but such things have baggage as well.

after 13 months -there is absolutely no pain while walking ,but while sleeping a little discomfort but not terrible-getting the other knee done dec.23 -if I get same end result ,I will consider myself fortunate. With the first knee it was hell for about 12 weeks then it all fell into place -lots of ice with my machine and lots of pills -but no pain no gain-

Many thanks. One night I sleep then the next I’m awake all night. My surgeon does believe that afterward I need to get off celebrex. I feel better after taking it. Why is that?

I have developed insomnia after my gallbladder surgery. I wake up at 2-3 am evey night, not able to sleep again. Please help!!!

Doc oh man 2 weeks in and this the worse part of it so far. Now range of motion the bend is a problem. Got to start working harder on the range.

sleep hygiene is almost impossible du to SO much Pain!!! 3 weeks PO TKR. and i am a regular meditation person . I can't do any physio exercises due to stiffness and then more pain. i ice and heat pack all day and night and i am so having " buyers regret" !! definitely not going get the second one done

Hi Adam, thanks for your information. I'm 7 weeks post bilateral TKR's. I'm having trouble sleeping but it's mainly due to the fact that I've developed significant restless leg syndrome post operatively. It's not something I was particularly bothered by pre-op. I was fit and active pre-op....walking, boogie boarding and gym and had a pretty active job which involved a lot of walking.....any thoughts on this? Once the legs start up when I go to bed, I just cannot get to sleep. I'm normally a pretty good sleeper. Interestingly, I was talking to my brother who had his knees replaced several years ago. He also developed the restless legs post-op.....and still suffers from it to this day!!!

Every time I get up I need to pee, last night about 6 times , must have peed out far more than I drank. Where is it all coming from. Pre op maybe once a night.

Your body is taking care of the swelling (fluid in the tissues) and when resting your kidneys work even though you are not doing anything. Did you have a catheter during surgery? Could be bladder irritation.

Have a tkr,it won't get better,so sooner rather than later.l guarantee you'll be better than you are now.

Currently recovering from 5 hour surgery on my broken tibia had to get a plate n screws in 12 days later I'm getting an average of two hours a night they should absolutely address this I'm about to go insane

Thank you! I was shocked knowing how important sleep is for healing no info or talk was given Pre or post surgery. Also I had allergic reaction to clear bandage over my knee after 10 days took it off ( norm Dr. order) and noticed the crusty rash on sides of incision: I read to take Benadryl for few days and found it helped my sleep and helped rash not blister as badly as others; was a life saver incision after that was perfect. Im going to try a Tylenol pm tonight . I did buy magnesium oil spray to help calm me to sleep: sprayed on feet and wrists helped but found to do one or the other not wrists and feet at same time, because slight head foginess next day. Also took homeopathic sublingual tablets called: arnicare by Boiron for stiffness and pain kept at bedside if I woke up just took 2 : And used their gel around and under knee not on incision helped swelling a lot! Easy to grab if you wake up. Hope those tips helps others!
