He seems snow extreme sadist. I don't think his kills were intended at the start. I think women who reacted to the pain satisfied his urge and got away. Women who passed out too often or took the pain without alot of Fuss died. He would have needed the visible, audible pain. Death is release. A truly evil sadist would want to avoid death unless the victim fucked up their power trip.
