Thank you so much for this video!!! I am 62 years old, I’ve been active and lifting weights for more than 40 years, so I was shocked to learn that I have moderate to high risk osteoporosis (no fractures). I live in Canada so support such as yours is not available to me. The FOOT plan was suggested to me. I follow the supplement recommendations along with 80 to 100 g of protein per day and a weight-bearing exercise program similar to the LIFTMORE study. Gut health was definitely a factor for me, it is now managed thanks to a wonderful naturopath. I appreciate your clarification of the FOOT Plan, super helpful for me as I figure my way through this silent disease. I feel like I’m on the right track, thanks in large part to you and Optimal Bone Health.

Agree good basic start but we all have unique medical profile and microbiome/biochemistry. No one plan fits all. My OP is from longterm pred and PPI use. Also pain meds. I’m working on tapering all but it’s complex. My drugs are common and some of hardest to taper. Many non MDs on YT say ‘treat the root cause.’ Not easy and not all can afford pills or alt health services. My specialist advises one change at a time to best monitor changes and manage them esp cases like mine. I’m glad an orthopod is finally providing advice and consults on OP. I’m in Canada but have a good team and self educate. Thx Dr Lucus.

Thank you! I would love to be able to treat patients in CA! For now I'll do my best to pass on my experience. You are correct in that prednisone and PPI are tough to get off of! There are medical conditions that require these drugs but not always. Working with your team to safely reduce dose or find alternatives is awesome! Through Pilates, I reversed osteoporosis in my back but not hips. I would like some good hip exercises. I take most of those supplements. Thank you so much for your suggestions and input.

Hi Nancy, Congrats on your progress! Spine and hips do respond to different activities. Look at our videos on exercise and vibration and you'll find that resistance training like that in the LIFTMORE trial and vib plates showed increases in hip BMD. Approach that type of resistance carefully and consider a trainer for oversight.

I've tried melatonin several times. It has the opposite effect on me. I'm wide awake for whole night. I do take all other suggestions you made, including exercise. Eat grass fed beef, salmon &eggs & no gluten. Diagnosed osteoporosis almost 2yrs ago. Good video. Ty

Thanks Mary, Melatonin does have that effect on some people. I have recommended for these patients to consider higher doses. I have some patients on 5, 20 and 100mg doses. Everyone's different and their metabolism of melatonin is unique.

Question.. what is your opinion on the FRAC scores. There is controversy about it.. my hospital uses this score. Also you said “seeing progress “.. ? Is there a way to measure progress other than FRAC or Dexa?

Hi Barb, Frax is a statistical tool. I use it. It is validated and provides some evidence of progress but it doesn't take into account many factors that I think are important. We use a Bone Health Score Card that incorporates things like vitamin D levels, B12/Folate levels. CTX/P1NP and many more markers that are much more dynamic. DEXA is an entirely different discussion that I write about at length in my upcoming book! It's another tool that provides data but should not be our only biomarker. Ultimately, we want to use biomarkers that help us understand dynamic progression and evidence of reversing the causes of bone loss not just what our bone density looks like.

Great news as always. My wife is athletic and has eaten perfectly for 20 years. She might be tad light on protein. Celiac buy is very careful and has good absorption by virtue of testing. High vit d levels. 58 years old. Had ovaries removed at 53 due to benign tumor. Last year testosterone dropped big. She had lowered dhea and that may be reason. Obviously estrogen non existent. Considering hrt but concerned with breast cancer risk. She has has some breast issues with spot they are tracking that’s unchanged in 4 years. But I do think if she was on estrogen or didn’t lose ovaries we would t be in this spot. Not going drug route. Too dangerous. She takes lots of supplements. Going to try the algaecal. Any other recs? Doctor I had stage0/stage1 breast cancer (7 yrs ago)and a lumpectomy. My breast doctor doesn’t want to give me estrógen or hrt because of my estrógen driven breast cancer. I have just been diagnosed with moderate osteopenia (tscore 2.0 and 2.1 for spine and hip). I exercise 3 times a week and walking more , also taking supplements (magnesium glycinate boron k2(mk7) omega 3 vit d (5000iu) vit c calcium - I’m confused bec I don’t want it to progress to osteoporosis but I’m not being recommended for estrógen at all. What do I do?

Hello, This is a tough problem we encounter often. Basically we recommend finding and utilizing everything else that you can in order to protect your bone. It can be done without estrogen but estrogen helps.

Have you ever researched a supplement called AdvaCal? My husband and I used to take it often and as I said in another, he had multiple myeloma. My husband never had a fracture during his entire ordeal with that cancer. I stopped taking up when the Fukushima nuclear reactor was destroyed because I believe their source of calcium is oyster shell and it's a Japanese company so I was a little bit concerned that that time. Supposed to have an different effect than General calcium because of the way it's made. Also, how do I know why I'm losing bone?

Hello, This product uses calcium salts which we don't recommend. They are absorbed poorly. However, it also has nice quantities of other micronutrients that are great for bone. Any benefit seen was more likely from the latter.

Hello dr.l go to a homeopath in ltaly for the osteoporosis.l take vit d, calcium, vit k2, and alot of minerals.for my diet my lovely dr advised me to eat lots of fruit and veg AND lots of fish and eggs and meat once max twice a week and lots of excersice.!!!! Oh he also gave me a med for stress.But l wish l could meet you.!!! Love from ltalia.!!!🥰

Hello, Very much depends on the individual and associated risks, duration of menopause and more. Sometimes we can use off label hormones such as testosterone and progesterone and for some even adding estrogen back but again risks have to be taken into consideration.

I already do yoga, body pump, walking 2+ miles every night… eat all my greens, take supplements… now I’m doubting myself on all of the above because I learned about le times, etc… is there a way to help fixate the calcium to my bones? I’m suspecting I need to improve the absorption as well. I did a complete … very complete blood panel and urinalysis couple of months ago and just need to consult with a specialist that will connect the dots and make recommendations… please help!

How are you going to monitor progress on bone health? I cant see the things and you can only get a DEXA every 2 years. Also...there are many healthy vegans with great bones..check loma linda population...so you are very biased toward your meat eating proclivity.

Hi there. I am biased, as we all are. I'm biased toward the research that I've read and my own experience with patients. @@Dr_DougLucas regardless of religious expectations...good bone in vegans is good bone. Unless you think ghat parameters for healthy bone change with religion. You have provided no research that shows meat builds stronger bones. Thus, you have interpreted everything based on your belief that this is so....because meat means something special to you. Does bone need amino acids...yes...must they come from meat...not a study has ever found that....provide one if you can.

@@betzib8021 Religious beliefs that effect behavior. True for 7th Day Adventists and LDS for which another study exists. I have presented data that shows higher fracture risk in vegans and vegetarians. That is adequate for me and my recommendations. I have always said that a vegan diet can be well designed and support bone and muscle health but it's very difficult and most that follow them don't get the adequate amino acids, minerals or other nutrients commonly found in meat products.
