Unohana didnt get defeated by kenpachi as a child though. When they fought and kenpachi stabbed her in the chest he supressed himself to enjoy the battle longer, thus weakening himself. Theres even a short scene in TYBW were Unohana and another shinigami were looking down om the defeated child kenpachi. She revived him because she found heraelf guilty if she killed kenpachi knowing at the time he would be the one to defeat her honourably in battle some day.

They were torn off during Zaraki's fight with Cien Granz and he never put them back in, also the eyepatch that he currently wears drains 10x more reiatsu than the one he used in the past. This was given to him at the end of SAFWY by Mayuri.

Because he has reiatsu on scale hitherto unfathomable. Never forget that when Yamamoto actually took the time to train the guy, simply teaching him to use both hands behind a simple cutting strike would amplify his power and that he still opted to fight with not one but two handicaps: his eye patch and using only one hand simply because otherwise it would be no fight at all but a straight up massacre. If he actually put his weight behind training from what we have seen during the Blood War, say, instead of what amounts to weeks thereabouts, he spent a century learning to not only control his reiatsu but also the ins and outs d outs of his zanpakuto, he’d probably be transcendent and likely be able to kill Aizen and the likes. The dude is BLEACH’s Gohan, he has on demand amps that can simply make him leap from barely factoring to outright the deciding factor.
